Type A behavior pattern and mortality after recurrent myocardial infarction: preliminary results from a follow-up study of 5 years.

The purpose of the current study is to examine the association between the type A Behavior Pattern (TABP) and recurrent myocardial infarction (RMI). Rosenman's Structured Interview was administered to a consecutive series of patients admitted to the hospital for myocardial infarction (n = 88). Incidence and mortality from RMI in relation to TABP categories were evaluated after a follow-up period of 5 years. The number of new episodes of myocardial infarction observed in the extreme categories was nearly the same but the number of subjects who dies was nearly twice as large in the B as in the A1 category (10.3% vs. 5.8%, chi 2 = 9.074, p less than 0.0283). No subject was observed to survive after RMI in the B group. In agreement with other recent studies, our preliminary results failed to confirm the association between TABP and RMI, but showed a protection from death for subjects displaying high TABP levels. This finding is discussed in terms of the possibility for type A subjects to cope better with the acute illness.

[Tachycardia and ventricular fibrillation in the arrhythmogenic right ventricle (arrhythmogenic dysplasia of the right ventricle). Clinical and electrocardiographic spectrum].

In this paper we report the arrhythmias recorded on basal ECG, on Holter monitoring or on exercise test, in 32 pts affected by arrhythmogenic right ventricle (ARV). A sustained ventricular tachycardia (VT) was present in 11 pts a non sustained VT in 15 pts, a slow VT in 2 pts, a ventricular fibrillation (VF) in 3 pts and both sustained VT and VF in 1 pt. All but 1 case of sustained VT showed a LBBB like pattern. The heart rate during VT ranged between 170 and 280 beats/min. The frontal axis of the VT showed a wide range of deviation. Among non sustained VT, 9 cases had LBBB like pattern and 6 cases had polymorphic configuration. The 2 cases of slow VT showed LBBB like pattern with right axis deviation. A comparison between ventricular arrhythmias and RV impairment was made. The data obtained suggest that the effort plays an important role in the induction of VT in pts with localized RV impairment. In conclusion a wide spectrum of ventricular tachyarrhythmias is present in the ARV. Probably the RV "arrhythmogenic" zones and the electrophysiological mechanism causing the arrhythmias are various.

Life events and myocardial infarction.

Life events reported to have occurred twelve months before the onset of the illness were compared in 55 in-patients who had a first episode of myocardial infarction and 55 control in-patients matched for age, sex, marital status and social class and afflicted with acute abdomen, trauma and multiple trauma. The Paykel interview for Recent Life Events was used. Myocardial infarction patients reported significantly more previous events than the control group (p less than 0.001) with more undesirable (p less than 0.01) and uncontrolled (p less than 0.01) events. Moreover, myocardial infarction patients had significantly more events (p less than 0.01) which had an "objective negative impact" (rated as being traumatic). These findings are consistent with the view that certain recent life events have a positive association with the onset of a first episode of myocardial infarction.

[Evaluation of clinical data and effort test in patients with angina pectoris: an approach to a multivariate analysis in different groups].

One-hundred and eighty-nine patients who underwent cardiac catheterization for suspected ischemic heart disease were assigned to four different groups depending on the number of diseased vessels (0, 1, 2, 3), the dominant artery and the presence of left main coronary stenosis. Among the 45 parameters obtained from history, physical examination, E.C.G., and exercise test multivariate analysis identified (positive stress test, maximal systolic blood pressure, documented myocardial infarction, maximal heart rate, ipercholesterolemia, lateral myocardial infarction, angina during stress test) that were statistically significant. A score system was devised on the basis of the summed-up value of the seven parameters. When the patients were assigned, according to the score, to one of the previously mentioned 4 groups, the classification proved to be correct in 54% of the cases. When we divided the patients in two groups with or without coronary artery disease so that the attribution turned out to be correct in 92% of the cases (sensitivity 94%, specificity 77%), whereas when based on ST segment depression only, the classification was correct in 85% of the cases (sensitivity 84%, specificity 88%).

[The heart in myotonic dystrophy. Clinical and instrumental study in 17 patients (author's transl)].

17 patients with myotonic dystrophy have been studied. The skeletal muscle disease has been assessed by history, physical examination, electro-myography and muscle biopsy. Cardiac evaluation has been obtained by history, physical examination, chest X-ray, ecg at rest and after exercise, vectorcardiography and echocardiography. Only two patients presented symptoms and clinical findings of cardiac disease, otherwise conduction disturbances have been founded in 15 cases, pseudoinfarct pattern in 5 cases and loss of anterior vectors in 2 cases. Has to be remembered that in myotonic dystrophy sudden death has a very high incidence, which could be explained by the development of complete a-v block, because of the impairment of the conduction system. Therefore, in those patients with bifascicular blocks (3 cases), the electrophysiological study may be useful in order to evaluate the opportunity to put on a permanent pace-maker, to prevent sudden death.
