A comparison between the sciences of epidemiology and statistics based on an examination of epidemiological or statistical studies on diabetes in Japan.

The authors selected 24 original papers which were regarded them as the epidemiological study and the statistical study from their titles, from the end of World War II to 1981. And these papers were selected from 3 medical journals of internal medicine, other medical journals and proceedings of 2 International Conferences (see Table 1), and also were the object of study, namely, theoretical considerations. Besides we classified these 24 papers into 2 sorts; papers for an epidemiological study and a statistical study, and made a comparative study of details of these papers theoretically. As the result we were able to clarify what the authors of 24 papers had considered about the natures of epidemiology and statistics as the science. It was clarified that two sciences, epidemiology and statistics, had been in the general trend without any recognition of the differences between two. And as the conclusion we pointed out that the field of activity of statistics was broader than that of epidemiology, and the nature of statistics as the science might be changeable according to the object, moreover, statistical theory might be a branch of mathematics and so on.

A clinical study of long-term administration of dextran sulfate (MDS Kowa) in patients with retinopathy.

We administered MDS at a daily dose of 1,800 mg to 25 patients with diabetic retinopathy for periods ranging from 12 to 48 months (mean: 28 months) and achieved the following results: 1) During the administration period total cholesterol, triglyceride and fibrinogen levels were all maintained at significantly lower levels. 2) Eye fundus findings showed an improvement in 38.9% no change in 47. 2% and a deterioration in 13.9% of the cases. 3) No side-effects or abnormalities in clinical findings were observed in any of the cases even though clinical tests of hepatic function, renal function, blood chemistry, and urinalysis were conducted. The above results demonstrate that MDS has a marked effect on diabetic retinopathy, especially in cases with moderate to severe symptoms during the non-proliferative stage.

Clinical study of cardiac arrhythmias using a 24-hour continuous electrocardiographic recorder (5th report)--antiarrhythmic action of coenzyme Q10 in diabetics.

An investigation was undertaken to evaluate the antiarrhythmic effect of CoQ10 on VPBs using the Holter ECG, in 27 patients with no clinical findings of organic cardiopathies. As a result, the effect of CoQ10 on VPBs was considered beneficial in 6 (22%) of 27 cases, consisting of 1 patient with hypertension and 5 patients with DM. Even in the remaining 2 patients with DM, the frequency of VPBs was reduced by 50% or more during treatment with CoQ10. The mean reduction of VPBs frequency in the 5 responders plus these 2 patients with DM was 85.7%. These findings suggest that CoQ10 exhibits an effective antiarrhythmic action not merely on organic heart disease but also on VPBs supervening on DM.

Incidence of hyperlipemia in diabetics in Okinawa and its relation to ischemic heart disease.

The average value for total cholesterol in the healthy population of Okinawa was significantly higher than that of Kyushu, but there were no differences in triglyceride levels. It is presumed that the difference in cholesterol levels was due to a 10 g difference in fat intake. The average values for total cholesterol and triglycerides in diabetics before treatment in both Kyushu and Okinawa were higher than in the healthy populations of Kyushu and Okinawa. The average values for total cholesterol and triglycerides in the diabetics in Okinawa were higher than those in the diabetics of Kyushu. The incidence of ischemic heart disease in diabetics in Okinawa classified by the Minnesota Code was higher than in the healthy population. It can be concluded that the development of ischemic heart disease in diabetics is influenced by both hypercholesterolemia and other metabolic disorders due to insufficiency of insulin action.

The manifestation of genotypes responsible for blood pressure and blood sugar level.

It has been known for a long time that hypertension and diabetes mellitus are often complicated by each other. The aim of this paper was to disclose possible causes of such complications by investigating 1) whether or not hypertension and diabetes mellitus are entirely two independent diseases, 2) whether or not they occur together only by chance, 3) whether or not there are some causal factors which are shared by these two diseases and 4) whether or not these two diseases occur together with some probability. The genetic analyses of these two diseases were carried out on the basis of the genetic analyses on blood pressures (BP) and blood sugar levels (BS) in the present studies. It was proved that the genotypic correlation coefficients of BP and BS ranged from 0.075 to 0.573 with a mean value of 0.337 and their environmental correlation coefficients were from -0.029 to -0.337 with a mean value of -0.164. These results suggest that 1) the respective polygenic systems responsible for BP (hypertension) and BS (diabetes mellitus) have multiple effects and 2) the environmental factors responsible for BP and BS have inverse effects. The negative values of the environmental correlation coefficients between BP and BS indicate that the environmental factors which lower BP may elevate BS, and vice versa. An induction of an abnormal glucose tolerance as one of the side effects of thiazides, the most widely used anti-hypertensive diuretics, coincides with the inverse environmental correlation between BP and BS. This finding may be of great importance in the field of clinical genetics.

Genetic control of fatty acid metabolism, especially study of genetic control of cholesterol.

The serum cholesterol of 1431 inhabitants over 10 years old in the general population was estimated by the method of Autoanalyzer. The correlation coefficients of the serum total cholesterol between parent and child and between siblings were obtained from other samples which completely excluded consanguinity. The genetic study of 41 pedigrees of familial hypercholesterolemia with xanthoma which described the cholesterol value of parent and child in Japan and other countries was done, and the following results were obtained. 1) Cholesterol is a quantitative character, therefore it will be controlled genetically by polygene. 2) It was presumed that the mode of inheritance of type II hypercholesterolemia (with or without xanthoma) was also controlled by polygene. 3) The correlation coefficients of cholesterol between parent and child and between siblings were found to be 0.26004, 0.27998, respectively. 4) The heritability of cholesterol was found to be 0.5995.
