[Neuroma of the cystic nerve stump. No explanation for post-cholecystectomy pain].

The investigation of 20 clinical patients and the autopsy material of 15 cases showed that neuromas of the cystic duct stump following cholecystectomy represent a regular pathomorphologic condition and have no correlation with any postoperative symptoms. Identical morphologic alterations can also be found on arterial stumps after resection of abdominal organs. Therefore, we suggest eliminating the cystic duct neuroma from the list of possible causes of symptoms after cholecystectomy.

[Synovial cysts of the hip joint].

Four cases of a ganglion of the hip joint are reported. Besides the symptomatology and the clinical findings, special attention is payed to the anatomical relationship. The weak part of the joint capsula medially to the longitudinal traction of the ligamentum iliofemorale and laterally to the ligamentum pubofemorale was found to be the source of these ganglions. The most helpful diagnostic measure is arthrography through a lateral access. Therapy consists of resection of the cystic tumor, and careful closure of the hip joint capsula will prevent recurrence.

[Carcinoma in Zenker's diverticulum (author's transl)].

Carcinoma arising in Zenker's diverticulum have to be thought of, if it has been present for long time, if there is blood mixed the food regurgitated, and if there are irregularities of the wall of the diverticulum to be detected radiologically. In most cases resection is the method of choice. A case of a 70 year old patient is reported, who harbored a microcarcinoma in the mucosa of a resected Zenker's diverticulum. Surgery was done 10 years ago and no recurrence or mestastases have been observed since.

[Cytobiopsy as the best present method of intraoperative diagnosis of tumors of the pancreas].

A report has been made over six years experience with a fine needle aspiration biopsy, from authors who have developed a cytological intraoperative quick diagnosis of pancreas tumours. 116 patients have been treated up to this time. This method has proved to be important for the diagnosis of small pancreas tumours, while especially in such cases the indication to pancreas resection is used only in case of malignant tumours. In relation to other normal histological freezer microtome, the cytological method of fine needle biopsy has the advantage of being completely without complication. One can make as many punctures as needed and exactly this is what assues accurate diagnosis. In 90 cases with certain diagnosis, 65 correct conclusions and 12 suspect conclusions of tumour were obtained, and only five cases were erroneously judged negative; in eight cases the poor material did not allow a diagnosis. Therefore, wide application of cytobiopsy can be recommended.

Occurrence of alpha-1-fetoprotein in the mammary gland tissue of men with gynecomastia.

Immunohistochemical proof of A1FP production in the mammary gland of a man with gynecomastia is published here for the first time. The immunohistochemical proof of this fetal alpha-1-globulin in the intraductal epithelium of the mammary gland is described on the basis of the case history of a 25-year-old man with a metastasizing malignant teratoblastoma of the gonads. The question, to what extent A1FP-neosynthesis is characteristic for malignant tumors or whether it is a cellular response to specific, in this case hormonal growth stiumli of primary tumors, is also discussed.

[Adrenal-like Leydig cells (author's transl)].

Adrenal-like lipoid-rich Leydig cells, which could be found in a cryptorchid testis, were investigated by light and electronmicroscopy. There were nodular and diffuse proliferation of these adrenal-like cells in the interstitium of the testis. Electronmicroscopically these cells are fasciculated and characterized by large liposomes, many tubulovesicular mitochondria, and a large smooth endoplasmatic reticulum. But the presence of crystals of Reinke in these cells underlined their relationship to Leydig cells. The clinical history of this case is characterised by an extreme adipositas (167 kg) and high urinary estrogenexcretion. This excretion could be suppressed with dexamethasone and stimulated with HCG. After orchiectomy estrogen excretion decreased for 4 months and then increased again, after ACTH stimulation performed by reason of adrenal insufficiency. At this time there is no evidence of adrenal tumor; in the contralateral, scrotal testis, spermiogenesis and Leydig cells are without pathologic changes as revealed by biopsy.

[Primary malignant melanoblastoma of the esophagus (author's transl)].

Clinical and morphological aspects based on the observation of one case of oesophageal melanoblastoma are discussed. We want to point out that the characteristic morphology of the tumor enables us to exclude melanoma metastases and thus give an exact diagnosis. The importance of oesophagoscopy for the preoperative diagnosis is emphasized; radical resection is recommended for therapy.
