Cytophotometric investigations of DNA-content in transitional cell tumors of the bladder. Comparison of results with clinical follow up.

Forty-nine patients with transitional cell tumors of the urinary bladder were followed up clinically for a period of at least 36 months (average time 61.4 months). Initial tumors were examined by means of DNA-Feulgencytophotometry. Histologic grading was performed according to Bergkvist et al. (1965). Clinical behavior clearly demonstrates the validity of the grading schedule used and shows the significance of DNA-Feulgencytophotometry for the standardization of bladder tumors and for the comparability of diagnostic and therapeutic results. Determination of the individual prognosis of low-malignancy tumors and the differentiation of histologic borderline cases does not appear to be possible by means of DNA-Feulgencytophotometry.

Leydig cell tumor of the testis. Morphological and endocrinological investigations in two cases.

Clinical, morphological and endocrine characteristics of 2 patients with Leydig cell tumours (1 benign unilateral and 1 malignant bilateral) are reported. The morphological investigation revealed that the neoplastic cells had the same ultrastructural characteristics as normal Leydig cells. The only ultrastructural difference was the presence of 'membranous whorls' that have never been found in normal human Leydig cells. The patient with benign unilateral Leydig cell tumour appeared to produce no excess of steroid hormones, but rather a substance of FSH-inhibiting activity. The second patient, with the malignant bilateral tumours however, besides producing large amounts of testosterone, oestradiol-17 beta and progesterone, showed typical signs of an ectopic ACTH syndrome. The endocrinological and morphological features are discussed.

[Generalized ceroid histiocytosis in type 5 hyperlipoproteinaemia (author's transl)].

Histochemical, fluorescence and electron microscopic features of histiocytes from the spleen, lymph nodes and liver were investigated in a patient with type 5 hyperlipoproteinaemia with splenomegaly (Fredrickson and Lees, 1965). The pigment stored in histocytes represented ceroid, a product of oxidation and polymerization of unsaturated lipids. The morphological features of ceroid and histogenesis of ceroid histiocytosis are discussed.

Sclerosing angioma of the lung. Case report and electron microscope investigation.

Electron microscopy of a sclerosing angioma of the lung, a coincidental finding in the upper lobe of a 32-year-old woman. The rare, benign tumor, whose vascular proliferation by light microscopy is reminiscent of an angioma, exhibits a clear epithelial structure by electron microscopy. The tumor may develop out of immature pneumocytes. The paper discusses histogenesis and problems of differential diagnosis (potential confusion with carcinomas).

[Papillomatous changes in the cervix uteri].

A case of condyloma acuminatum of the uterine cervix and a case of papilloma on the cervix with concomitant adenocarcinoma of the cervix are described. The diagnostic biopsy revealed only the papillomatous lesion, whereas the adenocarcinoma was diagnosed in the surgical specimen after hysterectomy. Referring to the studies of Meisels et al. on condylomatous lesions of the cervix, the possible frequent incidence of this cervical lesion and its role as one possible first step in the pathogenesis of cervical cancer are discussed. The in toto excision is recommended because of diagnostic and therapeutic reasons. Clinical, colposcopic and cytologic check-up has to be done carefully for years after primary treatment.

[Plasma cell granuloma of the lung (author's transl)].

Case-report of a plasma cell granuloma of the lung found in a girl aged 8 years 6 months. The lesion of the lung was first interpreted as a metastasis of a suspected primary tumor in the right calf. It was only later that plasmacell granuloma of the lung was diagnosed. Plasma cell granuloma of the lung often gives rise to confusion, a fact illustrated by the various names applied to it, e.g. pseudosarcoma, postinflammatory pseudotumor of the lung, histiocytoma, xanthoma, fibroxanthoma, xanthogranuloma. On histological examination, this tumor consists of proliferations mainly of plasmacells and reticulo-endothelial cells, with some polymorphism and some giant cells. The tumor appears as a single, sharply circumscribed mass, is usually asymptomatic and therefore found mostly on routine-chest-films. Rarely is it found as an obstructive mass in the bronchi or in the trachea. The histopathogenesis of the tumor is not known, the lesion benign. Treatment is resection.

Quantification of human sperm morphology and motility by means of semi-automatic image analysis systems.

Sperm morphology and motility are determined using a semi-automatic image analysis system under opto-manual control. The system consists of a microscope equipped with a drawing-tube, a digitizer-tablet, and a cursor interfaced to a small microcomputer. A light-emitting diode is mounted on the cursor, visible as a bright red pilot-light in the microscopic field. Sperm head and midpiece as well as the pathway of motile spermatozoa are traced with the cursor's red pilot light. The microcomputer calculates area, circumference, and length simultaneously. Motility and sperm density are determined altogether using microchamber technique. In a selected group of male subjects with normal, doubtful, and pathological semen analyses biometrical analyses of sperm morphology and motility were performed. Sperm morphology is best described by the variation of the head size and by the average midpiece width. Motility is best described by % motile spermatozoa and mean velocity determined 1 hour after semen collection. Biometrical semen analyses are superior to subjective evaluations regarding degree of information, objectivity, and reproducibility.

[Long-term observations on use-effectiveness of copper-IUD's (author's transl)].

Long-term observation of the use-effectiveness and safety of 745 Copper-IUD's has demonstrated: 1) The pregnancy rate decreases distinctly after 2 years; until now no pregnancy has been observed after 4 years continuous use. 2) The endometrium responds first with an inflammatory reaction, later it shows signs of deficient secretion, but after a longer period seems to become normal again. 3) Contamination of IUD's begins early, mostly with bacteria of vaginal origin, later the number of sterile IUD's increases. -- A septic abortion after pregnancy with an IUD in situ has not occurred amongst our cases. 4) The rate of copper elution continues steadily, but in spite of demonstrable corrosion there is no obse-vable decrease of contraceptive effectiveness. Copper-IUD's can remain as long as 5 years in situ, if no untoward side effects (pain, bleeding, infection) occur. It may be advisable to remove them after that period as corrosion may affect devices at present commercially available.

[The so-called cardiac myxoma. Histological, electron microscopical, immunofluorescence, and biochemical investigations in 3 cases (author's transl)].

Three cardiac Myxomas were studied by light-, immuno-fluorescence and electron microscopy. Acid mucopolysaccharides of the tumour were isolated chromatographically. The surfaces of the "myxomas" were papillary in two cases and smooth in one. The smooth surfaced tumour recurred six months after primary resection; histological specimens were more cellular than those of the papillary tumors and showed nuclear polymorphism and mitoses. The cells of two papillary tumours were identified as endocardial by electron microscopy, however, the cells of the smooth surfaced tumour were typical myofibroblasts. In both types of "myxomas" the cells were rich on cytoplasmic filament that contained acto-myosin. Biochemical investigation failed to reveal any difference between the acid mucopolysaccharide pattern of the two tumour types; isolated mucopolysaccharides being typical for embryonal mesenchymal tissue. The authors agree with Albertini (1963), that "cardiac myxoma" is a misnomer for two different typical endocardial tumours: a true endothelioma and a "special fibroma" (myofibroma or myofibrosarcoma).

[Humoral and cellular immune mechanisms in venous occlusive lung diseases].

In a 73-year-old female patient, pulmonary veno-occlusive disease was diagnosed by the typical histological findings in the lung. Immunofluorescence studies showed granular deposits of IgG and C1q in the lung, suggesting immune complex deposition and complement activation by the classical pathway. Furthermore, the presence of lymphocytes sensitized against collagen was demonstrated using the leukocyte migration inhibition system. These results demonstrate the significance of humoral and cellular immune mechanisms in the pathogenesis of this disease.

[Diagnostic and clinical features of atrial myxoma (author's transl)].

The most common benign cardiac tumours are artial myxomas. Characteristically they may interfere with the normal functioning of atrioventricular valves or manifest themselves by embolism to cerebral or peripheral arteries or they may mimic systemic diseases. Hence, early diagnosis and immediate surgical treatment are important. A case of left atrial myxoma is reported and the various non-invasive and invasive diagnositc methods and their significance are outlined. If the angiocardiographic findings are questionable, the echocardiographic features are believed to be of greater reliability. Finally, the histopathology of atrial myxoma, the rate of tumour growth, as well as the possibility of postoperative reappearance of the myxoma are discussed.

[A contribution to the origin of ganglia. A study of light-, immunofluorescence-, and electronmicroscopic changes (author's transl)].

10 ganglia were studied by means of light and electrone microscope and with immunfluorescence methods. The light microscopic findings in the wall of a ganglion resemble on developing hygroma. The mesenchymal cells ultrastructurally show differentiation to synovia-like cells, either surrounding the ganglion in the connective tissue wall or lining the cavity. The development of synovial tissue from undifferentiated mesenchymal cells is due to the typical mechanical stimulus affecting joints.
