["Cold" nodules of the thyroid gland. (Clinical statistical study)].

A series of 718 scintigraphically cold thyroid nodules observed between 1965 and 1976 at the Turin University Endocrine Surgery Centre is presented. This form was more common in women (80%) and mainly occurred in the 3rd and 5th decade of life. Malignancy was present in 88 cases (12.27%): 49 papilliferous tumours, 25 follicular, 9 Hurthle-cell, and 5 anaplastic. A reference is expressed for unilateral lobectomy followed by suppressive management as opposed to total thyroidectomy since malignancy is low in forms where a tumour has a single site in a cold nodule. Furthermore, this approach spares the patient from the severe consequences of inevitable post-operative hypothyroidism. Total resection is, however, mandatory in anaplastic forms. A detailed account is offered of the typical echotomograms associated with individual pathological situations. These are enough to support a preoperative diagnosis of malignancy.

[Murine hematopoietic stem cells after acute and chronic administration of vincristine].

The effects of vincristine on bone marrow hemopoietic stem cells were studied in mice. After the administration of a single dose of the drug there is a sharp decline in the content of CFUs. When the drug is given in small, weekly doses for 16 weeks, after an initial drop, the stem cell content rises at a normal level where it remains, although with wide oscillations. On the whole the effects of vincristine on b.m. CFUs are comparable with that observed after vinblastine administration.

Short heparin thrombin clotting time, increased fibrinogen and platelet aggregates count in coronary disease: a probable hypercoagulable state.

Several indices of plasmatic and platelet coagulability (H.T.C.T., AT III, fibrinogen and Wu-Hoak test) were studied in 82 patients with acute and chronic coronary artery disease. The results were compared with those obtained in a control group. The most interesting result is the consistent shortening of H.T.C.T. as compared to the control group, particularly in patients with acute myocardial infarction. H.T.C.T. was always inversely correlated with the fibrinogen level. Those data suggest an important influence of fibrinogen levels on H.T.C.T., but this observation does not rule out the possibility that the heparin neutralizing activity (PF 4) will also influence the test. No positive correlation between H.T.C.T. and AT III could be observed. The elevated levels of AT III in acute myocardial infarction did not confirm the existence of a consumption of AT III due to chronic intravascular coagulation in these patients. The Wu-Hoak test increased only in patients with acute coronary disease, never in the other groups. In conclusion, H.T.C.T. may be assumed to be a valid test for indicating the existence of a possible hypercoagulability state.

The effects of chronic administration of cyclophosphamide on haemopoietic stem cells.

The effects of cyclophosphamide given for 18 weeks in small daily doses on CFU-S, CFU-C, and CFU-E content in mice bone marrow and spleen were studied. In the bone marrow, after an initial drop, the stem cell content rises above the normal values and remains there throughout a long period. In the spleen the stem cell content shows an initial several fold increase followed by a steady decline. It seems possible that this behaviour reflects a compensative effect to the toxic action of the drug at more differentiated levels of the haemopoietic cells.
