Computed tomography in the diagnosis of prolapsed intervertebral disc.

In thirty-nine out of forty-one patients with radicular pain in the lumbar region, prolapse of the intervertebral disc could be diagnosed by computed tomography without intrathecal administration of contrast medium. In one patient a paravertebral tumour (malignant neurilemmoma) was found. One case could not be diagnosed by computed tomography. Myelographic findings in the patients corresponded with those of computed tomography, and the findings in CT scans made immediately after myelography. The technical requirements for non-invasive CT of the intervertebral disc are discussed.

[Aphthous ulcers--an early radiological sign of Crohn's disease? (author's transl)].

Thirty-eight patients with histologically confirmed Crohn's disease are described. Eleven of these showed so-called aphthous ulcers. These were most common in the colon (five patients) and terminal ileum (four patients). Aphthous ulcers are small mucous membrane erosions on the surface of hypertrophic lymph follicles with surrounding oedema. From our material and from the literature we conclude that aphthous ulcers are occasional radiological signs and not invariable early signs of Crohn's disease. Their specificity has not been proved. Differentiation from gastric erosions in cases of gastro-duodenal involvement is discussed.

[Marginal osteophytes as a radiological and clinical indicator of patellofemoral arthrosis (author's transl)].

Radiological and pathological comparisons carried out on patellae at autopsy have shown a statistical relationship between the size of marginal patellar osteophytes and radiologically invisible ulceration of the joint cartilage. Patellar osteophytes greater than 2 mm. show a 100% association with cartilage ulcers. The detritus originating from the ulcerated cartilage is a potent irritant of the synovial membrane. The radiological demonstration of marginal patellar osteophytes therefore provides evidence concerning the severity of the arthrosis; the larger the patellar marginal osteophytes, the greater is the need for treatment of the painful arthrosis. The lateral and upper margins of the patella are the sites of predelection for the osteophytes. The medial patellar facet is the site of predelection for the cartilagenous ulcers.

Exposure of an antigen of chromaffin granules on cell surface during exocytosis.

The synthesis rate of the membrane proteins of the catecholamine-storing vesicles (chromaffin granules) of the adrenal medulla is lower than that of the secretory proteins of the contents. Based on these results we proposed that after exocytosis the membranes of chromaffin granules are retrieved and are re-used for several secretion cycles (see also ref. 4). This concept of re-use of granule membranes has been further strengthened by the finding that exogenous markers which are taken up by secretory cells during stimulation can be traced to the Golgi region and to immature secretory organelles. However, one basic question remains: are the membranes of secretory organelles specifically and completely removed from the plasma membrane and if so, how fast is this process? By using an antiserum against a membrane glycoprotein of chromaffin granules we have now obtained quantitative data which demonstrate that during exocytosis this antigen becomes exposed on the cell surface and disappears again to a large degree within 30 min.

[Manifestation of rheumatic diseases of the lung. Roentgenologic changes].

This paper first tries to relate the four types of immuno-pathologic reactions to certain basis-reactions of the lung. Subsequently, radiologic patterns will be correlated to these basis-reactions and will be further related to certain rheumatic diseases. It is shown that these basis-reactions cannot be clinically and radiologically distinguished from one another in all cases, and that in most cases there are combinations of several types of reaction to be found. The radiological patterns in the most important rheumatic diseases are demonstrated in an immunological arrangement. The possibilities of conventional radiological techniques and modern imaging techniques are described.

[Early radiologic diagnosis of coxarthrosis. Relation between the cartilage lesion and femur head osteophytosis (including the so-called plaque sign)].

Radiological and pathological comparisons on specimen of femurhead and neck at autopsy have shown a statistical relationship between osteophytosis of the femoral head and ulcerations of the joint cartilage. Especially there are highly significant relationships between the length of osteophytes and the diameter of the ulcera. The "plaque"-sign shows to be a very sensitive indicator of early arthrosis. So there exist semiquantitative parameters for the X-ray diagnosis of early coxarthrosis.
