Prospective study of exposure to environmental tobacco smoke and dysmenorrhea.

Dysmenorrhea is a common gynecologic disorder in women of reproductive age. Previous studies have found an association between current cigarette smoking and prevalence of dysmenorrhea. This study investigated the association between exposure to environmental tobacco smoke (ETS) and the occurrence of dysmenorrhea among women without a history of this disorder. The study population consisted of 165 newly wed, nonsmoking Chinese women (in Shenyang, China), who intended to get pregnant and who had no past history of dysmenorrhea at the time of enrollment. These women completed a baseline questionnaire interview upon enrollment and were prospectively followed by daily diary. Dysmenorrhea was defined as a diary recording of abdominal pain or low back pain for at least 2 days during a menstrual period. A subject's ETS exposure was defined as the mean number of cigarettes smoked per day at home by household members over an entire menstrual cycle before the menstrual period. A logistic regression model was used to assess the effect of ETS on the risk of dysmenorrhea, with adjustment for age, body mass index, education, season, area of residence, occupation, shift work, perceived stress, passive smoking at work, and occupational exposure to chemical hazards, dust, and noise. Generalized estimating equations were used to account for autocorrelations as a result of multiple cycles per subject. This report is based on 625 prospectively followed menstrual cycles with complete baseline and diary data. ETS exposure was reported in 77% of cycles, within which average daily exposures throughout the cycle ranged from 0.02 to 10. 3 cigarettes. The incidence of dysmenorrhea was 9.7% and 13.3% among nonexposed and exposed cycles, respectively. Among ETS-exposed cycles, there was a positive dose-response relationship between the numbers of cigarettes smoked and the relative risk of dysmenorrhea. The adjusted odds ratios of dysmenorrhea associated with "low," "middle," and "high" tertiles of ETS exposure versus no exposure were 1.1 [95% confidence interval (CI), 0.5-2.6], 2.5 (CI, 0.9-6.7), and 3.1 (CI, 1.2-8.3), respectively. The findings were consistent with those of analyses limited to the first follow-up menstrual cycle from each woman. These data suggest a significant dose-response relationship between exposure to ETS and an increased incidence of dysmenorrhea in this cohort of young women.

Geographic variations in crash risk of general aviation and air taxis.

INTRODUCTION: In the past decade, numerous studies have been published regarding the relationship of crash risk and mortality with respect to geographic area. METHODS: We analyzed data (United States only) regarding general aviation and air taxi crashes from 1992-94 from the National Transportation Safety Board. Data regarding 1993 flight hours and standard error were obtained from the Federal Aviation Administration (FAA) and pooled to provide a reliable estimate of crash risk for 1992-94. Calculations were determined for each state and FAA region. RESULTS: During the 3-yr period, the calculated United States crash rate is 8.9 crashes per 100,000 flight hours. The Alaskan and Northwest Mountain regions had the highest crash rates and fatal crash rates. DISCUSSION: This is the first study to report on geographical differences in rates determined as crashes per 100,000 pilot hours. It shows that even when the amount of flying is controlled for, crash rates and fatal crash rates are highest in mountainous regions. Our results indicate that aviation safety in mountainous regions deserves more attention.

Child abuse and unintentional injuries: a 10-year retrospective.

OBJECTIVE: To identify differences between hospitalized children injured by child abuse and those with unintentional injuries. DESIGN: Comparative analysis of patients injured by child abuse (n = 1997) with patients injured unintentionally (n = 16 831), newborn to 4 years of age. MAIN OUTCOME MEASURES: Patient characteristics, nature and severity of injury, treatment, length of stay, survival, functional limitations, and disposition at discharge from the hospital. DATA SOURCE: Retrospective review of medical records submitted to the National Pediatric Trauma Registry between January 1, 1988, and December 31, 1997. RESULTS: During the 10-year study period, child abuse accounted for 10.6% of all blunt trauma to patients younger than 5 years. Children injured by child abuse were significantly younger (mean, 12.8 vs 25.5 months) and were more likely to have preinjury medical history (53% vs 14.1%) and retinal hemorrhages (27.8% vs 0.06%) than children with unintentional injuries. Abused children were mainly injured by battering (53%) and by shaking (10.3%); unintentionally injured children were hurt mainly by falls (58.4%) and by motor vehicle-related events (37.1%). Abused children were more likely than unintentionally injured children to sustain intracranial injury (42.2% vs 14.1%) and thoracic (12.5% vs 4.5%) and abdominal (11.4% vs 6.8%) injuries; to sustain very severe injuries (22.6% vs 6.3%); to be admitted to the intensive care unit (42.5% vs 26.9%); and to receive Child Protective Services (82.3% vs 8%) and Social Services (72.9% vs 27.6%) intervention. The mean length of stay for children who were abused was significantly longer (9.3 vs 3.8 days) and the survival to discharge from the hospital was significantly worse (87.3% vs 97.4%) than for those unintentionally injured. Among the survivors, children who were abused developed extensive functional limitations more frequently than those unintentionally injured (8.7% vs 2.7%). More than half (56.6%) of the children who were abused were discharged to custodial/foster/Child Protective Services care; most (96.1%) of the children unintentionally injured returned to their homes. CONCLUSIONS: Child abuse continues to be a serious cause of mortality and morbidity to infants and toddlers. On average, among children hospitalized for blunt trauma, those injured by abuse sustain more severe injuries, use more medical services, and have worse survival and functional outcome than children with unintentional injuries.

[A new potential promoter region of the human connexin 26 gene].

OBJECTIVE: To explain regulatory mechanism of the human connexin 26(Cx26) gene. METHODS: A DNase-1 hypersensitive 1.6 kb fragment upstream of the 5'-terminus of the Cx26 gene was sequenced and assayed with CAT reporter system. RESULTS: The Cx26-1.6 kb sequence serves as a powerful promoter. CONCLUSION: The Cx26-1.6 kb fragment contains two GT boxes (centering at-6158 and -6213 bp), and a TATA-less TTAAAA box (-6237/-6232 bp) which is another promoter region of the human Cx26.

Human herpesvirus 6 in hematologic diseases in China.

BACKGROUND AND OBJECTIVE: The prevalence and pathogenic role of human herpesvirus 6 (HHV-6) in various benign and malignant hematologic diseases remain largely unknown. The aim of this study was to search for a possible involvement of HHV-6 in the pathogenesis of hematologic diseases. DESIGN AND METHODS: The presence of HHV-6 DNA sequences was examined by polymerase chain reaction (PCR) in bone marrow mononuclear cells from 241 patients with benign and malignant hematologic diseases in China. Platelet-associated immunoglobulin (PAIg) of 66 idiopathic thrombocytopenic purpura (ITP) patients was measured by competitive enzyme-linked immunosorbent assay. The presence of HHV-6 DNA in sera from 31 ITP patients was examined by PCR. Paired serum samples from 19 ITP patients were analyzed for anti-HHV-6 IgG titers using an indirect immunofluorescence assay. RESULTS: HHV-6 DNA was detected in 41% and 37.5% of ITP and acute leukemia patients respectively, but in only 6.7% of patients with iron deficiency anemia. HHV-6 positivity for ITP patients with excessive PAIgG was significantly higher than in patients with a normal level of PAIgG. HHV-6 DNA was not detected in any of the serum samples from ITP patients. None of the 19 cases of ITP showed a significant increase in anti-HHV-6 antibody titers during the convalescent phase compared with the onset phase. INTERPRETATION AND CONCLUSIONS: Our results indicate that HHV-6 infection might be associated with excessive PAIgG in some cases of ITP, and that the virus persists in a latent state. The pathogenic role of HHV-6 in ITP needs to be confirmed by further investigations.

Detailed allelic loss mapping on 7q32 in nasopharyngeal carcinoma.

OBJECTIVE: The present research group had found high frequency of loss of heterozygosity(LOH) of D7S500-D7S495 on chromosome band 7q32 in nasopharyngeal carcinoma, this study was conducted to further find the deletion region. METHODS: Higher density of loci on 7q32 in 30 tumors was studied by using microsatellite analysis. RESULTS: The frequency of LOH was 63.3%. The highest frequency of LOH was identified at the loci of D7S500-D7S509-D7S495, among which a common deletion of D7S509 was found. CONCLUSION: A putative tumor suppressor gene may be present around D7S509.

[Molecular cloning of a novel gene located on chromosome 3p25.3 and an analysis of its expression in nasopharyngeal carcinoma].

OBJECTIVE: To obtain the novel genes associated with human nasopharyngeal carcinoma(NPC) on chromosome 3p24-26. METHODS: Twenty epithelial-derived expressed sequence tags(EST) were selected from chromosome 3p24-26 where loss of heterozygosity(LOH) frequently occurs in NPC tissues. Primers were designed based on the sequences of these ESTs. RT-PCR was used to amply their corresponding cDNA fragments from NPC cell line HNE1 and primary cultures of normal nasopharyngeal epithelial cells. The differential expression of two ESTs, T93093 and R41598, was confirmed by Northern blot. Then, expression of EST T93093 was further detected in 7 normal nasopharyngeal and 19 NPC biopsies. cDNA library screening was used to get its full cDNA sequence and the sequence of this novel gene was analyzed by bioinformatics. RESULTS: Thirteen ESTs(T62511, N39155, N68660, R61275, T95314, R06143, H52697, H66521, AA128685, AA284537, N52379, AA054180, and H98090) showed the similar expression level and 5 ESTs(R00732, R07573, R98052, H91759, H17566) showed no expression in both types of cells. EST T93093 was down-expressed, whereas EST R41598 up-expressed in NPC HNE1 cells. The EST T93093 was also found to be down-expressed in 26.3%(5/19) of NPC biopsies. The full length cDNA of this gene was obtained and named NAG-7, which is located at chromosome 3p25.3. Its 1677 bp full length cDNA has a potential open reading frame(ORF) predicting a 94 amino acid protein with a molecular weight of 11023.87 Dalton. Bioinformatics analysis of the NAG-7 gene shows that it is a transmembrane protein containing a protein kinase C(PKC) phosphorylation site and a myristyl site. It has no significant homology to any reported genes in database of GenBank(AF086709). CONCLUSION: NAG-7 is a novel gene down-expressed in NPC, which may be involved in the development of NPC.

A clinical investigation on zhi ling tang for treatment of senile dementia.

Zhi Ling Tang (ZLT [symbol: see text], a TCM prescription designed for replenishing essence, supplementing marrow, invigorating qi, warming yang, removing blood stasis and phlegm, tonifying the brain, and invigorating mental activity) was used in 32 cases of senile dementia (SD). After treatment, the levels of serum cholesterol (Tch), triglyceride (TG), and plasma lipid peroxides (LPO) were lowered; the content of high density lipoprotein (HDL) and the activity of superoxide dismutase (SOD) in RBC significantly elevated, the cerebral blood flow was increased; latent period of P300 and P3 waves was shortened while the amplitude of P3 elevated; and topographic electroencephalogram, revised Hasegawa dementia scales (HDS) scores (P < 0.05-0.01) and clinical symptoms were improved. The total effective rate was 81.3%, indicating that ZLT exhibits relatively good therapeutic effects for treating senile dementia.

The angiogenesis inhibitor SU5416 has long-lasting effects on vascular endothelial growth factor receptor phosphorylation and function.

SU5416, a selective inhibitor of the tyrosine kinase activity of the vascular endothelial growth factor (VEGF) receptor Flk-1/KDR, is currently in Phase III clinical trials for the treatment of advanced malignancies. In cellular assays, SU5416 inhibits the VEGF-dependent mitogenic/proliferative response of human umbilical vein endothelial cells (HUVECs). In tumor xenograft models, SU5416 inhibits the growth of tumors from a variety of origins by inhibiting tumor angiogenesis. In three different human tumor xenograft models, infrequent (once or twice a week) administration of SU5416 is efficacious despite the fact that it has a short plasma half-life (30 min), which suggests that SU5416 has long-lasting inhibitory activity in vivo. The goal of the present study was to determine the basis for the prolonged activity of SU5416. The results indicate that a short (3 h) exposure to 5 microM SU5416 (to mimic plasma levels of the compound as measured in patients who were receiving SU5416 therapy) produced long-lasting (at least 72 h) inhibition of the VEGF-dependent proliferation of HUVECs in culture, which indicate that SU5416 has long-lasting inhibitory activity in vitro as well as in vivo. SU5416 treatment of HUVECs did not affect surface expression of Flk-1/KDR or the affinity of the receptor for VEGF. Instead, the durability of the in vitro activity of SU5416 was shown to be attributable to its long-lasting ability to specifically inhibit VEGF-dependent phosphorylation of Flk-1/KDR and subsequent downstream signaling, although SU5416 is not an irreversible inhibitor of Flk-1/KDR tyrosine kinase activity. The long-lasting inhibition of cellular responses to VEGF was attributable to the accumulation of SU5416 in cells, as shown using radiolabeled compound, such that inhibitory cellular concentrations of SU5416 are maintained long after the removal of the compound from the medium. The long-lasting inhibitory activity of SU5416 in vitro is consistent with the finding that SU5416 has demonstrated evidence of biological activity in clinical studies when administered twice a week despite a short plasma half-life.

[Recent advance in studies of new intraaortic axial flow pump: "dynamic aortic valve"].

We have previously proposed and demonstrated the feasibility of a new concept of intraaortic axial flow blood pump, termed "Dynamic Aortic Valve(DAV)". Implanted in the position of aortic valve, DAV can serve as either an assistant pump or a mechanical valve subjected to the power input. It has several merits over the traditional axial blood pumps: (1) better anatomic compatibility and biologic compatibility; (2) little foreign material is to be introduced into the corporeality; (3) better resistance to infection; (4) puncturing power line could be eliminated; (5) promoting the failing heart to recovery; (6) simpler structure and better durability. In our recent studies, the structure of DAV is further simplified and performance improved greatly. The structure of DAV is merely an impeller fixed to a magnetic rotor while held by a central shaft hanging from a rigid supporting cage. The rotor-impeller can be driven by an alternative magnetic field derived from a remote magnet, implying that the magnetic source can even be arranged extracorporeally. The DAV can be driven by a rotational magnet placed at a distance of 60 mm. At the state of maximal output of hydraulic power, the flow rate (FR) generated by DAV reaches 5 L/min against an afterload of 70 mmHg(1 mmHg = 0.133 KPa). The maximal pressure differential (PD) maintained by DAV is 131 mmHg. The results further revealed the feasibility of the concept of DAV.

[Study on COL7A1 gene mutation in a epidermolysis bullosa pruriginosa family].

OBJECTIVE: To identify gene mutation of a epidermolysis bullosa pruriginosa family. METHODS: Polymerase chain reaction (PCR), DNA sequencing, multiplex PCR using allele-specific oligonucleotide primers. RESULTS: A G6100A transition of COL7A1 was found in patients, resulting in G2034R substitution in type VII collagen. The mutation was not found in control normal individuals. CONCLUSION: The mutation of G2034R is the underlying cause of epidermolysis bullosa pruriginosa in this family, not common polymorphism.

A clinical investigation on garlicin injectio for treatment of unstable angina pectoris and its actions on plasma endothelin and blood sugar levels.

To investigate the therapeutic effects and mechanisms of garlicin for treatment of unstable angina pectoris (UAP), garlicin injectio was intravenously dripped 60 mg/day in 34 cases for 10 days. Nitroglycerine was used in 21 cases of the control group. The results showed that the total effective rates in improving symptoms and electrocardiogram after garlicin treatment were respectively 82% and 62%, and that the plasma endothelin and blood sugar levels were markedly lowered in cases with hyperglycemia.

[In vitro testing of a new implantable ventricular assist rotary pump made in China].

We have measured the hemodynamic properties of the rotary pump in order to give directions for use in animal test and even in clinical application. In our mocking circulation circuit, under definite preload, the inner diameter of inlet and outlet cannulae and the afterload have been changed for measuring the pressure of input and output of the pump and the pump flowing rate when it runs under 75% maxim working condition. The results demonstrate that with the cannulae of inner diameter of 8 mm as drainage, the pump provides 5.32 L/min of output against a mean afterload of average 60 mmHg(1 mmHg = 0.133 kPa) with a filling pressure of 10 mmHg. With the cannulae usually used in clinical setting, the flowing rates are more than 1.5 L/min against a mean afterload of 90 mmHg, and all output pressure of the pump is higher than 120 mmHg. In conclusion, the volume of the rotary pump is small, the needed power is low, and the hemodynamic parameters are suitable for animal test and even for clinical use.

[Diagnosis and management of Mycoplasma pneumonia with cold agglutinin disease].

OBJECTIVE: To study the Mycoplasma pneumonia (MP) with cold agglutinin disease (CAD) and inquire into its diagnosis and treatment. METHODS: 3 report of cases and literature were reviewed. RESULTS: Accompanying with hemolysis and microcirculatory retardation, 3 patients with acute respiratory infection were finally diagnosed MP with CAD. Antimicrobial therapy and plasmapheresis showed favorable effect. CONCLUSIONS: MP patients would be highly suspected to be associated with CAD, if accompanying with hemolysis and microcirculatory retardation. Laboratory examinations are helpful for diagnosis. On the basis of combined therapy, plasmapheresis is effective.

[Establishment of a Hantaan virus infected mouse neuron cell model].

OBJECTIVE: To establish a neuron cells model for Hantaan virus (HTNV) infection by using the cultures of cortical neurons of mouse embryo. Changes and mechanism of the damage in neurons after HTNV infection were studied. METHODS: The cortical neurons of 16-19 day old mouse embryo were cultured and infected with HTNV (A9 strain). Indirect immunofluorescence test (IFT), MTT colorimetric analysis and immunohistochemistry assay (IHCA) were used to detect immunofluorescent antibody reaction of neurons, growth and development of the neurons. RESULTS: In neurons of HTNV infected mouse embryo, continually cultured for one week, lFT antigen was detected positive, the activity of the infected neurons was reduced obviously and the growth of neurons was obstructed. CONCLUSIONS: Direct infection of HTNV on the cultured neurons was identified, indicating that mouse embryonic critical neurons is one of the sensitive target cells for HTNV infection.

[Total nose reconstruction with a reversed mandibular flap pedicled with the facial artery].

OBJECTIVE: To investigate the method to repair different kinds of nasal defects. METHODS: The reversed mandibular flap pedicled with the facial artery was used. RESULTS: The treatment was completed in one stage and the results were satisfactory in 6 cases. The reconstructed noses showed good shape. CONCLUSIONS: Total nose reconstruction using a reversed mandibular flap is a new operation design and technique. The recipient and the donor sites are very close. The flap with excellent blood supply has a good colour and textural match. The operative procedure and flap transferring are easy.
