A blood unit is transfused to a female patient, urgently. The names written on the form and on the vial are not identical. The determination of blood group is made by one person, instead of two, because the second one has been affected to a new hospital which is opening its doors. The only name of the vial is read. That is two mistakes. Thirdly, the blood grouping of the patient had already been realized, but is not looked for. Fourthly, the anaesthetist makes a wrong verification in the operating room. On the wrong blood unit was written the right name of the patient. No clinical symptoms appears during the intervention, on by oliguria 7 hours later. Haemodialysis. 13 days later the patient goes out in good health.
E.N.T. specialists are obsessed by the diagnosis of neurinoma of the VIIIth cranial nerve. Yet none of the conventional otological or radiological examinations is entirely reliable. In a major centre all otological and radiological investigations can be carried out systematically, but diagnosing the disease with a minimum of examinations is important, would it only be to decide which patients should be referred to the neuroradiologist. Complementary otological and neuroradiological explorations are therefore requested in ever increasing number on the basis of initial symptoms and results of the first examination. Among exploratory methods, CT with injection of air occupies a prominent place as it enables neurinomas located inside the internal auditory canal to be diagnosed at an early stage.
The pterygo-maxillary fossa, hidden behind the face under the base os the present time. The scanner has transformed the investigation of this region and offers the possibility of discovery of processes limited to the pterygo-maxillary fossa which was above all known as an area affected by the extension of tumour processes arising in the maxillary sinus of parotid. A topographical division of the pterygo-maxillary fossa is suggested on the basis of CT pictures and surgical approaches. The region of the pterygoids may be easily reached via a transmandibular, the technique of which is described.
Systematic total parotidectomy is still recommended by some authors for treatment of mixed tumors of the parotid. This attitude is based on the notion of multifocal involvement. In fact, multifocal involvement seems exceptional Very wide exeresis of the tumor is sufficient for efficient treatment, but the anatomical constitution of the parotid makes topographical division difficult. An attempt for systematisation of exeresis following the topography of the tumor is suggested, mainly for cases where parotidectomy is sufficient.

Alfatesine in perfusion, associated with laryngo-tracheal local anesthesia, was used to carry out 131 bronchoscopies. The comfort of the patient and of the bronchoscopist were then assessed. A few cardio-vascular side-effects, e.g. severe hypotension in two cases, hypertensive effects and tachycardia during introduction of the bornchoscope, were noted.

A synthetic gene coding for a modified ferredoxin II of Desulfovibrio desulfuricans Norway strain was assembled from 10 oligonucleotides. This gene was cloned into various expression vectors allowing either cytoplasmic expression or export to the periplasmic space. In the latter case, two different constructs were made, each of which contained the OmpA signal peptide: one of these constructs contained 3 additional N-terminal amino acids as compared to the wild-type ferredoxin (56 amino acid residues). The expression of proteins encoded by the 3 constructs was assayed in E coli and the proteins were localized by cell fractionation and immunogold labelling. A low percentage of the periplasmic ferredoxin (approximately 5%) was secreted to the medium in the absence of cell lysis. The recombinant ferredoxin was purified and found to be correctly processed by the leader peptidase. However, due to the high cysteine content intramolecular and intermolecular disulfide bonds were formed and prevented binding of [4Fe-4S] clusters. Reconstitution experiments using these recombinant proteins are in progress.

The rigid part of the human body consists essentially of carbonated apatite (calcium phosphate). Biologists don't have any tools to study this "mineral" phase, though its origin is organic. A new approach of some compounds like enamel or bone is obtained with the Raman micro-characterisation by a very fine analysis of chemical bonds in a micrometric scale. This method allows the characterisation, the analysis and the dosage of ions, like carbonate, acid phosphates, proteins and fatty acids. The identification of other organic or mineral compounds (e.g. calcium carbonate, calcium oxide, substitutant ions...) is also possible. The Raman microspectrometry can also be used to study the chemical and physical properties of biomaterials and their evolution after implantation in a dental or bone site. On synthetical calcium phosphate, beta-TCP, brushite and hydroxyapatite can be distinguished and the impurities found in plasma spray deposits can be measured. The detection of alpha-, beta-, or gamma-pyrophosphates could be obtained in some commercial beta-TCP. The Raman microspectrometry is the only non-destructive method which allows the identification of the chemical bonds in a micrometric scale and gives the "fingerprint" of the studied component.
The interaction between biomaterials used in surgical procedures and the host bone is not yet perfectly understood. It appears that these problems may have been encountered because of insufficient characterisation of the basic component used in the synthesis of such biomaterials. Calcium carbonate (CaCO3) is interesting for bone filling or regeneration procedures because of its resorbability. The aim of this work is to compare different CaCO3 biomaterials and their basic source with the help of microRaman spectroscopy. Bionacre and Biocoral are analysed. The main bands of carbonate internal modes are observed around 1084-86 and 704-12 cm-1. In the lattice modes region, for both Biocoral and synthetic aragonite, two bands at 206 and 155 cm-1 are observed. The eggshell, oyster shell and synthetic calcite samples exhibit bands at 281 and 155 cm-1. Three bands are present at 280, 206 and 155 cm-1 on the Bionacre sample. The 206 and the 280 cm-1 bands are due to aragonite and calcite forms respectively. Therefore it appears to be a mixture of aragonite and calcite whereas Biocoral is pure aragonite. Additional Raman investigations should be of great interest in evaluating the structural modifications and their influence on the biological behaviour of these biomaterials. Lity index, the percentage of prematurity and of low birth weight are the indices of national health status.
