Antibodies to glutaraldehyde-polymerized human albumin (anti-pHSA) in viral hepatitis.

The antibody to polymerized human albumin (anti-pHSA) was studied in normal subjects and in the course of infection from hepatitis A virus, hepatitis B virus, and hepatitis non-A, non-B virus. Results show that anti-pHSA antibody was never found in normal subjects, but it appeared during virus liver pathologies. The behavior of anti-pHSA differs in acute type A hepatitis that does not change to chronic form and in those forms which tend to become chronic (B and nonA, nonB). In the type-A infection anti-pHSA disappears after the acute phase; in the other two forms it persists all along as the infection develops. Specifically in non-A, non-B infection only the IgM type anti-pHSA is produced.

High-dose Ara-C (HiDAC) plus asparaginase in elderly patients with acute non-lymphocytic leukemia: a pilot multicentric study by the Italian Cooperative Group GIMEMA.

A multicentric prospective pilot study using three different schedules of high-dose Ara-C at dosage of 3 g/m2 every 12 hours during 3 h of infusion was undertaken by the Italian Cooperative Group GIMEMA in order: 1. to evaluate the safety and efficacy of such treatment in previously untreated ANLL patients more than 50 years old; 2. to investigate whether the addition of a standard maintenance treatment after consolidation with 4 courses of DAT (Daunorubicin + Ara-C + 6-Thioguanine) could improve the duration of complete remission (CR) and the proportion of long-term survival. Overall 43/125 evaluable patients (34.4%) achieved CR. 32/125 died during the induction phase, the remaining 50 patients (40%) failed to achieve CR. As for the toxicity, the most significant toxicity of all schedules was hematologic. No substantial neurological or cardiac toxicity was observed. The multivariated analysis of several pretreatment characteristics revealed that age more than 60 yr, male sex and presence of infections at diagnosis were the most significant adverse factors for achievement of CR. The median duration of DFS for all responders was 9 months, with relapse-free survival at 4 yr estimated at 29%. The addition of maintenance treatment to consolidated patients had no advantages in respect to the control group, even though the statistical analysis revealed a p = 0.058. However, because of the small number of randomized patients, no conclusions can be drawn concerning the importance of maintenance treatment.

Preventive distinction of patients with primary or secondary hypertension by discriminant analysis of chronobiologic parameters estimated on 24-hour blood pressure patterns.

This investigation deals with a statistical probatory that patients with primary (PH) or secondary (SH) hypertension may be correctly diagnosed by a discriminant analysis of the chronobiologic characteristics computed on the 24-hour blood pressure (BP) patterns. The methodology concerning non-invasive 24-h BP monitoring, chronobiologic analysis and the discrimination process is detailed. Substantial dissimilarities were found in the statistical distribution for systolic and diastolic BP rhythmometric parameters (mesor, amplitude and acrophase) by a retrospective assessment of two groups, consisting of 54 patients with PH and 16 patients with SH. The group-related distribution for rhythmometric parameters was found to be significantly different to generate a statistically significant intergroup discriminatory boundary. The discriminant analysis correctly diagnosed patients with PH and SH in a percentage of about 91% and 63%, respectively. The high incidence of success is convincing that the combination of 24-h BP monitoring/chronobiologic analysis/discrimination process cna be a practical tool for confidently selecting patients with a presumable PH or SH.

OAP combination in the treatment of elderly leukaemic patients with preexisting severe internal disease.

Sixteen elderly patients affected by acute non lymphoblastic leukaemia (ANLL) with a preexisting severe internal disease were treated with a low systemic toxicity drugs combination: OAP (Vincristine, Cytarabine and Prednisone). Complete remission was achieved in 5 patients (31%) after 2 OAP courses. The mean duration of remission was 18 weeks. Six patients were resistant to the therapy. Six patients died during the treatment: 5 in induction phase and 1 in consolidation phase. Even though the duration of remission was short we retain that OAP combination may be still considered a good therapeutical approach in elderly ANLL patients with associated severe internal disease.

[Chyloperitoneum. Presentation of 2 cases].

The authors report two cases of chylous ascites recently observed. After a wide review of the literature, they point out the rarity of this disease and its particular therapeutic aspects.

[Etiopathogenetic and histopathologic aspects of acute necrotico-hemorrhagic pancreatitis in experimental animals].

In this study, the Authors have provoked experimentally acute pancreatitis in the rat by different methods, in order to find out morphological modifications of the pancreas in the initial stage of the disease. In case of biliary and pancreatic duct obstruction with pure pancreatic reflux, both oedema and inflammatory infiltrations were evident, whereas, in the presence of biliary reflux too, more serious histological features were detected. In conclusion, the first sign of alteration in acute pancreatitis could be represented by the association of intraductal hypertension and pure pancreatic reflux.

Central nervous system acute promyelocytic leukaemia: a report of three cases.

Authors report 3 cases of acute promyelocytic leukaemia, in which the central nervous system was involved in the first period of relapse. The clinical features of these patients are discussed. Authors conclude that central nervous system involvement in acute promyelocytic leukaemia must be considered a very bad prognostic sign, because in these cases a rapidly progressive course may be expected.

Cyclosporin A and psoriasis: a thermographic study.

The thermographic method permits the making of a rigorous evaluation of the cutaneous blood flow by means of thermal recovery times after thermostimulation at 5 degrees C for 20 sec. The aim of the present study was to examine the blood flow of the psoriatic skin of 7 patients before and after a course of cyclosporin therapy, by means of this method. The results show a constant, initial hyperthermia with precocious thermal recovery times, in comparison with the healthy perilesional skin. Following therapy, a prolongation of the thermal recovery time was observed in all of the patients, together with a significant improvement of the dermatosis.

[Echotomography in abdominal emergencies. Apropos of 16 controlled clinical cases. (A description of some of them)].

A personal series of abdominal emergencies subjected to ultrasound studies is reported. The results of these studies were identical to the surgical and histological findings. It is therefore concluded that even in the absence of other diagnostic techniques (scintiscans, angiographic, CAT scans, etc.) ultrasound imaging can make a major contribution to the diagnosis of intra and extraperitoneal abdominal emergencies especially in district hospitals.

Meningeal hematopoiesis causing exophthalmus and hemiparesis in myelofibrosis: effect of radiotherapy. A case report.

Meningeal myeloid metaplasia (MM) is very rarely observed in patients with myelofibrosis. We report the occurrence of meningeal MM causing exophthalmus and fever in a patient with myelofibrosis secondary to polycythemia vera. A computerized tomography (CT) scan showed multiple intracranial and intraorbital enhancing masses. A needle aspirate of retrobulbar space confirmed the diagnosis of extramedullary hematopoiesis. The patient subsequently developed a rapidly worsening tumor-like syndrome with hemiparesis, aphasia, and loss of sphinteric control. The administration of radiotherapy caused a complete and stable regression of clinical symptoms and a marked reduction of MM masses.

Antithrombin III molecular variants with defective binding to heparin or to serine proteases: evidence of two different abnormal patterns identified by crossed immunoelectrofocusing.

Molecular heterogeneity of antithrombin III (AT III) was investigated by a technique of crossed immunoelectrofocusing (CIEF) in plasma samples of patients from 16 families with AT III congenital defect, including 8 AT III molecular variants. The AT III CIEF pattern was normal in all the patients with AT III quantitative deficiency, showing a balanced decrease of all the peaks. Out of the 8 AT III variants investigated, 6 had an abnormal pattern: the three variants with defective binding to heparin (AT III Roma, AT III Barcelona, AT III Malmö) shared a similar abnormal pattern; three variants with defective binding to serine proteases (AT III Pescara, AT III Milano, AT III Tampere) had a common abnormal pattern clearly different from the first one, whereas the other two variants deficient in the inactivation of the serine proteases (AT III Chicago, AT III Milano 2) showed a normal pattern. The first type of pathological pattern (type Roma) was characterized by the presence of an abnormal peak overlapping the normal isoforms present at pH 4.8-4.6 and by an additional peak at pH 4.5. The second type of pattern (type Pescara) showed an additional peak at pH 4.5 and an abnormal quantitative distribution of the isoantithrombins all throughout the pH range (5.2-4.6). In order to separate the abnormal AT III fraction from the normal one, plasma of a patient with Roma defect and serum of a patient with Pescara defect were passed throughout an heparin-ultrogel column.(ABSTRACT TRUNCATED AT 250 WORDS)

Long-term cytogenetic effects of antineoplastic treatment in relation to secondary leukemia.

Chromosome translocations are consistently present in leukemias and lymphomas and are likely to represent primary events in the development of these neoplasias. A study of conditions that predispose to leukemia could shed some light on the origin of these translocations and therefore help in clarifying their exact role in the process of neoplastic transformation. Based on this assumption, we studied a group of individuals treated with radiochemotherapy for previous lymphoma and who were at increased risk of developing a secondary leukemia. The group comprised 14 Hodgkin's disease patients, 11 non-Hodgkin's lymphoma patients, and 13 controls. The patients were in remission and had been off therapy for at least 6 months. Chromosomes were studied from phytohemagglutinin (PHA)-stimulated peripheral lymphocytes and from bone marrow cells by the direct method and after short-term cultures (72 hours). The latter were also exposed to 5-bromodeoxyuridine (BrdU). Metaphases were scored for chromosome breaks, gaps, and other rearrangements. The percentage of gaps and breaks was significantly higher in patients than in controls. The difference was induced by BrdU and was apparent in bone marrow cells, but not in peripheral lymphocytes. We conclude that individuals exposed to the action of mutagenic agents (radiochemotherapy) have an increased chromosome instability that could be related to their increased risk of developing a secondary leukemia.
