[Late hip dislocation--treatment results of 1193 hips in attenuated abduction (Hanausek position)].

Results of 1193 dislocated and dysplastic hips in 695 children of a consistent treatment by skin traction and splinting in a mitigated abduction device (Hanausek position) are given with regard to the acetabular development and avascular head necroses. The acetabular development depends on the initial acetabular dysplasia and is overall satisfactory through final acetabular angles (Hilgenreiner) between 22 and 25 degrees even in highly dysplastic acetabula in late cases. The procedure is non-aggressive proven by low rate of femoral head necroses of 3.4%. Factors contributing to higher rates and degrees of iatrogenic damage to the femoral head are short time or no preceding longitudinal traction, low initial acetabular pathology and prolonged splinting.

MR imaging of the pancreas at high field strength: comparison of six sequences.

The authors compared six MR sequences comprising conventional breath-hold [rapid spin echo (RASE) repetition time (TR) 240 ms/echo time (TE) 8 ms/90 degrees, fast low angle shot (FLASH) 130/4.5/80 degrees, TurboFLASH 6.5/3.5/8 degrees], fat suppressed regular spin echo (FS SE 330/15/90 degrees), and two combined fat suppressed breath-hold sequences (FS FLASH 130/8/80 degrees, FS RASE 240/10/90 degrees) for studying the normal pancreas. Sequences were selected on the basis of features desirable for demonstrating the pancreas, particularly absence or decrease in artifacts and improved dynamic range of intraabdominal tissue signal intensities. Ten normal volunteers were studied, six at 1.5 T and four at 1.0 T, and comparison was made to regular short TR/TE SE. Quantitative pancreas signal-to-noise (S/N) and pancreas fat-to-noise (SD/N) measurements and qualitative evaluation of overall resolution and artifacts were determined. Fat suppressed FLASH had the highest S/N (44.1 +/- 10.8, p less than 0.0001) and SD/N (35.0 +/- 11.9, p less than 0.0001), and seven studies were considered good or very good. Fat suppressed SE had good S/N (32.6 +/- 7.7) and SD/N (19.0 +/- 3.6), and eight FS SE studies were considered good or very good. Among the nonsuppressed sequences, FLASH had the best combination of quantitative and qualitative measurements. Our results suggest that fat suppression may be important for studying the pancreas and that nonsuppressed FLASH may be a reasonable alternative.

A case of Asperger's syndrome first diagnosed in adulthood.

A 25-year-old white male patient was admitted to the Department of Psychiatry, Division of Social Psychiatry, of the University of Vienna, Austria, for severe social withdrawal, selective mutism and outbursts of violence with attacks on his mother. Careful examination revealed the presence of all the typical symptoms of Asperger's syndrome. The diagnosis had never been made before, although the patient had a history of a difficult childhood with several admissions to a child psychiatric inpatient unit for 'obsessional neurosis' and an institutional career. It is stressed that, in view of the availability of treatments and the deleterious effect of the untreated condition in the sensitive years of personality development, early recognition and diagnosis of Asperger's syndrome are of utmost importance.

Naturalistic course of obsessive compulsive disorder and comorbid depression. Longitudinal results of a prospective follow-up study of 74 actively treated patients.

Seventy-four patients who met DSM-III-R criteria for obsessive compulsive disorder (OCD) were studied in a prospective follow-up study in order to investigate course and prognosis of OCD with or without comorbid depressive symptomatology. Subjects were examined three times: at admission (baseline), 6 months later (follow-up 1) and 12 months after follow-up 1 (follow-up 2). At admission, 51 (72.9%) OCD patients were assessed as depressive by the Hamilton Depression Scale score. Between admission and follow-up 1, all patients received behavior therapy and a serotonin reuptake inhibitor, between follow-up 1 and follow-up 2 they received different kinds of treatment in order to maximize therapeutic effects. A 25% Yale-Brown Obsessive-Compulsive Scale (Y-BOCS) score reduction from admission to follow-up 2 and in addition, a total Y-BOCS score of below 16 at follow-up 2 was defined as 'good prognosis course'. The results obtained showed that OCD patients who followed a good prognosis course, showed no significant depressive symptomatology at follow-up 2 (p = 0.001). These results imply that patients with a diagnosis of OCD may present depression at admission and/or follow-up 1; however, if OC symptomatology decreases longitudinally, depressive symptoms disappear too. We may assume that OCD is dominant over depression, and it seems that a comorbid depression does not have any major influence on the prognosis of OCD.

Quality of life in depression and anxiety disorders: an exploratory follow-up study after intensive inpatient cognitive behaviour therapy.

Thirty-seven patients with depression and anxiety disorder, who participated in an intensive inpatient cognitive behaviour therapy program for 6 weeks, were interviewed before treatment and 6 weeks after the end of treatment; in addition to other measures, quality of life was assessed with the Berlin Quality of Life Profile. Substantial reduction in subjective quality of life, objective functioning and environmental assets was found at baseline. At follow-up, according to clinical global impression, 13.5% of the patients were very much improved, 45.9% much improved; in 26.3% only slight improvement and in 16.2% no improvement was reported. Quality of life changed for the better in areas like work and education, leisure, housing, social relations, psychological well-being and a global rating of satisfaction with life, but not in marital relations, health in general and in finances.

Protein kinase C-mediated in vitro invasion of human glioma cells through extracellular-signal-regulated kinase and ornithine decarboxylase.

We investigated the involvement of protein kinase C (PKC) in the in vitro invasiveness of the A-172, U-87 and U-373 human glioma cell lines, as well as the role of ornithine decarboxylase (ODC) and/or extracellular-signal-regulated kinase (ERK) in the actions of PKC. Thus, cells were treated under serum-free conditions with the PKC activator phorbol 12-myristate 13-acetate (PMA), or with the PKC inhibitors bisindolylmaleimide I (GF 109203X) or calphostin C in the absence or presence of the ODC inhibitor D,L-alpha-difluoromethylornithine (DFMO), and/or the mitogen-activated protein kinase/extracellular-signal-regulated kinase inhibitor 2'-amino-3'-methoxyflavone (PD 098059). Subsequently, cells were assessed for membrane-type 1 matrix metalloproteinase (MT1-MMP) mRNA contents, 72-kD latent, and 59/62-kD activated matrix metalloproteinase 2 (MMP-2) in conditioned media, as well as invasiveness. For these purposes, we used Northern blot analysis, gelatine zymography, and an in vitro filter invasion assay, respectively. Data were related to those found with untreated cells. PKC activity was 2- to 3-fold stimulated by PMA (100 nM for 30 min), and about 2-fold inhibited by calphostin C (40 nM for 2 h) or GF 109203X (5 microM for 20 min). This was accompanied by a similar increase or decrease, respectively, in MT1-MMP mRNA expression, 59/62-kD MMP-2 activity, and in vitro invasion. Inhibition of ODC activity (about 2-fold by 24 h DFMO 5 mM), ERK activation (almost completely by 20 min PD 098059 50 microM), or both these enzymes simultaneously led to a reduction by about half in levels of MT1-MMP mRNA, 59/62-kD MMP-2 activity, and invasion in untreated as well as PMA-stimulated cells. The use of these compounds did not significantly alter the inhibitory effects of GF 109203X or calphostin C. Modulation of PKC and/or ERK activity resulted in corresponding changes in ERK and/or ODC activities, but interference with ODC affected neither ERK nor PKC. Our data suggest a regulatory role for PKC, in co-operation with ERK and ODC, in glioma cell invasion, by modulation of MT1-MMP mRNA expression and MMP-2 activation.

[Semiology and nosology].

While distinguishing three successive steps in the making of a diagnosis, the authors point out the premises of modern semiology. They refer to the work of K. Schneider (pragmatic approach), Jaspers, Bleuler and Janzarik (approach based on theoretical concepts), Kasanin and finally Langfeldt (empirical approach). Current approaches of semiology try to define diagnostic criteria. In that line of thinking, the school of Vienna, while establishing the insufficiency of all systems, proposes the adoption of a polydiagnostic approach that would eventually define consistent semiologies that refer with certainty to real nosological entities.
