Prospective study of children exposed to variable amounts of alcohol in utero.

Forty children exposed to variable amounts of alcohol in utero and 40 control children were studied. All mothers had been enrolled in an antenatal programme aiming to identify and reduce alcohol use and abuse during pregnancy. Follow up was at the median age of 22 (18 to 27) months. A significant reduction in intrauterine growth was seen in children born to alcoholic mothers. Three of six children continuously exposed to high amounts of alcohol throughout fetal life showed growth retardation and physical abnormalities characteristic of fetal alcohol exposure, while infants whose mothers had stopped drinking did not suffer these effects. Psychological or behavioural disturbances were found in all but one of 13 children born to alcoholic mothers. The home environment during the first two years did not compensate for the effects of fetal alcohol exposure. Mothers classified as excessive drinkers but not abusers all reduced their alcohol consumption after the first trimester. Their children did not differ from controls with regard to physical development or behaviour but many were retarded in speech and, in addition had a more unstable family background.

Are the social workers prepared to assist a changing population of dysfunctional parents in Sweden?

What kind of support can an affluent society as the Swedish offer dysfunctional parents in order to give the infants an optimal start in life? The purpose of the present study was to analyze the social situation in families precipitating the admission of infants to institutional care in 1970, 1975 and 1980. The results show that in 1980 more complex forms of parental dysfunction were common including mental disorders, criminality, abuse of alcohol and drug addiction. The reported incidence of domestic violence had increased significantly from 1970 to 1980 as well as the number of maltreated infants. The social welfare agencies had greater difficulties and to a larger extent lacked adequate methods of providing assistance to the dysfunctional parents in 1980 than in 1970.

Operative management of vaginal vault prolapse following hysterectomy.

Nine women suffering from vaginal vault prolapse had an abdominal sacropexy between 1972 and 1983. Marlex mesh was used to anchor the vaginal vault to the promontory of the sacrum and was completely buried retroperitoneally. The women had all had previous attempts at surgical correction. There were no intra- or post-operative complications. No recurrences of vault prolapse occurred during a mean follow-up period of 3.9 years. One woman developed a moderate cystocoele 4 years after sacropexy.

Treatment of condylomata acuminata with the carbon dioxide laser.

Extensive and/or recurrent condylomata acuminata in 78 women were treated with the CO2 laser. Urethra, clitoris or the anal canal were involved in about one-fourth of the cases. Most were treated as outpatients with the more extensive lesions staged for multiple treatments. After a follow-up period of 6-33 months, 91% of women were free from disease and treatment failures occurred in 9%, some of which may have been new infections. No serological investigations were performed.

Use of an antifibrinolytic agent (tranexamic acid) and lateral sutures with laser conization of the cervix.

One hundred forty patients who underwent laser conization and 220 patients who underwent laser miniconizations were prospectively randomized into two study groups. One treatment group was given antifibrinolytic therapy in the form of tranexamic acid (Cyklokapron, KabiVitrum, Sweden) intraoperatively and for 14 days postoperatively. The other group did not receive antifibrinolytic therapy. In the group of 68 patients with laser conizations who were given antifibrinolytic therapy, no postoperative hemorrhages occurred, whereas there were eight such hemorrhages in 72 conizations (11%) in the untreated patients. This difference is statistically significant (P = .004, Fisher exact test for two proportions). Also, for laser miniconization, the frequency of postoperative hemorrhage was almost halved, from 9.1% in 110 patients not receiving antifibrinolytic therapy to 5.5% in the 110 treated patients. The use of lateral cervical sutures did not reduce the frequency of postoperative hemorrhage at laser conization in the present study.

Treatment of pelvic varicosities causing lower abdominal pain with extraperitoneal resection of the left ovarian vein.

Fifteen women with chronic pelvic pain in whom left sided renal phlebography had demonstrated pelvic varicosities were operated upon with extraperitoneal resection of the left ovarian vein during an eight year period. At follow-up eight patients were completely cured and three were considerably improved, but in four women no improvement occurred. Mean period of follow-up was 5.6 years (range 0.5 to 8 years). All but two women were followed for at least three years. The complication rate was low. One case of wound infection and one case with bleeding from a subcutaneous artery necessitating resuture were the only complications.

Evaluation of serum gamma-glutamyl transferase as a screening method for excessive alcohol consumption during pregnancy.

Serum gamma-glutamyl transferase was analyzed in 669 pregnant women. They were interviewed with special emphasis on their consumption of alcohol during the first trimester. Thirteen percent admitted a consumption of greater than 30 gm of pure alcohol per day, and 3%, greater than 125 gm per day. One child with a complete fetal alcohol syndrome and one with a partial one were born to mothers with a high consumption of alcohol and elevated serum gamma-glutamyl transferase. The ninety-fifth percentile for serum gamma-glutamyl transferase in women classified as low consumers (less than 30 gm of alcohol per day) was 0.44 mukat/L. With this 95% specificity level used as a cutoff, the sensitivity of the serum gamma-glutamyl transferase test in identifying those who consumed more than 30 gm of alcohol per day was 25%. The conclusion was that screening based on analyses of serum gamma-glutamyl transferase is not to be recommended for routine use in prenatal care.

Prognostic factors in early invasive carcinoma of the uterine cervix. A clinical, histopathologic, and statistical analysis of 343 cases.

Three hundred forty-three cases of early invasive carcinoma of the uterine cervix were analyzed. Depth of infiltration and lateral extension of the tumors varied between 0.2 and 9.0 mm and 0.4 and 17.2 mm and were found to be of no prognostic importance. A multivariate statistical analysis revealed several risk factors: an epithelialized portio at diagnosis, suspected or evident tumor invasion in capillary-like spaces, absence of tumor-free margins at conization, and treatment with conization or simple hysterectomy, especially in the presence of the above-mentioned factors. Tumors of a large cell keratinizing type were prognostically favorable. Confluent growth was not associated with increased risk of recurrence or death from recurrence. Cervical biopsies proved to be inadequate as diagnostic material. Recurrences were almost always local and developed late. In absence of risk factors conization is sufficient treatment. In the presence of risk factors treatment should be the same as that performed for frankly invasive Stage IB carcinomas.

Laser miniconization in mild and moderate dysplasia of the uterine cervix.

Mass cytologic screening for cervical cancer often reveals only mild dysplasia not indicating conization but necessitating continual checkup. Such routine checkups are often insufficient, beside which the patients find them frustrating. Therefore a new method, called miniconization, for treatment of patients with vaginal smears showing mild or moderate dysplasia (cervical intraepithelial neoplasia/CIN/I-II), was developed. With the CO2 laser handpiece a 5-mm-thick disc of the cervix, including the whole transformation zone, was removed. This was followed by endocervical curettage. The advantage of the method over cryosurgery, electrocoagulation, and laser vaporization, for example, is that the tissue specimen as a whole disc including the transformation zone can be sectioned and examined histologically. Another advantage is the decreased risk of postoperative bleeding, which enables ambulant care. One hundred and fifty-one patients have hitherto been treated and carcinoma in situ has been found in 15.2% and microinvasive carcinoma in 1.3% of all these patients in whom vaginal smears showed mild or moderate dysplasia (CIN I-II).

Institutional care of infants in Sweden: criteria for admission in 1970, 1975 and 1980.

The purpose of the present study was to analyze the criteria used for admission of infants to institutional care in the metropolitan area of Stockholm in 1970, 1975 and 1980. The study included 337 infants. Their records from the institutional care have been studied. A significant decrease was observed from 1970 to 1980 on the following grounds for admission: teen-age mother, adoption, unsatisfactory housing situation, hospitalization of the mother. The following criteria for admission showed a significant increase: maternal alcohol and/or drug abuse as well as psychiatric disorders. It is concluded that there has been a dramatic change in the criteria used for admission of infants to institutional care from 1970 to 1980. Voluntary admissions of infants for maternal health reasons or social problems of a temporary nature have apparently been supplanted by non-voluntary admissions owing to maternal addiction and psychiatric insufficiency.

Prevention of fetal alcohol effects. An antenatal program for early detection of pregnancies at risk.

The increased consumption of alcoholic beverages by young people must be viewed with great concern, particularly as regards prospective parents. A study was therefore started at four Maternal Health Clinics to develop methods for the early detection of maternal alcohol abuse and for treatment of the women concerned. The study involved 464 consecutive pregnant women. A systematic drinking anamnesis was obtained from all patients. Attention was also paid to additional indicators of maternal alcohol abuse. Four per cent of the women were classified as alcohol abusers, and a further 7% as excessive drinkers. The majority of the mothers reduced their drinking altogether or stopped after receiving information about alcohol. The alcoholic women, on the other hand, needed intensive counseling. Most women welcomed the opportunity to obtain an accurate assessment of the risks of alcohol consumption during pregnancy. One infant was given a diagnosis of fetal alcohol syndrome at birth and another neonate had a partial fetal alcohol syndrome. The antenatal staff identified significantly more women with excessive alcohol consumption, compared with the situation the year prior to starting the program. Family guidance provided by a multidisciplinary team in pregnancy may result in lifelong benefits to both the mother and her child.

A comparison of complications of laser and cold knife conization.

A comparison was made of intraoperative and postoperative complications of laser conization and cold knife conization. Of 428 women hospitalized for cold knife conization, 23.6% had one or more complications; 14.3% had postoperative hemorrhages, 6.8% had infections, and 4.7% suffered from stenosis. Two hundred sixteen women were hospitalized for laser conization; 5.1% of them suffered complications. Postoperative hemorrhages occurred in 2.8% and infections in 2.3%. No case of stenosis occurred. Laser conization as an outpatient procedure was undertaken in 240 women; 4.2% had complications. Postoperative hemorrhages occurred in only 2.5%, infections in 1.3%, and stenosis in 0.8%. Laser conization has the same complication rate as laser vaporization and can be safely performed on an outpatient basis, thereby reducing hospital costs.
