[Surgical management of inflammatory disease in the frontal and ethmoid sinuses (author's transl)].

The opening microscope became very important in nasal sinus surgery, especially for operating procedures of the ethmoid sinus. For many authors nasal polyposis without putrid secretion is an indication not only for polypectomy, but for ethmoidectomy. Using the operating mikroscope frontal sinus surgery can be performed in a much more delicate way. The best cosmetic results are seen after osteoplastic opening of the frontal sinus. This technique can not be used if ostitis or osteomyelitis is present. There are several ways to treat mucosal changes in the frontal sinus. Its lumen may be drained as soon as there are no severe mucosal changes. In severe cases the mucosa must be removed and the sinus lumen should obliterate either spontaneously or after implantation of autologous material. - Endonasal surgery of the frontal and ethmoid sinus can be hazardous in unexperienced hands.

[Glucose determination in diluted whole blood using an enzyme electrode].

The construction and operation of a measuring device containing an enzyme electrode is described. Glucose oxidase is enclosed in an acrylamide film by means of photopolymerization. The maximal increase of the anodic current of the H2O2 being formed in the enzyme reaction serves as measuring signal. The device gives for diluted serum and blood correlation coefficients above 0,99 according to the methods given in the German Pharmacopeia 7. The measuring time for each specimen is roughly 90 s, the serial variation coefficient 2%.

The anti-proteolytic behavior of lathyrogens.

The lathyrogens, beta-aminopropionitrile and alpha-aminoacetonitrile inhibit both the esterolytic and proteolytic activity of trypsin at a concentration of 100 mM. Lineweaver-Burk plots demonstrate that inhibition is competitive, with alpha-aminoacetonitrile being the more potent inhibitor. The enzyme associated with and capable of digesting tropoelastin is inhibited by both lathyrogens when tested against its natural substrate, tropoelastin. Administration of alpha-aminoacetonitrile-HCl to the diet of young chicks (0.1% w/w) resulted in a 62% increase in the yield of tropoelastin and significant reduction in fatality as compared to beta-aminopropionitrile-fumarate.

Circulating immune complexes in patients with malignant lymphomas and solid tumors.

Sera from 50 patients with Hodgkin's disease, 78 patients with non-Hodgkin non-leukemic malignant lymphomas, and 75 patients with different types of solid malignant tumors were investigated for the presence of immune complexes using the (125I) C1q-binding test. All patients were untreated. An increased serum C1q-binding activity was found in 22% of the patients with Hodgkin's disease, 35.9% of the non-Hodgkin lymphoma patients and in 37.3% of the patients with solid tumors. The C1q-binding material detected in the patients' sera had properties similar to those of immune complexes. On sucrose density gradient it sedimented as a 10-30 s material. It contained IgG which were dissociated under acid conditions. Passage through anti-IgG immunoabsorbent removed its C1q-binding properties. A prevalent association was found between the presence of serum immune complexes and disseminated disease stages in all the patient groups included. A similar association was found between the presence of serum immune complexes and general symptoms among the malignant lymphoma patients. The nature of the antigens involved in the complexes remains unknown.

Interaction of myogenic and neurogenic mechanisms that control heart rate.

Interaction of neurogenic and myogenic factors that control heart rate was studied. Sympathetic nerve actions and stretch of the sinoatrial node both have an accelerator effect which appears to be competitive, and additive rather than facilitatory. In contrast with this, the accelerator effects of sinoatrial node stretch were proportionately greater when stretch was applied during a slowing produced by vagus nerve stimulation. The hyperpolarizing effect of vagus nerve stimulation or acetylcholine apparently potentiates the accelerator effects consquent to the depolarizing actions of stretch. The myogenic reaction can be considered as playing a role in maintaining cardiac output and preventing excessive slowing or diastolic distension and a deficient circulation.

[The intratympanic treatment of Menière's disease with ototoxic antibiotics. A follow-up study of 55 cases (author's transl)].

Report about 55 patients suffering from severe unilateral Menières disease, seen over a period of 3-10 years. They were treated with intratympanic administration of Streptomycinesulfate or- better -Gentamycinesulfate. The medication was given using a plastic tube inserted behind the annulus by transmeatal approach. 0.1 ml Gentamycinesulfate (earlier Streptomycinesulfate were instillated every 5 hours until the first signs of inner-ear-reaction (nystagmus or vertigo) appeared. -In 90% of the cases vertiginous attacks ceased after therapy. Hearing was preserved in 76%, tinnitus and periaural pressure were influenced markedly.

[Valve shunt for use in heart surgery and surgery on the great vessels].

Surgery with extracorporeal circulation must be carried out within temporary limits because of trauma to the blood by the pumping system and partly because of the oxygenators. For vascular replacement extracorporeal circulation can only conditionally be used because heparinization is required. The authors deviced a bypass for certain surgical interventions on the heart and thoracic aorta without the above mentioned drawbacks, which can be used for a longer period without pump and heparinization. This valve shunt stood all trials in experiment.

[On the causes of intra- and post-operative consumption coagulopathies and postoperative thromboembolism].

The determination of the onset of a disseminated intravascular coagulation (DIC) is examined shortly after its intraoperative andpost-operative dissolution with the help of easily performable haematological and physiological clotting tests in 20 patients. In this connection the operation is appreciated as a model even for other processes defined at the beginning, where DIC can be observed. Whereas the aethanol test, the determination of fibrinogen split products (FSP) and the euglobulin lysis time indicate the beginning of DIC more clearly in the form of average values, the aethanol test, partial thromboplastin time and thrombin time have a prognostic value for each patient. As it is too time consuming to determine FSP, the counting of basophilie granulocytes may be used for the diagnosis. In the initial phase of and post-operative DIC will determine the essential share of predisposition to post-operative thromboembolism.

[Problems of postoperative respiratory insufficiency (author's transl)].

Postoperatively patients are particularly endangered by the development of respiratory failure. All parts of the gas-exchange between the atmospharic air and the metabolizing cell are to be considered as a unit. The increase of dead-volume-respiration and intrapulmonary shunting is of great importance for the development of arterial hypoxaemia. Early diagnosis of impaired gas-exchange and an adequate treatment are emphasized. CPAP and intermittent mandatory ventilation are therapeutic methods, well approved in clinical practice.

[Endoscopic-radiologic cholangio-pancreaticography (author's transl)].

Results of 408 own endoscropic-radiologic cholangio-pancreatico-graphies are presented, indications and diagnostic relevance of the method are discussed. Low rate of complications and its high diagnostic value are reasons for ERCP's clinical importance even as a routine procedure at gastro-enterological centers. In the future, cytological, immunological and biochemical analysis of juices and aspirates as well as endoscopic surgery will contribute to an even wider clinical use of the method. In 337 patients (82.6%) cannulation of the papilla of Vater and visualization of one or both duct systems were successful. In 198 cases with visualization of ducts (58.8%) pathological lesions were found. Thus observation of indications and contra-indications of ERCP is documented, of a method, which is not too risky, but relatively expensive and difficult to employ.

[Cryotherapy of eyelid and conjunctival tumors].

Report is given of cryobiological principles, technique and results of cryotherapy for eyelid or conjunctival tumors in 181 cases. In the majority of cases they were basaliomas. In all cases the therapy was successful. No eyelid contractions or tear duct stenosis were noted. The follow-up of 4,5 years showed only 2 recidives. Histologically, cryonecrosis showed capillary ectasis with hyperemia.
