Changes in dental development in paediatric patients with chronic kidney disease.

Dental examination of 73 paediatric patients with nephrotic syndrome and 49 with chronic renal failure revealed enamel changes of permanent teeth in about 50%. They consisted usually of white discolorations and hypoplasia of enamel. In nephrotic patients they were observed particularly after the administration of high doses of corticosteroids. In patients with renal failure enamel changes occur very early. Their location was usually compatible with an insult on enamelisation at the time of manifestation of kidney disease. The prevalence of caries was almost twice as high in patients with nephrotic syndrome than in those with renal failure.

[Prosthetic aspects in the planning of progenia operations].

Operative planning for progenia is carried out using prosthetic principles. Correction of a malpositioned mandible, modelling of incongruent dental arches, functional distribution of piers, and a favorable interalveolar connecting line form the basis for stable occlusion and articulation. The fitting of a prosthesis later is simplified by such intensive interdisciplinary cooperation.

[Aspects of orthodontic-prosthetic planning in adults].

The introduction of firm fitting orthodontic appliances has increased considerably the treatment potential of adults. Many aesthetic and functional problems allow a more satisfactory solution through preparatory orthodontic treatment than was possible up to now by prosthetic means. Surgical measures can facilitate and complement this therapy. In addition dental orthopaedic measures must be stabilised by prosthesis for reasons of retention.

[Aspects of early treatment in cleft lip and palate patients].

The Heidelberg Concept of combined surgical otorhinolaryngological and orthodontic treatment of cleft patients starts immediately after birth with the incorporation of an orthopaedic plate. In a study on 38 children with complete unilateral and bilateral cleft lips and palates, maxillary alveolar arch morphology was analyzed by standardized photographs of casts. The importance of early treatment with plates for guided growth and functional coordination is demonstrated.
