Effect of calcitonin on gastric emptying and on serum insulin and gastrin concentrations after ingestion of a mixed solid-liquid meal in humans.

In a double-blind placebo-controlled study, we examined the effect of calcitonin on gastric emptying, and on serum concentrations of gastrin, insulin, glucose, calcium, and phosphorus after a mixed solid-liquid meal in 11 healthy men. Synthetic salmon calcitonin was administered as a 415 pmol i.v. bolus injection followed by a 90-min infusion to reach an overall dose of 62.25 pmol/kg body mass. Gastric emptying of a radiolabeled meal was surveyed by means of a gamma camera. A pronounced inhibition of gastric emptying with calcitonin was observed in all subjects (median gastric half emptying time 60.3 min after placebo versus 197.6 min after calcitonin; p less than 0.001). Calcitonin did not effect the postprandial gastrin release, nor did it change significantly the serum calcium or phosphorus concentrations. A decreased postprandial insulin release by calcitonin (mean +/- SEM area under the insulin curve 2,124.6 +/- 382.0 min mU L-1 after placebo versus 640.9 +/- 124.0 min mU L-1 after calcitonin; p less than 0.002) was accompanied by a different pattern of serum glucose concentrations during the infusion of the hormone when compared to the situation with a placebo. We discuss potential mechanisms and clinical relevance of our findings.

[Current state of knowledge about the application of cryotherapy for treatment of musculoskeletal diseases].

On the basis of literature data and his own investigations, the author discusses differential reactions of the human organism to cooling, the varieties of cryotherapeutic procedures, their therapeutic efficacy and current views on their mechanisms of action, and contraindications to their application. The present data indicate that the reflexotherapeutic mechanism plays a fundamental role in the analgetic action of cryotherapeutic procedures. The transient antiinflammatory influence of a single cryotherapeutic procedure does not undergo any additive effect during repetitive application of cooling. Regarding the facilitation of the kinesitherapy, cryotherapy appears a valuable means in the symptomatic management of inflammatory and noninflammatory diseases of the locomotor system.

[Effect of calcitonin on gastric emptying after isotope-labeled solid meal in patients with stomach ulcer].

The effect of calcitonin on gastric emptying of a radiolabelled test meal was examined in 8 male patients with active gastric ulcer. The patients ate the test meal twice, whilst during one of the examinations they were given synthetic salmon calcitonin (415 pmol i.v. bolus + continuous infusion during 90 min to reach an overall dose of 62.25 pmol.kg-1 body mass) and during the other one they received placebo--in randomized order, according to a double-blind study protocol. In every patient a pronounced delay in gastric emptying after calcitonin was observed --the emptying index, Ix: 2.33 +/- 0.22 x 10(-2) min-1 (placebo) vs 0.81 +/- 0.18 x 10(-2) min-1 (calcitonin), p less than 0.001. Calcitonin delayed and significantly lowered the postprandial gastrin release, as well as suppressed the postprandial insulin release with a secondary change in the serum glucose concentration pattern, whereas the serum calcium and phosphorus remained unaffected. The authors conclude that salmon calcitonin in a pharmacological dose elicits a strong inhibitory effect on gastric emptying in gastric ulcer patients.

[Effect of calcitonin on gastric emptying and postprandial secretion of gastrin and insulin in patients with duodenal ulcer].

The effect of calcitonin on gastric emptying of a radiolabelled test meal was examined in 10 patients with an endoscopically confirmed duodenal bulb ulcer. According to a double-blind study protocol, the patients were given on two different days placebo or synthetic salmon calcitonin (415 pmol i.v. bolus followed by a 90-min infusion to reach an overall dose of 62.25 pmol.kg-1 body mass)--in randomized order. Calcitonin did not affect the postprandial gastrin release nor did it change significantly the serum calcium or phosphorus concentration. The abolished postprandial insulin release by calcitonin was accompanied by a different pattern of serum glucose concentration, when compared to the situation with placebo. In all patients examined calcitonin evoked a profound delay in gastric emptying--the mean gastric transit time, MTT90: 34.1 +/- 1.4 min (placebo) vs 41.0 +/- 1.1 min (calcitonin), p less than 0.001.

[Subchronic toxic effect of the grain fungicide PF-70 on the electrolyte barrier of the gastric mucosa in rats].

The fungicide for corn PF-70 contains 21% of sulphur, 8.4% of mancozeb, and 0.6% of triadimefon . The aim of the study was to determine the effect of the fungicide on the gastric mucosa electrolyte barrier in rats. Mucosa lesion is known to be one of the risk factors of chronic peptic ulcer disease. The experiments were carried out on 80 rats of Wistar strain. The animals were divided into 5 groups--males and females, separately. The fungicide was given for 13 weeks, at the following doses: 100, 400, 1600, and 6400 mg of active substance per 1 kg of fodder. The gastric mucosa electrolyte barrier was examined by determining the transmural electric potential differences (PD) in mucosa before and after treatment with 150 mmlo of hydrochloric acid and 40 mmol of acetylsalicylic acid solutions. No changes in general condition of the animals were observed. An increase in fodder consumption was observed in males receiving the fungicide at doses of 1600 and 6400 mg per 1 kg of fodder. Females exhibited a decrease in body mass after intoxication at concentration of 6400 mg/kg fodder. A decrease in PD was found in all groups intoxicated with the fungicide, irrespective of the animal sex.

Effect of calcitonin on gastric emptying and on postprandial gastrin and insulin release in patients with type I gastric ulcer.

In a double-blind placebo-controlled study, the effect of calcitonin on gastric emptying and on serum concentrations of gastrin, insulin, glucose, calcium and phosphorus after a mixed solid-liquid meal was examined in six patients with type I gastric ulcer. Synthetic salmon calcitonin 415 pmol i.v. was given as a bolus followed by a 90-min infusion to reach an overall dose of 62.25 pmol.kg-1. Gastric emptying of a radiolabelled meal was measured with a gamma camera. Calcitonin suppressed gastric emptying in all patients examined. The mean gastric transit time, MTT90, increased from 38.1 +/- 0.4 min after placebo to 43.1 +/- 0.6 min after calcitonin (P less than 0.001). Calcitonin significantly blunted the postprandial gastrin release: AUC0-90 10,398 +/- 2886 ng. l-1 min (placebo) and 8238 +/- 2573 ng. l-1 min (calcitonin), P less than 0.05, and abolished the postprandial insulin release--AUC0-90 2244 +/- 230 mU.l-1 min (placebo) vs. 638 +/- 198 mU.l-1 min (calcitonin), P less than 0.01. A steady increase in the serum glucose during calcitonin infusion, reaching up to 5.6 +/- 0.31 mmol.l-1 at the end of the infusion, was observed. Calcitonin did not significantly affect serum calcium or phosphorus concentrations. The authors conclude that a delayed gastric emptying is to be expected in patients undergoing calcitonin treatment.

[Acetylation phenotype as a risk factor of diabetic neuropathy].

Acetylation phenotype was determined in 184 diabetics: 102 patients with Type I (insulin-dependent) and 82 patients with Type II (non-insulin-dependent) diabetes mellitus, who underwent complex investigations to assess the development of sensorimotor and autonomic neuropathy. The control group consisted of 121 healthy subjects of the same age distribution. The results obtained imply that the slow acetylation phenotype may be considered as a significant risk factor of diabetic neuropathy.

[Usefulness of determining the blood protein synthesis index in the evaluation of liver damage].

UNLABELLED: The purpose of report was the presentation of the variability range of the plasma protein synthesis index (PSI) in dependence on the chronic character of the liver parenchyma lesion in man or the extent of parallel indices of PSI with other biochemical parameters usually determined in the diagnostics of the liver parenchyma diseases. The studies have been performed in 20 patients with viral hepatitis (VH ), 20 patients with balanced liver cirrhosis (BLC), 20 patients with unbalanced liver cirrhosis and in 43 healthy volunteers. The PSI, total protein, albumin, gamma-globulin, bilirubin, prothrombin index and the alanine aminotransferase and the choline esterase activity has been determined. The most important lowering of the PSI values has been observed in patients with BLC. In those with VH the prolonged depression of the protein synthesis may indicate the chronic character of the inflammatory process. The extent of correlation of PSI with other parameters was statistically not significant. CONCLUSIONS: 1. PSI is a result of the liver lesion and its values are lowest in patients with MWN. 2. The WSB value does not depend on the age of subjects studied. 3. The diagnostic usefulness of WSB for evaluation of the liver parenchyma lesion is greater than of the albumin, choline esterase or prothrombin index determination. 4. The WSB determination is of value in the complex evaluation of the liver function in patients with lesions of that organ.

[The effect of pharmacologic therapy of hyperthyroidism on automatic heart conduction].

Investigations of the heart automatism were carried out in 10 patients with hyperthyreoidism before thiamazol therapy and after restoration of the euthyroid status. A control group was comprised of 34 healthy subjects. In order to determine the electrophysiologic parameters the transoesophageal atrium extrastimulus technique and the over-driving stimulation were applied. Restoration of the euthyroid status was accompanied by a significant prolongation of the atrial refraction, the border interval, the border conductivity and the sinus rhythm recurrence time. In a half of the patients were atrioventricular and intraventricular conductibility disturbances observed. The authors conclude, that in the early period of the euthyroid status restoration after thiamazole therapy there still exists a risk of cardiologic complications.

Effect of a synthetic prostaglandin E1 derivative, misoprostol, on fasted gallbladder volume and on meal-induced gallbladder emptying in man.

The effect of a single oral dose of 400 micrograms of misoprostol versus placebo on the fasted gallbladder volume and on meal-induced gallbladder emptying was assessed in 10 healthy subjects according to a double-blind study protocol. Gallbladder volumes were measured by means of real-time ultrasonography. The fasted gallbladder volume before drug administration was 18.9 +/- 1.1 cm3 on the placebo day, and 18.8 +/- 1.4 cm3 on the misoprostol day. No significant change in the gallbladder volume was observed 30 minutes after drug administration (18.3 +/- 1.3 cm3 placebo vs 20.2 +/- 1.8 cm3 misoprostol). The gallbladder volume 10 min after the test meal was significantly larger after misoprostol (16.9 +/- 1.7 cm3) than after placebo (12.8 +/- 1.6 cm3), p less than 0.02. However, the differences at subsequent time points (i.e. 20, 30, and 40 min after the test meal) proved to be insignificant. An insignificant effect of misoprostol on gallbladder emptying was revealed also by the analysis of the respective gallbladder ejection fractions. We conclude that misoprostol does not have any significant effect on fasted gallbladder volume or on meal-induced gallbladder emptying in humans.

[Effect of drug therapy of hyperthyroidism on the electrophysiological parameters of the heart].

Investigations of heart automatism have been performed in 10 patients with hyperthyreosis prior and after the euthyreosis obtaining with the methizol therapy. The control group consisted of 34 healthy volunteers. The electrophysiologic parameters of heart were determined by the transoesophageal stimulation of atrium using the method of single impulse and rapid stimulation. Instantly after obtaining the remission the normorhytmia has been observed; moreover, the significant elongation of the effective time of atium refraction and of the border interval, has been noted; the last two parameter were shorter than in the control group. The normalization of the border atrioventricular conduction and the sinusal rhythmus return as well as appearance in half of patients treated of latent block of intracardial conductivity has also been stated. According to the authors' opinion the early period after the obtaining of euthyreosis by the use of methizol is characterized by cardiological risk.

[Effect of calcitonin on gallbladder volume between food intake and on its emptying after meals in humans].

The effect of increasing i.v. doses of synthetic salmon calcitonin: 0.0044, 0.0088, 0.0175, and 0.0350 i.u..kg-1 min-1 versus placebo on the fasted gallbladder volume was assessed in 7 normal subjects according to a double-blind study protocol. In addition the action of calcitonin on meal-induced gallbladder emptying was examined. Gallbladder volumes were measured by means of real-time ultrasonography. An excellent day-to-day reproducibility of the fasted gallbladder volume was reflected by a coefficient of variation amounting to 12.2%, whereas the correlation coefficient between the fasted gallbladder volumes on day 1 and 2 amounted to r = 0.98, p less than 0.001. Calcitonin evoked a dose-dependent relaxation of the fasted gallbladder. A statistically significant increase of the fasted gallbladder volume was observed with 0.0175 (23.4 +/- 5.50 cm3 placebo vs 33.9 +/- 7.74 cm3 calcitonin, p less than 0.001) and 0.0350 (21.4 +/- 4.62 cm3 placebo vs 36.1 +/- 8.42 cm3 calcitonin, p less than 0.01) i.u..kg-1 min-1 calcitonin, where as mean increase of the gallbladder volume amounted to 32.1 and 46.5%, respectively. A significant delay of the gallbladder emptying after calcitonin was reflected by a decrease of the ejection fraction: 23.2 +/- 8.33% calcitonin vs 57.8 +/- 6.94% placebo (p less than 0.02) at 20 min, and 40.5 +/- 8.76% calcitonin vs 67.2 +/- 3.75% placebo (p less than 0.02) at 30 min after the test meal. Calcitonin is concluded to increase significantly the interdigestive gallbladder volume and to delay the meal-induced gallbladder emptying in humans.

Effect of calcitonin on gastric emptying.

The effect on gastric emptying of synthetic salmon calcitonin administered intravenously (415 pmol bolus injection followed by infusion to reach an overall dose of 62.25 pmol.kg-1 body mass) versus placebo was examined in 13 healthy men. Gastric emptying of a radiolabeled solid meal was assessed with a gamma camera. The gastric emptying course was analyzed with the use of the power-exponential fitting. A marked delay in gastric emptying was observed in all subjects studied: the median gastric half emptying time was 60.3 min after placebo and 196.5 min after calcitonin (p less than 0.001). At the same time, analysis of the parameter S revealed that calcitonin did not elicit any consistent change in the shape of gastric emptying curves.

A pharmacological dose of glucagon suppresses gastric emptying of a radiolabelled solid meal in humans.

The effect of glucagon (143 nmol i.v. bolus followed by 430 nmol infused at a constant rate over 90 min) vs placebo (normal saline) on gastric emptying was examined in a blind randomized study in eight healthy males. The gastric emptying of a radiolabelled solid meal was measured with the use of a gamma camera. Glucagon elicited a pronounced delay in gastric emptying in all subjects examined--mean gastric transit time MTT90 glucagon 44.2 +/- 0.22 min vs placebo 38.6 +/- 0.74 min, p less than 0.001.
