[Interdigestive gallbladder volume and and meal-induced gallbladder emptying in patients with diabetes].

The aim of the study was to find whether the type of diabetes and the duration of this disease causes any influence on gallbladder (GB) emptying, after exclusion of possible effect of obesity and a hyperglycaemia over 7 mmol/l (126 mg%) on this phenomenon. The investigations were carried out in 44 subjects with diabetes mellitus, among them 22 with type I and 22 with type II. These groups were divided according to disease duration: shorter or longer than 3 years. Two control groups (CI, CII) comprised each 20 healthy subjects. The age of CI and CII corresponded to the age of diabetes groups type I and II, respectively. All subgroups as well as the control groups were matched according to sex percentage structure, body mass index (BMI) and body surface (BS). In groups with type I and II diabetes, the number of cases with diabetic neuropathy was comparable. There was not found any significant (p > 0.05) influence of diabetes on the interdigestive GB volume, neither non-standardized nor standardized with regard to standard BS (1.73 m2). In comparison to the control groups, after a test meal, there was in all diabetic groups a statistically significant (0.001 > p < 0.05) reduction of GB emptying observed. This diminution was most expressed in the group with long-lasting type I diabetes.

Serum amino-terminal propeptide of type III procollagen in patients with hyperthyroidism or hypothyroidism during therapy.

Changes in serum amino-terminal type III procollagen propeptide (PIIIPN) and hydroxyproline were compared with indices of thyroid function in 33 patients with hyperthyroidism and 16 patients with hypothyroidism before and after treatment. Control values were obtained from 26 healthy individuals. Hyperthyroidism was shown to be associated with an increased level of PIIIPN and hypothyroidism with a decreased PIIIPN level. An increase in PIIIPN was found in 100% of hyperthyroid patients and a decrease in 31% of those with hypothyroidism, while hydroxyproline increased in only 54% of patients with hyperthyroidism and its level was decreased in only 6% of patients with hypothyroidism. Correlation between PIIIPN and thyroxine and triiodothyronine levels was shown. The results obtained suggest that PIIIPN is a valuable index of tissue response to thyroid hormones.

[Effect of misoprostol, a synthetic prostaglandin E analog, on levels of serum C-peptide in serum and glucose tolerance].

The aim of the study was to investigate whether oral application of misoprostol (MI), which is an analogue of prostaglandin E1, does change the secretion of insulin and the blood glucose level. The investigations were carried out in 15 subjects, aged 21-40 yrs, with a gastric or duodenal ulcer and without any disturbances of the digestive tract motility. In 7 of them (group A), without stated diabetes a single oral dose of 400 mg MI versus placebo (PL) and 15 min later 75 g glucose p.o., was applied in randomized order, on different days, in fasting state. In 8 subjects with mild diabetes type II (gr B) MI versus PL and after 15 min. i.v. 1.0 mg glucagon was similarly administered. In both groups the concentrations of glucose and C-peptide in blood were determined. In comparison to PL, the application of MI did not cause any statistically significant differences of C-peptide in serum and blood glucose levels neither before and after oral glucose loading nor after i.v. administration of glucagon. Statistically not significant were also the differences in AUC (p > 0.05).

[Influence of prolonged application of captopril upon pulmonary ventilation in patients with chronic congestive heart failure].

Study was designed to investigate the effect of prolonged application of captopril on pulmonary ventilation in patients with chronic congestive heart failure (CCHF) without hypertension. Studies were carried out in 13 men aged 54-65 yrs with CCHF due to ischaemic heart disease. They were classified hemodynamically as III or IV stage according to the NYHA classification. Two periods of management were investigated. In the 1st period, lasting at least one month, cardiac glycosides, diuretics, nitrates, as well as diet and restricted physical activity were applied. In the 2nd period, lasting 21 days, additionally captopril was given in a daily dose of 2 x 12.5 or 2 x 25 mg depending on the drug tolerance. When the periods were terminated pulmonary ventilation was measured. The following indices were determined: FEF25, FEF50, FEF75, FEF25-75, FEV1, FVC,FEV1/FVC, SVC, FIVC, FIF25, FIF50, FIF75, FIF25-75. No significant effect of captopril upon pulmonary ventilation was found (p < 0.05). In any of the patients captopril-induced cough was not observed. It is concluded that prolonged management with captopril in patients with CCHF without hypertension has no disadvantageous influence upon pulmonary ventilation with exception of patients who are hypersensitive to the drug.

[Influence of chronic intoxication with phospho-organic insecticide dimethoate on the hemopoietic system in rats].

Toxic effect of insecticide dimethoate applied to 400 rats for two years in concentrations of 2 to 200 mg/kg fodder was investigated. During the last six months of observation the toxic influence of the larger doses of this preparation was manifested by a higher mortality of the animals. Neither disadvantageous influence upon the maturation of each individual haemopoietic series, nor action inducing pathologic proliferation in the haemopoietic system of rats were found.

[Effect of maprotiline on bronchodilation in patients with bronchospasm].

The bronchodilatory effect of maprotiline (Ludiomil, firm Ciba-Geigy) was investigated in 20 patients with bronchospasm (FEV %below 60%). The effect of a single dose of 75 mg maprotiline was compared with that produced by 500 mg of aminophylline, applied each time in 500 ml normal saline during 90 min, on two different days, in randomized sequence, always at the same day time. Prior to and at 5, 10, 20, 30, 60, 90, 120, 150 and 180 minutes after commencement of the infusion the following parameters of pulmonary ventilation have been determined: VC, FEV1, FEV1%., MEFR and MBC. The stated differences between the efficacy of maprotiline and aminophylline were statistically not significant (p > 0.05). In 18 patients maprotiline caused sedation and even somnolence. The authors conclude that maprotiline is an efficacious bronchodilator in patients with bronchospasm and is well tolerated by the patients.

[Influence of progressive systemic sclerosis on gastric emptying].

Gastric motility was measured with an isotopic method in patients with progressive systemic sclerosis (PSS). Twenty-six women aged 48 +/- 10 yr with PSS were investigated. The duration of PSS prior to the study ranged from 2 to 25 yr. Control values were obtained from 12 healthy female volunteers aged 34 +/- 8 yr. Endoscopic examination was carried out in all investigated individuals to exclude abnormalities that were non-related to PSS. Measurements were done at morning, after an overnight fast. Patients and controls received a test meal of 1650 kJ containing human albumin microspheres labelled with 7.4 MBq of 99mTc. Subjects were investigated with a gamma-camera. It was shown that gastric emptying (GE) t 1/2 was delayed in the patient with PSS (289 +/- 325 min) as compared to the controls (92 +/- 19 min), p < 0.001. Delayed t 1/2 over x + 2SD of the control group was found in 69% of patients with PSS while in the control group O. The mean transit time (MTT90) was in the PSS group significantly longer than in the control group, 42.3 +/- 1.4 and 40 +/- 0.9 min respectively (p < 0.001). There was no correlation found between GE t 1/2 and the time of PSS duration (r = 0.08), its type II or III as well as the titer of Scl-70 antibodies in blood serum (r = 0.29, p > 0.1).

[Volume of the functioning gallbladder and its emptying in patients with progressive systemic sclerosis].

UNLABELLED: The aim of the study was to measure the gallbladder volume (GV) and its emptying after meal stimulus in patients with progressive systemic sclerosis (PSS). 18 women with PSS, aged 49.8 +/- 9.5 yr, participated in the study. Control values were obtained from 30 healthy women aged 26.5 +/- 3.5 yr. The duration of PSS symptoms ranged from 2 to 25 yr. Subjects with gallstones or cholecystitis as well those receiving drugs affecting the gastroduodenal motor function were excluded from the study. GV was measured according to Dodds et all (1985) with the real-time sonographic apparatus (Sonoline SL Siemens, Germany), after an overnight fast and after a test meal. Measurements were repeated in 10 min intervals through a 40 min duration of the study. Similar volumes of fasted gallbladder were found in the PSS group and in the control group: 25.4 cm3, SEM 3.0 (PSS), vs. 20.9 cm3, SEM 1.6 (control), p > 0.05. There was not any correlation between the fasted GV and duration of the disease symptoms. The test meal evoked in both groups a similar trend of the decrease in the GV during 40 min and the increase in the ejection fraction at this time. Although the changes observed were statistically not significant (p > 0.05) there were small differences in the gallbladder ejection fraction in two patients, not exceeding 14% and 18%, and being apparently below of any individual value in the control group. CONCLUSION: the gallbladder motor function is very rarely disturbed in patients with PSS.

Effect of cimetidine and ranitidine on absorption of [125I]levothyroxine administered orally.

Female patients with a simple goiter were pretreated on 2 occasions (at an interval of 4 wk) with po placebo or 400 mg cimetidine (Cim) (Group A, n = 10), or with placebo or 30 mg ranitidine (Ran) (Group B, n = 10), 90 and 150 min, respectively, prior to the po gelatin capsules containing [125I]levothyroxine ([125I]LT4). A double-blind randomized study protocol was kept. Venous blood samples were taken at 15, 30, 45, 60, 75, 90, 105, 120, 150, 180, 210, and 240 min after po [125I]LT4 and the radioactivities in serum were counted. Similar [125I]LT4 radioactivities were found after placebo pretreatment in both groups: AUC 467 +/- 82 in Group A vs 459 +/- 109 in Group B. Cimetidine decreased the serum [125I]LT4 radioactivities: AUC371 +/- 72 (Cim) vs 467 +/- 82 (placebo) (P < 0.01), but Ran did not: AUC 477 +/- 132 (Ran) vs 459 +/- 109 (placebo) (P > 0.05).

[Influence of treating hypothyroidism and hyperthyroidism upon psychical reaction time].

The aim of the study was answering the question whether determination of psychical reaction time may be useful for the monitoring of treatment of hypothyreosis and hyperthyreosis. The correlation of disease symptoms (diagnostic index for hypothyroidism after Murray and for hypothyroidism according to Crooks), concentration of triiodothyronine, thyroxine, index of free thyroxine in blood serum and Achilles tendon reflex--with the simple reaction time (srt) as well as with the choice reaction time (chrt) before and during treatment was investigated in 18 patients with primary hypothyroidism and in 24 with hyperthyroidism caused by Graves-Basedow's disease. Two control groups comprised 84 healthy persons. Either in hypothyroidism or in hyperthyroidism the srt and chrt was significantly prolonged. The substitution therapy in hypothyroidism resulted in a normalization of both parameters. On the other hand, administration of thiamazole, especially in the early period of treatment of hyperthyroidism, further prolonged the srt and chrt. A shortening of the time of each of the two reactions occurred, however, when the restoration of euthyroidism was achieved, which was accompanied by reduction of thiamazole dosage. The obtained data point to the usefulness of srt and chrt determination for the diagnosis and therapy monitoring of functional disturbances of the thyroid gland.

[Effect of diabetes mellitus type 1 on electric resistance of the skin and galvanic skin response].

Basal electrical resistance of the skin and the galvanic skin response were determined in 102 patients with type I (insulin-dependent) controlled diabetes mellitus. The control group consisted of 121 healthy subjects of the same age and sex distribution. The results obtained imply that diabetes mellitus causes an increase of electrical resistance of the skin, a prolongation of the latency time and or even lacking of the galvanic skin response.

Effect of cold stress on gallbladder contractility in humans.

The effect of cold stress in the form of repeated hand immersion in ice cold water or repeated application to a hand of extremely cold - nitrogen gas, at a temperature of -180 degrees C (93 K), on the emptying of a meal-stimulated gallbladder was examined in seven healthy volunteers of both sexes. The control procedure consisted of repeated application to a hand of air at neutral temperatures (+35 degrees(-)+37 degrees C). Each subject underwent three examinations on separate days, the procedures being applied in random order. Gallbladder volume was measured by means of real-time ultrasonography. Measurements of volumes of the fasted gallbladder were also made on the three study days and gave similar values: 20.3 cm3, SEM 2.7 (control), 22.1 cm3, SEM 2.5 (cryotherapy), and 20.0 cm3, SEM 2.5 (cold immersion), F(2, 12) = 2.47, P greater than 0.1. A significant delaying effect of cold stress on postprandial gallbladder contractility was observed: F(2, 72) = 7.44, P less than 0.005, and F(2, 72) = 4.45, P less than 0.025 for gallbladder volume and ejection fraction, respectively. A significant difference was detected between the local application of cold gas and the control procedure (P less than 0.005) and local cryotherapy and the cold pressor test (P less than 0.05) in the case of the postprandial gallbladder volume, as well as between the local application of cold gas and the control procedure (P less than 0.025) for the ejection fraction.(ABSTRACT TRUNCATED AT 250 WORDS)

[Effect of work load on arteriosclerosis risk factors in employees of 2 Upper Silesia industries].

Investigations were undertaken to estimate various energy expenditure during professional work on incidence of hypertension, overweight, obesity, hyperuricaemia and hypercholesterolemia in workers of two enterprises in Upper Silesia. The study was carried out in 1957 individuals (1392 male, 565 female) divided into 5 groups according to the hardness of their work. Serum level of uric acid and cholesterol was determined in all investigated subjects. Incidence of hypertension, overweight and obesity was estimated. It has been stated that incidence of hypertension, hypercholesterolaemia, overweight and obesity in men and hyperuricaemia in both sexes decreased with hardness of the work. The incidence of hypertension, overweight and obesity was not related to hardness of the work in women. The authors conclude that physical activity associated with professional work can be much more in men than in women listed as a preventive measure against arteriosclerosis.
