Cytophotometric investigations of DNA-content in transitional cell tumors of the bladder. Comparison of results with clinical follow up.

Forty-nine patients with transitional cell tumors of the urinary bladder were followed up clinically for a period of at least 36 months (average time 61.4 months). Initial tumors were examined by means of DNA-Feulgencytophotometry. Histologic grading was performed according to Bergkvist et al. (1965). Clinical behavior clearly demonstrates the validity of the grading schedule used and shows the significance of DNA-Feulgencytophotometry for the standardization of bladder tumors and for the comparability of diagnostic and therapeutic results. Determination of the individual prognosis of low-malignancy tumors and the differentiation of histologic borderline cases does not appear to be possible by means of DNA-Feulgencytophotometry.

[Carcinoma in situ of the urinary bladder: a diagnostic and therapeutic challenge].

Carcinoma in situ (Tis) of the urinary bladder means a challenge for the urologist, because of the uncharacteristic symptoms and the various treatment modalities. Early detection requires wide use of urinary cytology and multiple cold-cup biopsies. Cytology has proven to be sensitive in 95%. Histological grading of the Tis and the concomitant overt tumor is in accordance in 85%. A dedifferentiation of the tumor during the course of the disease was observed in only 14%. Modes of conservative treatment can achieve long-lasing tumor control or remission. Therefore, a cystectomy cannot be justified as the primary treatment in any case.

[Percutaneous nephrostomy in ureteric fistulas following antireflux operations (author's transl)].

Ureteric fistulae after reflux operations are rare. The percutaneous nephrostomy is a minor procedure and best suited for temporary urinary diversion. The interval before further surgery is about two months. The area of the ureteric fistula is not to be opened and the ureteric defect is bridged by the psoas hitch technique. A spontaneous closure of the fistula during urinary diversion can be observed.

[Adenocarcinoma of the bladder (author's transl)].

During a 10 year follow-up, 18 patients with primary adenocarcinoma of the bladder or urachus were treated operatively. We report on the therapy and results. The criteria for differentiation between primary adenocarcinoma of the bladder and urachal carcinoma are stressed. Chronic irritation of the urothelium is most likely involved in the natural histroy of primary adenocarcinoma. Cystectomy should be the therapy of choice for the adenocarcinoma of the bladder. A partial resection is recommended for urachal carcinoma. Palliation is possible with cytotoxic agents. Irradiation seems to be of no beneficial effect on tumor growth.

[Stauffer's syndrome. Reversible hepatic dysfunction in renal cell carcinoma (author's transl)].

Stauffer's syndrome represents a paraneoplastic liver disorder associated with renal cell carcinoma and is characterized by elevation of the serum alkaline phosphatase, increased bromsulphthalein retention, hypalbuminaemia, elevation of alpha-2-globulin and hypoprothrombinaemia, as well as hepatosplenomegaly. Two cases are reported in which this syndrome was the presenting feature and operation was undertaken on the basis of suspected primary biliary tract disease. The aetiology of the typical findings of Stauffer's syndrome are discussed. As they may be the only symptoms of an otherwise occult renal cell carcinoma, their presence should guide the diagnostic efforts in the right direction. Moreover, the possibility of predicting the postoperative course by follow-up control of the liver function tests is stressed.

Urachal signet-ring cell carcinoma, a rare variant of vesical adenocarcinoma: incidence and pathological criteria.

Of 715 cases of vesical tumors reviewed 18 adenocarcinomas were selected for further study, 5 of which fulfilled all criteria of urachal origin. Herein is reported 1 rare case of such a carcinoma of the signet-ring cell type and the morphological criteria of this variant are discussed briefly. After undergoing radical cystectomy and ureterosigmoidostomy the patient has been free of recurrent tumor or metastasis for 6 years.

Further experiences with the specific red cell adherence test (SRCA) in bladder cancer. a histological and cytological study.

The red cell adherence test allows the disclosure of the blood group antigens incorporated on the surface of the urothelium. In the course of dedifferentiation the urothelium will lose its antigenicity. We applied this test on superfical bladder tumours (T1), carcinoma in situ and cytological specimens and observed a close correlation between the test results and the course of the disease, e.g. if the test was positive the recurrent tumour would be superficial and vice versa. As a rule the negative results revealed atypical hyperplasia and carcinoma in situ as areas of potentially malignant and invasive tumours. With the application of the red cell adherence test to cytological preparations we can probably improve the accuracy of the cytological examination in cases of transitional cancer of low malignancy.

[HL-A antigenicity in bladder tumors].

30 patients with papillary transitional bladder tumours were treated by TUR. The stage O and A tumour specimens obtained were examined for presence or loss of antigenicity by the specific red cell adherence test (SRCA). The patients were followed up between 4 and 17 years. A correlation between loss of antigenicity and secondary occurrence of tumours with more aggressive biological behaviour was apparent. Possible therapeutic consequences are discussed.

Radioimmunoassays of ethinyl-norgestrienone (R-2323) and medroxyprogesterone acetate (MPA) and their clinical applicability.

Radioimmunoassays for 2 synthetic progestins (Ethinyl-norgestrienone, R 2323 and medroxyprogesterone acetate, MPA) are demonstrated. 10 patients aged 31 to 72 years were treated with ethinyl-norgestrienone with different schedules and 3 men suffering from benign prostatic hypertrophy were treated with medroxygesterone acetate. Plasma levels of testosterone, LH, FSH were monitored before, during and after treatment.

[Winter sports injuries of the urogenital tract (author's transl)].

During 1964-1974 112 injuries of the urogenital tract caused by winter sports were treated at the University Hospital Innsbruck, Department of Urology. Eighty-eight patients suffered skiing injuries, 20 tobogganing injuries, and one injury each was caused by ski jumping and bobsleighing accidents, two traumas resulted from a fall from a chair lift. On the basis of typical case reports the most common types of trauma of the urogenital tract are demonstrated and the basic mechanisms of the accidents are discussed. Particular attention is paid to the obvious increase of lesions of the external genitalia and the urethra in the last few years caused by the so-called spinning ski, as well as the frequency of kidney traumas, especially in winters with little snow. Tobogganing accidents caused injuries to the kidneys as well as to bladder and urethra. In contrast to traumas caused by skiing, tobogganing injuries were mostly multiple. Analysis of patients records shows an increase of these injuries, which were really not typical for winter sports. The possible reasons as well as their prevention are discussed.

[Is skeletal alkaline phosphatase a valid staging marker in detection of osteoblastic skeletal metastases of prostate carcinoma?].

PURPOSE: For patients with prostate cancer (CaP) the proof of osteoblastic bone metastases is decisive regarding the prognosis as well as the therapeutical concept. To evaluate the efficiency of skeletal alkaline phosphatase (SAP) as staging marker for bone metastases in prostate cancer, SAP was measured in CaP-patients with and without bone metastases compared with prostate-specific antigen (PSA) as the marker of choice till now. METHOD: 73 patients with histological proven, but still untreated CaP were entered into the study. After staging the patients were divided into 3 groups: group I: patients with CaP and bone metastases (n = 21), group II: patients with locally advanced CaP without bone metastases (n = 26), group III: patients with clinically localized CaP without bone metastases (n = 26). Serum concentration for SAP and PSA were determined using radioimmunassay. As reference range we defined serum concentrations for SAP < 19 ng/ml and for PSA < 100 ng/ml. RESULTS: 71% of the patients with bone metastases (group I) showed elevated SAP and PSA serum concentrations. In contrast, patients without bone metastases (group II + III) have normal SAP-values (<19 ng/) and in 19% of the cases elevated PSA-values (>100 ng/ml). This resulted in a sensitivity and specificity of 71% and 100% for SAP and 71% and 81% for PSA. The positive predictive value for osteoblastic bone metastases was 100% for SAP and 60% for PSA. CONCLUSION: SAP is a useful staging marker in prostate cancer and can contribute for an early detection of osteoblastic bone metastases.
