Increased lymphatic pumping ability in pregnant ewes.

OBJECTIVE: Daily the lymphatic system returns several liters of fluid to the systemic circulation. Unlike blood, lymph is pumped against a pressure gradient. Because vessels in the systemic circulation are hyporesponsive to vasoconstrictor hormones during pregnancy, we decided to assess whether lymphatic pumping ability was decreased during pregnancy. STUDY DESIGN: Ten pregnant ewes were chronically catheterized in the left thoracic duct and jugular vein. Four days after surgery thoracic duct pumping ability was determined by measuring the lymph flow rate as a function of outflow pressure by varying the height at which lymph flow drains in an open-flow system. RESULTS: Lymph flow was plotted against outflow pressure, and the value at which lymph flow decreases is defined as the "breaking point." The outflow pressure at which lymph flow equals zero is the "stopflow" pressure; this value defines the pumping ability. The pregnant ewes had a breaking point of -1 +/- 2.7 versus 14.4 +/- 3.0 mm Hg (mean +/- SD) for the nonpregnant sheep. Stopflow pressures were 62 +/- 7.3 versus 23.9 +/- 4.7 mm Hg (mean +/- SD) for the nonpregnant ewes (p < 0.05). CONCLUSIONS: The ability of the lymphatic system to generate pressure is increased during pregnancy, but the breaking point is diminished. Alterations of lymphatic pumping ability could explain fluid retention under pathophysiologic conditions.

What is nursing ethics?

This article tries to bring some clarity into the current confusion about the nature of nursing ethics. It distinguishes between the empirical, prescriptive and analytic dimensions of ethics. It then describes six contemporary understandings of the parameters and aims of ethics via six different approaches: moral education, management, professional conduct, bioethics, clinical ethics and political ethics.

Biochemical and physiological characterization of the efrotomycin fermentation.

An efrotomycin fermentation was characterized through physical, chemical and biochemical studies. Growth of the actinomycete, Nocardia lactamdurans occurred during the first 50 h of the fermentation cycle at the expense of glucose, protein, and triglycerides. The initiation of efrotomycin biosynthesis was observed when glucose dropped to a low concentration. Upon glucose depletion, cell growth ceased and a switch in the respiratory quotient occurred. Efrotomycin biosynthesis was supported by the utilization of soybean oil and starch. Analysis of triglyceride metabolism showed that no diglycerides or monoglycerides accumulated during the fermentation. The activity of extracellular enzymes (lipase, protease, and amylase) increased during the cell growth phase and decreased significantly after 150 h. The concentrations of DNA, tetrahydro-vitamin K2 (a membrane component), and free amino acids in the supernatant increased dramatically late in the fermentation cycle (225 h), indicating massive cell lysis. During this same time period, a reduction in cellular respiratory activity and efrotomycin biosynthesis were observed.

Schizophrenia and indeterminacy: the problem of validity.

The paper attempts to account for the confusion over the validity of the concept of 'schizophrenia' in terms of two closely related aspects of conceptual indeterminacy. Firstly, it is identified on the basis of a breakdown in intelligibility, but what constitutes such a breakdown is indeterminate. Secondly, the concept sits between the categories of natural disease or illness on the one hand, and character trait or moral failing or gift on the other. This entails an indeterminacy in attempting to define the role that physiological explanation could have. Light may be thrown on the concept by exploring a distinction between a life story in which the schizophrenic condition emerges as the conclusion of the story and a causal process in which the condition is the end result or final consequence.

Arteriogram scoring systems and Pulse Generated Run-off in the assessment of patients with critical ischaemia for femorodistal bypass.

Conventional arteriography is the investigation of choice in patients with critical ischaemia to select the site for the distal anastomosis of a femorodistal bypass. Several arteriographic scoring systems have been devised in an attempt to quantify the run-off. More recently Pulse Generated Run-off and intraoperative peripheral resistance measurements have been proposed as better predictors of early graft patency. Eighty-eight consecutive femorodistal bypass grafts were studied using Pulse Generated Run-off (PGR), conventional and intra-arterial digital subtraction arteriography (DSA) and peripheral resistance measurement. Three widely used arteriographic scoring systems were used to grade the arteriographic run-off. There were significant correlations between the scoring systems and the measured peripheral resistance, although PGR correlated better with peripheral resistance than arteriography (rs = -0.59, P less than 0.001). Intra-arterial DSA improved the correlation between the arteriogram scoring systems and the measured peripheral resistance in the distal popliteal artery, but not in single calf vessels. In contrast, PGR correlated much better with peripheral resistance in single calf vessels (rs = -0.82, P less than 0.001). We recommend that selection for femorodistal reconstruction be based on a combination of intra-arterial DSA and PGR. PGR will identify patent distal calf vessels not seen on arteriography and determine their continuity with the pedal arch, two features which are essential prerequisites for successful femorodistal bypass.

The role of laminin in cancer invasion and metastasis.

Laminin, a basement membrane glycoprotein, has been implicated in a number of stages in tumour invasion and metastasis. In addition to its roles in cell adhesion and migration, laminin may be important in mediating interactions of tumour cells with the immune system and have more subtle roles in controlling metastatic behaviour. Recent work on the prognostic significance of laminin and the possible role of the molecule in antimetastasis therapy will also be discussed.

Spontaneous sister chromatid exchange in metastatic variants of the murine B16 melanoma and human astrocytomas in culture.

Three metastatic variants, BL6 (high metastasis), F1 (nonmalignant) and F10 (intermediate malignancy) of the B16 murine melanoma, and a pulmonary metastatic line BL6-ML8 of the BL6 primary tumour have been examined for spontaneous sister chromatid exchange (SCE). Two human astrocytoma cell lines were also examined. SCE was encountered in 29 and 13% of second division metaphases of BL6 and F10. In contrast, only 3% of second division metaphases of the F1 showed SCE. In BL6-ML8, 40% of the metaphases showed SCE. Approximately 2-4% of the human astrocytoma second division cells showed SCE. The variant lines were karyotypically heterogeneous. The pattern of cell distribution according to chromosome number showed an overall similar profile in the melanoma variants. However, the metastatic BL6-ML lines showed a marked shift to a hypertriploid state. SCEs occurred with higher frequency in this hypertriploid subpopulation of BL6 and F10 cells than in F1. SCE incidence in the hypertriploid subpopulation was twofold higher in the metastatic line than in the primary BL6 line. The number of SCEs per chromosome was twice as high in F10, BL6 and BL6-ML8 as in the F1 cells. This hypertriploid subpopulation showed a marked increase of SCEs on exposure to mitomycin C and ethyl methane sulphonate, indicating their mutability. It is suggested that the parallelism between SCE and metastatic potential may be relevant in the context of the generation of the metastatic phenotype.

Glucocorticoids and the cell surface of human glioma cells: relationship to cytostasis.

The glucocorticoid hormones methyl prednisolone and dexamethasone were shown to be cytostatic, but not cytotoxic, at high cell densities for early passage and continuous cell lines from human glioma at 0.25 microM and above, in the presence or absence of serum. In the absence of serum both steroids at 2.5 nM increased the saturation density close to the level reached in serum. Examination of the iodinated glycoproteins of the cell surface by gel electrophoresis did not reveal any consistent change. However, gel exclusion chromatography of protease digests of the cell surface and of material released into the medium showed an increase in incorporation of 3H-glucosamine in pronase digests after treatment with methyl prednisolone. Ion exchange chromatography showed that sulphated glycosaminoglycans, particularly heparan sulphate, increased and hyaluronic acid decreased in response to steroids, and there was increased retention of GAGs on the cell surface relative to the released fraction. It was concluded that glucocorticoid hormones modify the cell surface of human glioma cells and that this may contribute to enhanced cell intraction and lead to increased density limitation of cell proliferation.

Dexamethasone suppression test (DST) and plasma dexamethasone levels in depressed patients.

The dexamethasone suppression test (DST) was evaluated in newly hospitalized patients with a DSM-III diagnosis of major depression. Patients with other psychiatric disorders and a normal control group were also studied. Plasma dexamethasone levels were obtained in all patients, and the relationship between plasma cortisol and plasma dexamethasone was examined. Rates of non-suppression in patients with major depression (39%) were not significantly different from those in patients with minor depression (25%), mania (38%), or other psychiatric illnesses (17%). The ranges of dexamethasone levels at 8 a.m. and 4 p.m. were similar between patient groups and controls. However, there was a significant difference in dexamethasone levels between suppressors and nonsuppressors, irrespective of diagnosis, which could not be explained by differences in weight or plasma dexamethasone half-life. Inappropriately high dexamethasone levels were found in some patients with a 1 mg test, a problem that critically affects the sensitivity of the test procedure.

Impaired opsonophagocytosis of serotypes Ib and II of group B streptococci as compared with serotypes Ia and III: role of the alternative pathway of complement in opsonisation of serotype III of group B streptococci.

Using the technique of phagocytic chemiluminescence, we have shown that serotypes Ib and II of group B streptococci are resistant to opsonophagocytosis. The resistant strains became susceptible to opsonophagocytosis by trypsin treatment, but neuraminidase had no effect. Several studies have failed to define a significant role for the alternative pathway of complement in opsonisation of group B streptococci. By simple chelation and heat inactivation studies, we have shown that the alternative pathway of complement is activated by serotype III of group B streptococci.
