Coping with a stoma - a comparative study of patients with rectal carcinoma of inflammatory bowel diseases.

409 patients who had either an implantation of a stoma or a bowel resection for either colorectal carcinoma of colitis/ileitis answered a 200-item questionnaire. A discriminant analysis revealed a higher degree of pre- and postoperative depression and a decrease of social activities for stoma patients who also more often lived in rural areas in comparison with patients with the same diagnosis who only had a resection. A comparison of operated cancer patients with patients operated for ulcerative colitis or Crohn's disease showed less depression before and after surgery in cancer patients. Cancer patients also frequently ceased sexual activities after the operation, and they came from much more remote rural areas than the comparison group, irrespective of the type of operation performed.

Studies on the biosynthesis of bacterio-opsin. Demonstration of the existence of protein species structurally related to bacterio-opsin.

1. The kinetics of processing newly synthesized bacterio-opsin from the non-crystalline state within the brown membrane to the crystalline state within the purple membrane was followed by pulse-chase experiments. 2. Biosynthesis of bacterio-opsin was found to be highly resistant to RNA-synthesis inhibitors like rifampicin and ethidium bromide. In the presence of ethidium bromide, only five protein species continue to be synthesized in halobacteria, one of them being bacterio-opsin. 3. In spheroplasts, synthesis of bacterio-opsin is found to be selectively disturbed. The purple membrane isolated from spheroplasts contains new, additional protein species with apparent molecular weights of 19 000, 23 000 and 29 000. These proteins share common amino acid sequences with bacterio-opsin. 4. In the halobacterial cell membrane, two membrane proteins with apparent molecular weights of 30 000 and 36 000 were detected which are structurally related to bacterio-opsin. 5. Bacterio-opsin as well as the 30 000 Mr and 36 000-Mr proteins contain covalently bound sulphate.

[Benign tumors of the liver-diagnosis and therapy (author's transl)].

From 1971 to 1976 surgery was done on 11 patients with benign tumors of the liver in the Surgical Department of the University of Mainz. Histological examinations revealed focal nodular hyperplasia in 5 cases, liver cell adenoma in 2 cases, caverneous hemangioma in 2 cases, cystic liver disease in 1 case, and an idiopatic peritoneal liver cyst in 1 case. Benign tumors of the liver are rare. Clinical symptoms in these cases are inconspicuous. In 5 patients the diagnosis was made coincidentally. Preoperative diagnosis depends mainly on angiography. Since an exact histological diagnosis cannot be made otherwise, surgery is imperative. Big tumors leading to displacement of intestinal or biliary organs ought to be removed in toto; this allows complete histological work up and exclusion of malignancy, and it does prevent recurrence of the tumor as well. In 8 of 11 patients tumors localized peripherally in the liver could be removed surgically without complications.

[Longterm results of mitral valve replacement (author's transl)].

210 patients were followed up by the actuary method for over 5 years after isolated mitral valve replacement or a double valve replacement. After isolated valve replacement the one month survival including the operative mortality was 92+/-2%. The survival after one year was 83+/-3% and after 5 years 66+/-7%. The five year survival of patients in preoperative class III (according to the NYHA) was 73+/-8% and of class IV 57+/-8% (P less than or equal to 0.1). A comparison of valve replacements for pure mitral stenosis or mitral insufficiency showed no statistically significant differences. In the 37 patients who had a double valve replacement the survival risk was not increased in comparison with those patients who had had a single valve replacement. Age above 45 years and a preoperative markedly raised pulmonary arteriolar resistance reduced the chances of survival.

Fluorination of 6-methyluracil and its nlcleosides.

6-Methyluracil and its perbenzoylated 1-(beta-D-ribofuranosyl) and 1-(2-deoxy-beta-D-ribofuranosyl) derivatives afford on treatment with elemental fluorine in acetic acid solutions, the corresponding derivatives of 5-fluoro-6-methyluracil and 5-fluoro-6-fluoromethyluracil. The free nucleosides have been obtained from the protected derivatives by methanolysis. The CH2F linkage in 5-fluoro-6-fluoromethyluracil derivatives is stable towards hydrolysis and nucleophilic agents.

[Echinococcus alveolaris and cysticus of the liver].

From 1964-1976 34 patients were treated for Echinococcus of the liver at the Mainz University department of surgery. There were 25 cases of E. cysticus and 9 cases of E. alveolaris. Decisive for the diagnosis are Casoni's intracutaneous test, scout film of the abdomen, liver scan and arteriography. In all cases a surgical therapy is indicated. A central localization of the parasite cysts makes problems to the surgical management. In these cases we prefere the cystectomy. All other echinococcal cysts were removed totally, either by pericystectomy or hepatic lobectomy. Such a radicality is the exception in E. alveolaris. Twice we performed right hepatic lobectomy. In the other cases operative treatment was carried out to confirm diagnosis and ensure bile passage by a hepato-enterostomy. However, palliative bile draining procedures may prolong survival time to more than 10 years.

[Objectivation of the cardiac insufficiency load index--a comparison between spiroergometry and stretch-index].

It was shown that reduction in exercise capacity because of latent heart insufficiency could be significantly demonstrated by the so-called pre-ejection index PI, measured under conditions of rest. On the other hand, normals did not show any significant interrelation between PI and parameters related to exercise capacity, obtained by spiroergometric measurements. The pre-ejection index PI is a non-invasive parameter which compares the pre-ejection period in upright and supine posture. Since spiroergometric measurements could not be made under vita-maxima conditions, PI values were compared with parameters of submaximal exercise. Exercise was performed on the bicycle ergometer, with oxygen consumption and heart rate being recorded. Measuring results also revealed that by means of the pre-ejection index the necessity and effectiveness of glycoside medication may be objectively defined. Measuring techniques are described and statistically processed data are presented.
