[Cell-mediated immune-phenomenona in hyperthyroidism and euthyroid goiter (studies using the lymphocyte transformation test)].

For the establishment of cellular immune phenomena in hyperthyroidism and the euthyreotic struma 27 patients with hyperthyroidism, 18 patients with euthyreotic struma and 33 healthy control persons were examined with the help of the lymphocyte transformation test as to their ability to the blast cell transformation under addition of watery extract of the thyroid gland. In the cases of hyperthyroidism significantly increased transformation indices were found compared with the controls. Also in the euthyreotic strumae increased values were found compared with the controls. They lay at the borderline to significance. Three different extracts were used as antigen. The transformation indices calculated in these cases were partly significantly different. From this is concluded that not in every watery extract of the thyroid gland the responsible antigen exists in sufficient quantity and/or quality.

Idiopathic chronic hiccup: combination therapy with cisapride, omeprazole, and baclofen.

Idiopathic chronic hiccup (ICH) is defined as recurring hiccup attacks that last for longer than an arbitrary time limit (eg, 1 month) and for which no organic cause can be found. In patients with ICH, therapy is largely empiric. For practical purposes, idiopathic hiccup can be assumed to have its origin either in the viscera (gastrointestinal tract) or in the central nervous system. Cisapride and omeprazole--through reduction of gastric acid production and facilitation of gastric emptying, respectively--are thought to reduce an assumed afferent input from the periphery to a putative supraspinal hiccup center. Baclofen is thought to reduce excitability and depress reflex hiccup activity. Fifteen male patients (mean [+/- SD] age, 68.2 +/- 11.6 years) who had recurring hiccup attacks for a mean duration of 100.8 +/- 134.1 months (range, 12 to 564 months) were treated for ICH with a combination of cisapride, omeprazole, and baclofen (COB). Therapy led to a total disappearance of hiccup in 40% (6 of 15) of the treated patients. An additional 20% (3 of 15) of patients experienced substantial relief. A Mann-Whitney rank order test showed a highly significant reduction in the severity of the hiccup attacks as reflected in the subjective assessment scale scores taken before therapy (8.6 +/- 1.3) compared with those taken after 20 weeks of therapy (4.1 +/- 3.8). Thus we concluded that COB is an effective empiric therapy in at least some patients with ICH.

Our experiences with the early operative treatment of congenital clubfoot.

In operative clubfoot therapy, early operation plays an important role. Between 1975 and 1982, we operated on 55 children with 72 clubfeet: 56 posteroplantar releases in patients with an average age of 5.8 months, and 16 posteromedial releases in patients with an average age of 8.1 months. The average follow-up was 3.5 years. The results, based on an investigation scheme according to Henkel, were good and very good in 94%. Operative treatment is indicated after 3 to 4 months of intensive plaster treatment if there has been no further correction.

[The suppressor cell activity in rheumatoid arthritis].

The rate of 3H-thymidine incorporation into peripheral blood lymphocytes was determined immediately after the addition of 1 microgram of Con A and after 24 h of preincubation period the stimulation in normal controls was 3 times higher than after the immediate addition of mitogen. In rheumatics the increase was 1,7-fold only. The results were interpreted as indicating a lack of suppressor cell activity in the patients with rheumatoid arthritis.

[The etiopathogenetic significance of vitamin D3 in osteoarthropathia psoriatica].

1.25 (OH)2 D3 is the most effective natural metabolite of vitamin D. By binding to a specific receptor (VDR) it develops genomic as well as non genomic effects which exceed the effects on calcium regulation. Exercising an influence on immunological phenomena and on the behavior of cellular differentiation and proliferation, the different manifestations of psoriatic osteoarthropathy (skin lesions, chronic synovialitis, increased bone remodeling) may be due to a common pathogenetic process of a disturbed vitamin D metabolism. However, the supposed fault has not yet been defined and this concept is still hypothetical. Moreover, it is probable that it represents a partial aspect only in the quite unclear pathogenesis of the disease.

[Rheumatoid factor activity, age at manifestation and roentgenologic progression of rheumatoid arthritis--a retrospective study].

The outcome of RF-activity (measured by hemagglutination in the modification of Podliachouk-Harboe) was investigated in 95 patients with RA. In 52 of these patients the radiological progression (modification of Larsen index for hands and feet) in correlation to the outcome of RF was assessed. The results can be summarized in the following way: 1. Elderly RA patients show a significant elevation of RF titer. 2. There is a statistically insignificant correlation between age of RA manifestation and RF level. 3. The investigation of individual RF outcome shows that 54% of the patients have a relatively constant RF level, 15% tend to a decrease of the level of RF activity. Increased RF activity could only be demonstrated in 31% of the patients in the follow-up. 4. We more often observed a decrease of RF activity in RA cases with a disease manifestation < 30 y. We found no significant decrease in the follow-up in cases with manifestation > 60 y. 5. RA patients with a high level of RF activity (HAR > 1:512) have a significantly higher radiological progression index than cases with a low RF activity (HAR < 1:512).
