[Plasmacytoma].

Plasmocytoma is generally a systemic disorder and has to be differentiated from solitary plasmocytoma of bones and connective tissue. Diagnosis is based on the typical bone marrow findings, the demonstration of monoclonal paraprotein and the radiological skeletal changes. Prognosis is poor, life expectancy limited to about 18 months. Specific therapy with cytotoxic drugs leads in many cases to marked improvement of the general condition, relative freedom of pain and a decreased complication rate.

[Clinical experiences with holoxan within the framework of sequential combination therapy (author's transl)].

153 patients with inoperable malignant tumors were treated by the so-called synchronization therapy with vincristine and ifosfamide. 13 complete remissions (8.5%) and 47 partial remissions (30.7%) were obtained. 5 times cessation of tumor growth occurred (3.3%). In 88 cases the therapy failed (57.5%), but in 11 cases a subjective improvement was noted. The most favourable results of treatment were obtained in cases of malignant lymphoma, small cell carcinoma of the lung and testicular carcinomas. Although the duration of remission was short in most cases, it could be demonstrated, that a targeted polychemotherapy is possible using the proliferation kinetic data in addition to the knowledge of the cell-phase-specific effect of cytostatic drugs. This therapy can succeed in causing a tumor regression even in metastatic carcinomas.

[Rat intestinal disaccharidases during cyclophosphamide treatment (author's transl)].

Rat intestinal disaccharidase activities were investigated during treatment with different doses of 2-[bis-(2-chlorethyl)-amino]-tetrahydro-2H-1,3,2-oxazaphosphorine-2-oxide (cyclophosphamide). In all experiments, disaccharidase activities decreased--although not too drastically--within 2--3 days after beginning treatment. Loss of activity was not reversed during the period of experimentation. Contrary to previous investigations, cyclophosphamide did not exert direct toxic effects on intestinal disaccharidases. Decrease of intestinal disaccharidase activities can be interpreted to be due to cyclophosphamide effects on the proliferation cycle of epithelial crypt cells resulting in a decreased number of full-function cells.

[Nitroblue tetrazolium test in Hodgkin's disease (author's transl)].

Nitroblue tetrazolium dye (NBT) test was performed in 60 patients suffering from Hodgkin's disease. The control group consisted of 60 healthy adults. 23 patients with inactive lymphogranulomatosis had a mean NBT-balue of 30 +/- 9% formazan cells. There was no difference compared with controls (32 +/- 10% formazan cells). 37 patients suffered from active Hodgkin's disease. In this group 31 patients showed an elevated value, 6 had normal levels. On average the level was significantly raised with 59 +/- 20% formazan cells (p less than 0,01). A correlation between the extent of the disease and the height of the NBT figure could not be demonstrated. The same applies to the different histological types. In comparison between the NBT value and the simultaneously examined ALP it was found that ALP is a more sensitive parameter but that an increased ALP is not indicative of an active stage. It is therefore advisable to determined both parameters to get more security whether there is active or inactive Hodgkin's disease.

[Pregnancy in ulcerative colitis and Crohn's disease (author's transl)].

Between 1967 and 1973 15 women with ulcerative colitis had 20 pregnancies. 14 ended with normal deliveries, 4 with spontaneous abortions, and 2 with therapeutic abortions. Fertility was normal. Malformations in the children were not observed. In the case of conception during remission or a latent phase of the disease in general a normal pregnancy is to be expected (12 out of 14). On the other hand conception during an active phase can lead to an unpredictable threat to the life of mother and child. Therapeutic abortion should be seriously considered when ulcerative colitis and pregnancy begin simultaneously. In the same period of time in 15 comparable patients with Crohn's disease (12 with ileo-colitis and 3 with terminal ileitis) who were not taking ovulation inhibitors 4 pregnancies were observed. Fertility is not significantly reduced in terminal ileitis whereas in ileo-colitis it is highly significantly reduced (bilateral tubal irritation). As in ulcerative colitis a normal pregnancy can be expected after conception in an inactive phase (2 out of 4).

[Effects of colchicine treatment on rat intestinal disaccharidase activities].

UNLABELLED: The changes in morphology and proliferation kinetics produced by colchicine in the intestinal mucosa are known. Contrarily, the functional changes have yet not been investigated systematically. The aim of this study is to investigate the effects of colchicine treatment on rat intestinal disaccharidase activities. MATERIAL AND METHODS: For the experiments male SPF-Wistar rats, body weight 180--200 g (supplied by Mus-Rattus AG, 8011 Brunnthal, FRG) were used. The animals were kept under constant conditions (6 rats per cage) and fed Altromin (Altromin Gmb H,Lage/Lippe), tap water ad libitum. Per day 6 animals of each test group were scarificed by cervical dislocation at defined time. About 3 cm of the intestine were removed 8--10 cm distally of the pylorus and cut longitudinally. After cautious cleaning the mucosa was detached using a scalpel. The thus obtained specimens of the mucosa were frozen at -- 20 degrees C in an air-tight humidity chamber and preserved there until further handling. For histological examination the correspondent parts of the intestine were fixed in 10% formol. From each specimen 4 different stainings were made (haematoxylin-eosin, Giemsa, PAS and Feulgen stain). Per test series 60--66 rats were used. The following doses of colchicine were applied: 1 X 1.0 mg/kg body weight, 1 X 0.5 mg/kg body weight, 5 X 0.1 mg/kg body weight within 24 hours. The single doses were dissolved in about 0.5 physiological saline and applied intraperitoneally. In each 6 rats per group the activities of the following diasccharidases were measured at 24 hours intervals from the 1st to 10th or 14th day: sucrase, maltase, lactase, and cellobiase. The enzyme activities were determined after the two-step method developed by DAHLQVIST (5,6) which can be used as micromethod. The ascertained enzyme activities were always related to the protein content. Protein determination was done after the method of Folin-Ciocalteu as modified by LOWRY et al. (14). The enzyme activities were expressed in international units per g protein (U/g protein). the results obtained were statistically ascertained by the non-parametric test after Kruskal-Wallis, variance analysis with linear contrasts after Scheffe as well as by double variance analyses (26).

[Congenital dyserythropoietic anemia type II. Another case report].

Hematologic evaluation of a 29-year old man with jaundice since childhood demonstrated the characteristic findings of congenital dyserythropoietic anemia (CDA) type II. This type is characterized by ineffective erythropoiesis, multinuclearity of erythroblasts, electron-microscopic abnormalities of the red cell membrane, and abnormal serological reactions. Because of severe anemia repeated transfusions were necessary. After splenectomy thrombosis of the portal vein occurred. The patient died from hemorrhage due to ruptured esophageal varices.

[Familial occurrence of chronic lymphocytic leukemia: report of three siblings (author's transl)].

Chronic lymphocytic leukemia was diagnosed in three siblings. Familiar occurrence speaks in favor of a predisposition in this disorder of unknown etiology. According to the present status of knowledge endogenous - genetically determined - factors combined with exogenous factors (viruses, radiation exposure, toxic substances) participate in the etiopathogenesis of the different forms of leukemia.

[Acute leukemia as terminal stage of Hodgkin's disease (author's transl)].

In a 61 year-old female patient with Hodgkin's disease treated with cytotoxic drugs and radiation therapy acute undifferentiated leukemia developed 10 years later. The possibility of a carcinogenic effect of the cytotoxic and actinic therapy is discussed. Leukemia complicating the course of Hodgkin's disease must be considered an independent disease and not a iatrogenic transformation of a preexisting lesion.

[Plasmocytoma. A clinical and statistical survey (author's transl)].

Analysis of the case-records of 61 patients with plasmocytoma revealed an equal sex-distribution with a mean age of 58.6 years. The most common initial symptom of the disorder was ossary pain. The diagnosis was confirmed by bone marrow cytology, demonstration of paraproteins in serum and urine and the characteristic radiological changes. Renal insufficiency, immunodeficiency syndrome, fractures and neurological defects complicated the clinical course in many patients.

[Eosinophilia and sarcoma of the small intestine (author's transl)].

Eosinophilia occurs as a concomitant sign in multiple diseases of different etiology. Especially its occurrence in malignant diseases is still unknown. In three cases of primary sarcoma of the small intestine a constant eosinophilia is described. In one case this pronounced eosinophilia was present long before the final diagnose was made. The eosinophilic count was highest in the presence of clear symptoms of obstruction and normalized after surgical removal of the tumor. In one case a relapse was indicated by an increasing eosinophilic count. It is difficult to diagnose the sarcoma of the small intestine especially in its early stage because of the lack of typical symptoms. Concerning the described cases it is therefore pointed out, that in connection with undifferentiated abdominal complaints an obscure eosinophilia should be considered to be due to sarcomatous small bowel disease.

[The effect of cytostatic therapy with vincristin sulphate on disaccarchidases of rat intestinal mucosa (author's transl)].

This report shows that appropriate doses of vincristin sulphate may decrease disaccharidase activities of intestinal mucosa. With the higher doses of the cytostatic drug, the drastic drop of enzyme activities is associated with morphological alterations of the mucosa; disacchardiase activities remain depressed at least for a couple of days even after full morphological restoration of the mucosa. Studies in man should reveal whether similar intestinal lesions occur due to therapeutic doses of vincristin sulphate.
