Longstanding isolated bilateral Achilles tendinitis.

The case of a patient with a 15-year history of isolated HLA-B27-associated bilateral Achilles tendinitis without other features of seronegative spondyloarthropathies is reported. This case suggests that Achilles tendinitis may be, for a long time, the only clinical manifestation of an HLA-B27-associated disease process. The theory of a separate B27-associated disease entity cannot be excluded yet.

Drug-induced anticentromere antibody?

Anticentromere antibody (ACA) was previously described as a serological marker for CREST, progressive systemic sclerosis (PSS), Raynaud's syndrome (RS) and primary biliary cirrhosis (PBC). We have observed ACA appearing and disappearing during D-penicillamine (DPA) treatment in six patients without any features of CREST, PSS, RS, PBC or related syndromes. ACA also could be DPA-induced.

[The effect of HLA-DRB4 on the clinical picture of chronic polyarthritis].

61 patients suffering from classic erosive rheumatoid arthritis (RA) with a condition persisting for more than 5 years, were examined together with 69 healthy control subjects with regard to associations to "supertypic" HLA-D specificities. We found a significant increase of HLA-DRw53 in patients with seropositive RA (SPRA) (31/42 patients = 73.8%, Chi 2 = 18.4, p less than 0.01, RR = 7.3). Our results confirm the hypothesis, that the HLA-DRB4 gene is closer related to the disease susceptibility genes of SPRA than of DR4 (DRB1).

[CRST syndrome and primary biliary cirrhosis].

The case of a female patient with the history of amanita poisoning, CRST syndrome and primary biliary cirrhosis (PBC) is reported. Immunological features (IgM-rheumatoid factor, antinuclear-, anticentromere- and antimitochondrial antibodies) and HLA-antigens are studied. Immunological abnormalities occurring in CRST and PBC suggest a common autoimmune base, which also makes concomitance of these two disease states more than coincidental.

[Cross-reacting HLA-B alleles as genetic markers of HLA-B27 negative ankylosing spondylitis].

15 east Austrian patients (EUC) with HLA-B27-negative ankylosing spondylitis (AS) were tissue-typed for HLA-B-antigens. HLA-B-alleles of the B27-CREG (B7, B13, Bw22, B40) were found in 14/15 patients (= 93,3%; RR = 37,8; Chi2 = 23,21; p less than 0.05) of this group. We may postulate a common partial-antigen for AS, carried by HLA-alleles belonging to the B27-CREG. This partial-antigen may either react in a direct pathway with the unknown pathogenic agent of AS or represent a marker for the "disease-susceptibility genes" of AS.

[Epidemiology of medication suicide attempt in the elderly].

We studied 38 geriatric patients admitted to our observation ward after attempted suicide by drug ingestion. Age and sex distribution, social status, time of attempt, drugs and therapeutic procedures were registered and compared to suicide attempts of other age groups (n = 231). Our analysis showed that geriatric attempted suicides differed from those of other age groups by less exogenous influences, in motivation, methods and therapeutic procedures. They may represent an own entity of disease nearer to suicide than to attempted suicide.
