[Optimizing the clinical value of polyreactive autoantibodies to double-stranded DNA].

The serological characteristic of systemic lupus erythematosus is the presence of antibodies to double stranded (ds) DNA. Raised levels of these antibodies have been found occasionally in patients with other inflammatory rheumatic diseases. Evaluating 67 patients with anti-dsDNA antibodies we searched for additionally immunological features to improve their clinical relevance for connective tissue diseases. We found out that coexistent antinuclear antibodies, antibodies to Sm, U1-nRNP, Ro and La, except in D-penicillamine induced lupus syndrome, were most specific for connective tissue diseases. Permanent antinuclear antibody negative systemic lupus erythematosus may not really exist.

Induction of anticentromere antibody in patients receiving treatment with D-penicillamine.

Anticentromere antibody (ACA) was previously described as a specific marker for CREST, progressive systemic sclerosis (PSS), scleroderma (SCL), Raynaud's phenomenon (RP) and primary biliary cirrhosis (PBC). In this present investigation ACA was observed appearing and disappearing due to D-penicillamine (DPA) treatment in 7 patients without any features of CREST, PSS, SCL, RP or PBC. HLA-DR3 was found in 5/7 of these patients. DPA induced ACA should be added to adverse immunologic drug reactions.

[Circulating anticellular antibodies in male patients: coincidence of antibodies to intermediate filaments and HLA-B27 in Reiter syndrome].

Evaluating 57 male patients with anticellular antibodies (detected on HEp-2 cells) we could find an association of Reiter's syndrome (17/57), IgG antibodies to intermediate filaments (vimentin type) of the cytoskeleton (17/17) and HLA-B27 (13/17). We conclude, that both, genetic and hormonal factors may influence the humoral immune response in males with Reiter's syndrome. IgG antibodies to intermediate filaments might be precious additional markers for diagnosis of Reiter's syndrome in males.

[Recent knowledge and hypotheses on the significance of the HLA-D region in rheumatoid arthritis].

Rheumatoid arthritis (RA) may result from an interaction between genetic and environmental factors. There is a well documented association between RA and the serologically defined class II MHC antigen haplotype DR4-DRw53-DQw3. Cellular HLA typing reveals five Dw subtypes (Dw4, 10, 13, 14, 15) of this haplotype. DNA coding for this Dw-subtypes occurs in the third hypervariable region (3. HVR) of the DR beta 1 domain suggesting, that this part of the molecule is recognized by T cells. Relatively circumscribed amino acid changes in this 3. HVR, which contains an alpha-helical structure, may result in immune response changes. Susceptibility to RA may be due to related epitopes found in non-Dw10 subtypes of DR4, as well as in DR1 alleles. Studies, using T cell clones specific for the Dw14 subtype of DR4 found Dw14-associated epitopes in 100% (!) also on non-DR4 haplotypes, in the population with RA. Typing with primed lymphocytes may give rise to the association of HLA and RA.

[Detection of circulating anticellular antibodies by indirect immunofluorescence on HEp-2 cell substrates: study of the clinical value of rare fluorescence patterns].

We detected anticellular antibodies (Ab) in 867 sera from patients with inflammatory rheumatic disorders by indirect immunofluorescence technique on HEp-2 cells and searched for clinically relevance of the rare fluorescence staining pattern (n = 133). We could find associations of 1. spekled nuclear pattern and Ab to U1-nRNP and La, 2. nucleolar pattern and scleroderma, 3. peripheral nuclear membrane pattern, Ab to dsDNA and SLE, 4. centromere staining and CRST syndrome, 5. intermediär filament pattern and permanent cellular destructions in erosive arthritis, 6. mitochondrial staining and inflammatory hepatic disorders. The anticellular Ab staining patterns on HEp-2 cells might be precious additionally markers for diagnosis and prognosis of inflammatory rheumatic diseases.

[Rational HLA-A, -B, -C typing in rheumatology].

In the own laboratory of our department from 1984-1987 367 patients with several rheumatic diseases were tissue-typed for HLA-A, B, C-antigens. Associations with diseases and clinical parameters were examined. We could only find significant associations (P corr. less than 0.05) of HLA-B 27 with reactive arthritis and iliosacral arthritis. We conclude, that the B 27-test is sufficient for economic HLA-typing in rheumatology.

Selective binding of rheumatoid factors to antigen structures of cytomegalovirus (CMV).

In rheumatoid arthritis patients with malignant progress, short remissions and unimprovement response of activity to therapy Ab with both properties (antiviral and anti IgG) are compared. The selective binding of RF to antigen structures of CMV were tested by 180 patients in a new established ELISA with antiidiotypic goat antibodies of CMV neutralizing human monoclonal Ab and against virus antigen directly. Displacement reactions on the idiotypic binding side as well as comparative ELISA proved the special RF activity. 24.2% of patient suffering from RA with IgM RF showed selective bindings to antigen structures of CMV and anti IgG activity. The evidence of the specific binding capacity of RF to virus antigen in chronical CMV infection as well as their defined binding capacity to the Fc Fragment of IgG suggest CMV specific RF as pathogenetic factor of malignant forms of inflammatory rheumatism.

HLA-DR versus HLA-B5-CREG antigens in rheumatoid arthritis (RA).

Thirty caucasian patients suffering from classic erosive rheumatoid arthritis (RA), with a condition persisting for more than 8 years, were examined together with 67 healthy control subjects with regard to associations to HLA-DR and HLA-B-CREG antigens (B5-, B8-, B12-, B16- and B27-CREG). We found a significant correlation of seropositive RA (SPRA) with HLA-DR4 (9/14 patients = 64% chi 2 = 11.8, p less than 0.05), and seronegative RA (SNRA) with B5-CREG antigens (14/16 patients = 88%, chi 2 = 7.8, p less than 0.05). This HLA locus-differing association of SPRA and SNRA could, in addition to the known differences in clinicophenomenological parameters, be additional proof of the independent entity of these two forms of illness.

Rheumatoid factor isotypes and circulating immune complexes in rheumatoid arthritis.

Solid phase enzyme immunoassays were here used to quantify rheumatoid factors (RF) of the IgM, IgG and IgA classes and the immune complexes (IK) by their ability to bind to C1q or conglutinin in both the serum and synovial fluid of patients with rheumatoid arthritis (RA). Elevated serum levels of any RF isotype could be found in all patients with seropositive RA (IgM: 63%, IgG: 87%, IgA: 90%). Seronegative patients with RA presented to a significantly lesser extent with elevated levels of all the RF isotypes tested (IgM: 0%, IgG: 40%, IgA: 32%). Synovial fluid RF levels were significantly higher in SPRA patients than in SNRA patients with the exception of IgG-RF. All of the RF classes in both RA groups, however, were elevated when compared to RF in the synovial fluid of patients with osteoarthrosis. Both C1q binding and conglutinin binding immune complexes were significantly higher in the synovial fluid than in the serum of RA patients. The erythrocyte sedimentation rate and plasma iron levels were correlated with the levels of C1q binding immune complexes (IC) in the synovial fluid; total iron binding capacity showed an inverse relationship to synovial fluid IgG-RF levels. A radiographic index was also correlated with IgG-RF levels in the synovial fluid. Extraarticular manifestations were significantly more frequent in patients with elevated serum levels of IgM-RF or conglutinin binding IC. These findings indicate that IgG-RF in the synovial fluid and the formation of IC determined by their ability to bind C1q seem to be closely related to clinical features of local disease.(ABSTRACT TRUNCATED AT 250 WORDS)
