[Influence on load transfer of different femoral neck endoprostheses].

AIM: Aiming to reduce known complications of stemmed implants, such as resorptive bone remodeling or bone damage in revision, implants with only epi-metaphyseal anchorage have been developed. In the following study the influence of three different femoral neck endoprostheses, CUT, CIGAR (ESKA Implants Lübeck) and TPP (SulzerMedica) on the postoperative load transfer to the femur was investigated in comparison to a cementless hip stem. METHODS: Using a composite femur model and photoelastic coating technique, the pre- and postoperative osseous strain was measured under static loading. The load corresponded to the absolute maximum of the hip joint load during walking. Statistical analysis was based on the interval of 99 % confidence which was generated by the preoperative measurements. RESULTS: The different anchorage concepts of the femoral neck endoprostheses exhibited a significant influence on the load transfer, especially along the medial and lateral cortical bone. But, in comparison, the cementless hip stem caused more pronounced stress-shielding which can induce resorptive bone remodelling. CONCLUSION: One specific femoral neck anchorage concept led to a change from preoperative tension to postoperative compression at the lateral cortical bone. This regionally limited effect may influence in the mid- or long-term the local bone remodeling in a negative manner.

[Ultrasonographic measurement of leg length and leg length differences].

A new diagnostic method for measuring the leg length and the leg length difference by means of ultrasound is presented. A special device for holding and moving the ultrasound transducer was constructed. The measuring points on the hip, knee and upper ankle can be visualised by means of a 5- or 7.5-MHz linear scanner. The measuring device gives the distance of the points in cm so that the difference correspond to the length of the leg, femur and tibia. Tests conducted on corpes and clinical examples show that ultrasound in combination with our special device is an ideal method for determining the exact lengths of the leg. The ultrasound measuring of the leg length offers a reliable, non invasive and easily performed method. Because ultrasound is not limited by radiation hazards, our technique can be used for clinical screening.

[Sonographic determination of the humerus retrotorsion angle].

In patients with habitual dislocation of the shoulder and post-fracture malrotation the measurement of humeral torsion is important in preoperative diagnostics. An easy method of ultrasonic measurement of humeral torsion is proposed. The correlation with anthropometric, radiological and CT-measurement is good, what could be shown in 20 macerated humeri. In 111 patients with normal glenohumeral joints the average of humeral retrotorsion was 61 degrees. 86% had a torsion between 40 and 80 degrees.

[Value of ultrasound examination in lesion of the medial collateral ligament of the knee joint].

AIM: The present prospective clinical study investigates the value of ultrasonic examination of injuries of the medial collateral ligament of the knee. METHOD: The sonographical examination was done in a real-time method with an ultrasound unit Type AI 3200 (Dornier, Neufahrn, Germany) and with a linear transducer (frequency 7.5 Mhz). This functional measurement is being performed in neutral position and in valgus stress on both knee joints to verify the opening of the medial joint space. 42 patients who have been suspicious for a lesion of the medial collateral ligament of the knee were compared with 80 healthy persons. RESULTS: The average medial opening of the medial joint space in the patient group using manual dynamic ultrasonic examination was 4.4 mm +/- 1.1 mm (2.5-8.0) applying the force manual and 4.0 mm 1.0 mm (1.3 mm-8.0) using the Telos fixation device to apply the stress force. The control group showed an average medial opening of 2.2 mm +/- 0.5 (0-0.5) as stress was applied manual and 2.5 mm +/- 0.7 mm (0-0.4) with the apparative stress examination using the Telos fixation device. CONCLUSION: The ultrasonic verification of the stability of the medial collateral ligament complex in the medial longitudinal view using defined bony landmarks is a very reliable method as a diagnostic tool for acute or older lesions of the medial collateral ligament of the knee. CLINICAL RELEVANCE: The advantages of the ultrasonic examination are the immediate availability, the avoidance of radiation and the visualization of movements with dynamic examinations. In contrast to X-ray examination the ultrasonic investigation can provide useful additional information on soft tissue structures.

Human papillomavirus status in advanced cervical cancer: predictive and prognostic significance for curative radiation treatment.

Human papillomavirus (HPV) infection plays a major role in oncogenesis of squamous cell carcinoma of the cervix. This study was performed to investigate if HPV status and E2 gene integrity are prognostic parameters for clinical outcome and predictive for radiation response. Forty women with locally advanced cervical cancer treated with curative radiotherapy were analyzed for HPV infection and E2 gene integrity by multiplex polymerase chain reaction. Statistical analyses were performed for overall survival, disease-free survival (DFS), local progression-free survival, and treatment response (clinical complete remission). Twenty-eight (70%) of 40 carcinomas were HPV positive. The only significant factor for a better overall survival, DFS, and local progression-free survival was HPV positivity (P < 0.02, P= 0.02, and P < 0.05, log-rank, respectively). HPV-positive tumors had a significantly better clinical complete remission (67% vs 33%, P= 0.04, Fisher's exact test). An intact E2 gene region showed a trend for a better DFS (P= 0.1, log-rank). This study reveals HPV as an independent prognostic parameter for outcome and radiation response. Integration of the virus genome into host cell DNA might be a molecular target to determine the treatment response of HPV-positive cancers.
