Combination chemotherapy with cisplatin and bleomycin in advanced cervical cancer.

Seventeen patients with advanced and recurrent squamous cell cervical cancer were treated with a combination of cisplatin and bleomycin. One complete and nine partial responses were observed (53%). The median duration of response was 8 months and the median survival was 12.7 months (5 months for nonresponders). Response was documented, particularly in patients with nodal disease and lung metastases. No life-threatening toxic effects were reported. This combination appears to be as effective as other combinations, has low toxicity, is well-tolerated, and merits further study.

Monoclonal antibodies modify acetylcholine-induced ionic channel properties in cultured chick myoballs.

Monoclonal antibodies directed against the cholinergic binding site of the acetylcholine receptor were found to alter the ion channel properties in cultured chick "myoballs." Time and dose dependent reduction in acetylcholine sensitivity was observed. Noise analysis experiments indicated a decrease in the mean single channel conductance and an increase in the mean single channel open time.

Evidence for acetylcholine receptor blockade by intracellular hydrogen ions in cultured chick myoballs.

Acetylcholine (ACh)-induced membrane currents were recorded in voltage-clamped myoballs in culture. The ACh sensitivity and the charge transfer per channel (Qc) were doubled in NH4+ at normal extracellular pH (pHo). Immediately after wash-out of NH4+, the ACh sensitivity and Qc undershot below the control. The recovery period of the ACh sensitivity and Qc was about 20 min. The ACh sensitivity and Qc greatly increased at alkaline pHo and decreased at acid pHo. We suggest that the intracellular H+ ions can block the ACh-induced channel. The pHo effects may be mediated by a similar (but smaller) change in the intracellular pH (pHi). Intracellular alkalinization can improve the channel function.

Mouse rDNA: sequences and evolutionary analysis of spacer and mature RNA regions.

Two regions of mouse rDNA were sequenced. One contained the last 323 nucleotides of the external transcribed spacer and the first 595 nucleotides of 18S rRNA; the other spanned the entire internal transcribed spacer and included the 3' end of 18S rRNA, 5.8S rRNA, and the 5' end of 28S rRNA. The mature rRNA sequences are very highly conserved from yeast to mouse (unit evolutionary period, the time required for a 1% divergence of sequence, was 30 X 10(6) to 100 X 10(6) years). In 18S rRNA, at least some of the evolutionary expansion and increase in G + C content is due to a progressive accretion of discrete G + C-rich insertions. Spacer sequence comparisons between mouse and rat rRNA reveal much more extensive and frequent insertions and substitutions of G + C-rich segments. As a result, spacers conserve overall G + C richness but not sequence (UEP, 0.3 X 10(6) years) or specific base-paired stems. Although no stems analogous to those bracketing 16S and 23S rRNA in Escherichia coli pre-rRNA are evident, certain features of the spacer regions flanking eucaryotic mature rRNAs are conserved and could be involved in rRNA processing or ribosome formation. These conserved regions include some short homologous sequence patterns and closely spaced direct repeats.

Medial frontal cortex infarction and the alien hand sign.

Two right-handed patients with infarction involving the left medial frontal cortex are described. Both patients exhibited a form of transcortical motor aphasia and a psychomotor disturbance involving the right arm characterized by forced grasping, motor perseveration, and the presence of apparently purposeful movements that appeared to be dissociated from conscious volition. The latter feature is noted to be identical to the alien hand sign as described in the left arm of commissurotomized patients and patients with callosal neoplasm or infarction. It is suggested that the observed dissociation of conscious intention from purposeful movement may be related to dysfunction of the medial frontal cortex contralateral to the impaired extremity as much as to hemispheric disconnection.

The preoperative patient wit hypertension.

Preoperative medical evaluation of the hypertensive patient has a dual focus: (1) to establish an appropriate level of blood pressure control before anesthesia and surgery; and (2) to assess the degree of cardiovascular impairment, with particular attention to brain, heart, and kidney. Close communication between clinician and anesthesiologist can do much to ensure the patient's safety. Interpretation of the results of the preoperative examination is aided by the clinician's prior knowledge of the patient: the onset and history of his hypertension, his record of compliance to an antihypertensive regimen, his blood pressure response during previous hospitalizations and surgical procedures. Though there is no longer any question that antihypertensive medications should be continued in the preoperative period, side-effects of thiazide therapy may need treatment before or during surgery. Again, the clinician's fuller knowledge of the patient will help to alert the anesthesiologist to the need for special caution.

DNA-directed in vitro synthesis of proteins involved in bacterial transcription and translation.

The in vitro synthesis of elongation factor (EF)-Tu (tufB), the beta beta' subunits of RNA polymerase, ribosomal proteins L10 and L12 directed by DNA from the transducing phage lambda rifd 18, EF-Tu (tufA), EF-G, and the alpha subunit of RNA polymerase directed by DNA from the transducing phage lambda fus3 has been investigated in a crude and a partially defined protein-synthesizing system. Proteins L10 and L12 are synthesized in the partially defined system almost as well as in the crude system. However, the synthesis of EF-Tu, EF-G, and the alpha and beta beta' subunits of RNA polymerase is far less efficient in the partially defined system. An active fraction that stimulates the synthesis of these latter proteins has been obtained by fractionation of a high-speed supernatant on DEAE-cellulose. Because previous studies showed that this fraction (1 M DEAE salt eluate) contains a protein, called L factor, that stimulates beta-galactosidase synthesis in vitro, L factor was tested for activity. Although L factor stimulates the synthesis of the beta beta' subunits, it has little or no effect on the in vitro synthesis of the other products studied. In the present experiments, the ratio of L12/L10 and of EF-Tu (tufA)/EF-G formed is 4-6. These values are consistent with in vivo results.

In vitro regulation of DNA-dependent synthesis of Escherichia coli ribosomal protein L12.

The DNA of the transducing phage lambdarifd18 contains, among others, the genes for the ribosomal proteins L11, L1, L10, and L12 and the beta and beta' subunits of RNA polymerase (nucleosidetriphosphate:RNA nucleotidyltransferase, EC 2.7.7.6). In a coupled in vitro protein-synthesis system, lambdarifd18 DNA directs the synthesis of about four to five molecules of L12 per molecule of L10. This is consistent with the finding that there are four copies of L12 per ribosome. The ratio of L12/L10 was also examined from an EcoRI fragment of lambdarifd18 that contains the L10 gene and about 50% of the L12 gene. A significantly lower ratio of truncated L12/L10 was observed compared to the intact phage. The binding of RNA polymerase to various lambdarifd18 DNA restriction fragments was used to locate possible promoter sites. These binding experiments suggest that the beta and beta' subunits of RNA polymerase are cotranscribed with at least ribosomal protein L12 and, also, that there may be an additional promoter site for the L12 gene within the structural gene for L10.
