Serum parvalbumin, an indicator of muscle disease in murine dystrophy and myotonia.

The soluble Ca(++)-binding protein parvalbumin (PV) is highly concentrated in fast muscle fibers of the wild type mouse. Employing Sandwich ELISA, we have shown that PV is present in the serum of normal mice and that its level is indicative of the disease status of muscle. Elevated PV levels were found in mice with X-linked dystrophy (mdx) and reduced levels in myotonic (ADR) mice. Serum creatine kinase (CK) levels were elevated in mdx and normal in ADR mice. Because myotonic mouse muscle has a strongly reduced PV content, the reduced PV serum level in ADR mice indicated that serum PV is derived from skeletal muscle. Serum PV in mdx mice, in which muscle PV content is close to normal, is a measure of the necrosis of fast muscle fibers. Serum levels of PV and CK were not significantly elevated in heterozygous (mdx/+) carrier females. Serum PV in Duchenne patients was below the limit of detection.

[Urine analysis for legal confirmation in opiate addicts].

Urine samples of 31 probands who had come in conflict with the Dangerous Drugs act and therefore had to take part in urinalysis tests where analysed during a period of 34 months. The purpose of this study was to find out whether urinalysis testing can reduce drug abuse. The urine samples were tested on morphine/codeine, cannabis and barbiturates. 28 out of 31 persons were to be tested on morphine, 13 on cannabis and 26 on barbiturates. 32% of the persons tested on morphine showed no positive results during the whole period of investigation. 3% had continuous relapses (greater than 30% positive results) during this time. A significant (x2 = 28.98) decrease of positive samples at the end of the investigation period was found. The tests on barbiturates showed no significant (x2 = 0) influence of the urinalysis on the frequency of positive results. Tests on cannabis showed a slightly significant (x2 = 4.26) increase of positive results at the end of the investigation period. Most of the test persons showed an obvious improvement of their social and family conditions during this period.

[Serum testosterone concentrations in cannabis and opiate users].

The object of this study was to establish possible influences of long-term cannabis usage on plasma testosterone levels. The plasma testosterone levels of 66 male Pakistani who for years had smoked cannabis daily or drank cannabis regularly where measured after chronic and acute intake of the drug and compared with a material of 41 normal controls, i.e. persons who did not use cannabis. An evaluation of the results showed that there were no significant differences between the two groups. No influence of long-term cannabis usage on plasma testosterone levels was found. Furthermore we wished to find out wether long-term heroin abuse showed an effect on plasma testosterone levels. The concentrations of testosterone in the plasma of 102 heroin addicts assigned to a Methadone Program were measured and compared with the values of 29 male healthy students as controls. Plasma testosterone levels were found to be significantly decreased in heroin addicts as compared to controls.

[Cardioactive contaminants and actual active ingredient content of various digitalis preparations].

To establish possible cardioactive impurities which have not been totally separated while isolating the compound from plant material several preparations containing digoxin (NOVODIGAL = beta-acetyldigoxin, LANITOP = beta-methyldigoxin and LANICOR = digoxin) were analyzed by HPLC-chromatography after careful extraction. Impurities were found (digoxigenin, digoxigenin-monodigitoxosid, digoxigenin-bisdigitoxosid) which on the one hand occur as natural lanatosid-derivates in digitalis leaves but on the other hand also are described as possible metabolites of digoxin in human organism. The experiments have shown that the results of studies in metabolization of digoxin without knowledge of purity of the substance applied have to be interpreted with care.

[Two atavistic characteristics of the laryngeal skeleton].

In the analysis of laryngeal anomalies in 30 selected phoniatric patients by CT examination, 2 atavisms of the laryngeal skeleton were found. The ventral enclosure of the thyroid cartilage by the hyoid bone presents an inhibition malformation of the laryngeal skeleton with essential reduced vocal ability. The posterior junction between the cornu majus ossis hyoidei and the cornu superius cartilaginis thyroideae is marked by the direct contact of these cornua lacking the ligamentum thyrohyoideum laterale. Both anomalies are characterized by the persistence of the close relation between the hyoid bone and the thyroid cartilage, which normally exists phylogenetically as well as ontogenetically. That could be shown by anatomical sections performed on 4 human newborns in the horizontal plane.

[Morphology of the hyoid bone joint].

Anatomical, histological and CT examinations were performed on the joint between the body and the greater comu of the hyoid. The existence of an articular surface, articular capsule and joint cavity revealed that in many cases there is a true atriculation not only in newborns but even in adults. The joint cavity appears as a cleft on X-ray, sometimes leading to the wrong diagnosis of fracture of the hyoid bone.

[Personality and coping with stress in patients suffering from functional dysphonia].

The examination covers the personality of 20 functional dysphonics, 14 individuals suffering from organic voice disorders and a group of 20 control persons when exposed to stress and its working up. In examining the group of functional dysphonics a significant positive self-assessment regarding their personality with a clear tendency to restraint has become clear. In comparison with the control persons an intensified anxiety state in situations causing stress is the result. Thus the influence of life events has been shown to have an important effect on the etiopathogenesis of functional dysphonia.

[Ventral enclosure of the thyroid cartilage by the hyoid bone. A complex inhibition abnormality of the laryngeal structure].

Performing laryngeal CTs we identified a "ventral enclosure of the thyroid cartilage by the hyoid bone" in 3 patients with vocal disturbances and demonstrated for the first time the morphological and clinical characteristics of this complex inhibition malformation: --persistence of the embryonal close relation between the thyroid cartilage and the hyoid bone with normal position of the total larynx --feminine configuration of the thyroid cartilage --tendency to disturbances of growth of the thyroid cartilage with effect on the endolarynx --reduced vocal ability and predisposition to vocal disturbances.

[Computed tomography of the larynx in the diagnosis of dysphonia].

By comparing CT scans of the larynx with anatomical sections in the horizontal plane as well as by examining patients, a method of investigation especially aimed at phoniatric questions has been worked out: CT scans are performed every 4 mm parallel to the vocal cord plane using a center of 50 HE and a window of 350 HE. The examination is done during regular respiration and if required can also be performed during phonation when producing the vowel /u/. Using this technique the laryngeal structures of clinical relevance can be demonstrated with the help of at least 10 scans. It requires little time and the radiation exposure for the patient is minimal. The accuracy of CT measurements corresponds excellently to the anatomical features. Allowing an exact evaluation of anomalies of the larynx in shape and position, CT essentially improves phoniatric diagnostics.

The results show that the group of functional dysphonics produced a significant more positive self-evaluation regarding their personality with a clear tendency to restraint. Compared to the control group they showed a higher level of state anxiety in situations causing stress. Furthermore the influence of proceeding life events for the etiopathogenesis of functional dysphonia has become clear.

[Prevention of post-puncture complications by rotation of the puncture needle by 90 degrees].

We investigated whether the incidence of post-lumbar puncture headache could be reduced by using a new technique. By rotating the puncture needle the spinal meninges become penetrated with the bevel adjusted parallel to the main fibre direction. Therefore the fibres are only dissociated and not cut. We used this method in 20 patients and only 2 of them complained of headache. By contrast 14 out of another group of 20 patients suffered from headache after conventional lumbar puncture. These results demonstrate that post-lumbar puncture headache can be prevented in a high percentage of patients without using special puncture needles or sophisticated techniques.

Absorption and bioavailability of nebulized morphine.

During anaesthesia seven patients received a bolus of morphine 10 mg injected into the nebulization reservoir placed between the tracheal tube and the anaesthetic circle (IH). Five days after operation the same seven patients received morphine 10 mg i.m. On both occasions, venous blood samples were taken before and every 15 min after administration over 4.5 h for measurement of free morphine immunoreactivity by radioimmunoassay. There was marked individual variation in the serum morphine concentrations produced following each route of administration. The maximum serum morphine concentration following inhaled morphine was approx. six times lower than that after morphine i.m. and the time of occurrence differed significantly (P less than 0.001). The individual relative bioavailabilities of inhaled morphine varied from 9% to 35%, with a mean of 17%.

Excretion of tiapamil in breast milk.

The excretion of tiapamil in breast milk was studied in six lactating mothers (3-7 days post partum) following a single oral 600 mg dose of the drug. The milk/plasma ratio of tiapamil derived from the areas under the plasma and milk concentration-time curves was 0.44 +/- 0.10 mean +/- s.d.). Assuming an intake of 350 ml of milk during a dosing interval of 12 h, the newborn would be exposed at the maximum to 0.053 mg tiapamil. This small amount does not represent a risk for the baby.

[Morphometry of the larynx in horizontal sections. Normal data for the quantitative evaluation of current imaging technics].

Laryngeal anomalies as a predisposing factor for pathogenesis of vocal disturbances have been discussed for a long time. Laryngeal structures can be made visible in vivo, and structural details be measured, only since the development of new imaging techniques, mainly of CT. As adequate descriptions have not been published in literature unto now, we measured horizontal sections of laryngeal specimens to collect reference data for a quantitative analysis of CT images. An evaluation technique was therefore developed, which, by clear definition of the planes and points of measurements, yields reproducible results. Direct measurement of the thyroid angles can be abandoned, since these can be easily calculated from the known distances.

[Herpes zoster cephalicus].

The clinical features of ENT infections induced by the herpes zoster virus are presented. Detailed descriptions of two cases affecting the ear and one affecting the larynx are given, supplemented by photographs. The possible cranial nerve lesions are demonstrated by a review of the literature. An attempt is made to assign the distribution of dermomucosal eruptions and associated sensory and motor dysfunctions to the appropriate neuroanatomical structures.

[Manifestations of actinomycosis of the head and neck].

The clinical features of actinomycosis affecting the ear, nose and throat are presented. One patient with actinomycosis of the ear suffering from mastoiditis and a Bezold abscess is discussed in detail. The disease is rare: only 10 patients with actinomycosis of the tympanomastoid area have been described in the English literature. The diagnosis is therefore often delayed. Effective treatment requires combined surgery and long-term chemotherapy.

[Impression of the thyroid cartilage lamina--differential diagnosis in hyperplasia of the ventricular fold].

Impression of the thyroid lamina is presented by means of CT examinations of the larynx and in correlation with the clinical features and with serial sections of the larynx in the horizontal plane. This anomaly of the thyroid lamina produces the laryngoscopic aspect of a hyperplastic false vocal cord. Measurements proved that the horizontal extension of the convex thyroid lamina is always longer than the lamina of the other side. The pathogenetic mechanism in the development of this anomaly lies--in our opinion--in the irregular increase in size that probably begins at the onset of puberty. The impression of the thyroid lamina is predominantly found in men and mainly on the left side. In its effects this anomaly must be classified as a dysplastic dysphonia and as such it reduces the capacity of the vocal organ and predisposes to the development of functional dysphonia. There should be careful differentiation between the impression of the thyroid lamina and the "genuine" hyperplastic ventricular fold, because these clinical syndromes are completely different from each other in aetiology, therapy and consequences.
