Inhibition of lignin formation by L-alpha-aminooxy-beta-phenylpropionic acid, an inhibitor of phenylalanine ammonia-lyase.

Mungbean (Vigna radiata (L.) Wilczek) seedlings grown for 9 days on filter paper soaked with 0.3 to 1 mM L-alpha-aminooxy-beta-phenylpropionic acid (AOPP), a potent inhibitor of L-phenylalanine ammonia-lyase, had a greatly reduced anthocyanin content, and the cell walls of the xylem vessels did not stain with the phloroglucinol/HCl or safranine/astrablue reagents indicating the absence of lignin-like material. Furthermore, vanillin was detectable in nitro-benzene-oxidized lignin preparations only from control seedlings, but not from AOPP-treated seedlings. Scanning electron microscopy of hypocotyl cross sections revealed collapsed xylem vessels in seedlings grown in the presence of AOPP indicating that lignin is required for resistance against the tensile forces in the conducting cells of the xylem. AOPP enhanced the growth of cultured cells of Lonicera prolifera Rehd. while it inhibited the production of extracellular material that gave a positive reaction with phloroglucinol/HCl.

[Results of electrophysiologically guided surgery in ventricular tachycardia].

We report on 33 patients (pts) with medically refractory ventricular tachycardia (VT) after myocardial infarction in whom electrophysiologically guided surgery was performed. In 18 pts the origin of the VT was identified preoperatively by endocardial catheter mapping. Intraoperative endocardial mapping was performed in 32 pts. The origin of the VT was detected in 24 pts, whereas in 8 pts the surgical procedure was guided by the results of intraoperative mapping during sinus rhythm. In one patient no intraoperative mapping was performed and surgery was guided only according to the results of endocardial catheter mapping. A deep endomyocardial incision was performed in 14 pts, whereas local endocardial resection was done in 19 pts. Six pts (18%) died postoperatively still being in the hospital, 27 pts (82%) were discharged. There were 4 late deaths during mean follow up period of 18 months. One patient had a spontaneous recurrence of VT. 8 pts remained on antiarrhythmic medication due to complex ventricular arrhythmias. We conclude that in case of medically refractory VT in coronary artery disease, surgical therapy becomes an acceptable alternative when guided by pre- and intraoperative electrophysiological mapping.

CT scan and metrizamide CT cisternography in arachnoid cysts of the middle cranial fossa: classification and pathophysiological aspects.

Thirty-one cases of arachnoid cysts of the middle cranial fossa were investigated by CT scan, 7 of them undergoing additional metrizamide CT cisternography. In this paper we analyze and correlate the different findings from these two examinations and propose a classification of arachnoid cysts of the middle cranial fossa into three basic types. Pathophysiological and surgical implications are discussed.

CT scan prediction of late post-traumatic epilepsy.

Out of 233 patients admitted for head trauma during 1977-1978, 93 had a CT scan examination within the first 48 hours. Forty-nine of these had at least one clinical risk factor for post-traumatic epilepsy. Ten of this group developed post-traumatic epilepsy. In all cases early CT scan showed focal brain damage, which was related more significantly to post-traumatic epilepsy than to risk factors.

Stereotaxic rostral mesencephalotomy in treatment of malignant faciothoracobrachial pain syndromes. A survey of 14 treated patients.

Stereotaxic rostral mesencephalotomy was performed 19 times in 14 patients suffering from intractable pain syndromes due to malignant diseases. The satisfying results in terms of pain relief during a necessarily short follow-up period (mean 4.9 months) are outlined. Undesired side effects were mainly confined to oculomotor disorders, which partly subsided over the months following the operation. Some technical aspects of the procedure and the pertinent literature are briefly discussed.

Direct surgical therapy of ventricular arrhythmias in coronary heart disease.

The aim of surgical therapy for ventricular arrhythmias is to eliminate the areas of diseases myocardium from which ventricular tachycardias originate. According to the results of endocardial mapping a 2 to 3 mm deep endocardial incision is performed around the identified arrhythmogenic area. Within the past 12 months, 19 patients with coronary heart disease and complex ventricular arrhythmias were operated upon. In 16 cases an endocardial incision was performed. In 5 a left ventricular aneurysm was resected as well, 4 patients received one to 4 coronary vein grafts, and in 7 patients aneurysmectomy and bypass grafting were combined. Except for one relapse, no ventricular tachycardia has reappeared since the operation. Ventricular tachycardia could not be initiated by intracardiac stimulation postoperatively in any of the cases restudied. Four patients died from causes unrelated to ventricular arrhythmias. Direct surgical therapy was proven a promising new approach in patients with ventricular tachycardia or complex ventricular arrhythmia refractory to medical treatment. Our technique of endocardial incision appears to be effective and less traumatic to the myocardium than the original transmural encircling incision.

Abnormal control of growth hormone secretion by opiate systems in acromegalic patients: a study with naloxone and 2-Br-alpha-ergocryptine (CB 154).

The present study was undertaken with the aim of exploring the role that opiate systems may have in neuroendocrine control in acromegaly. The effects of naloxone (0.4 mg, i.m.), of 2-Br-alpha-ergocryptine (CB154, 2.5 mg, p.o.) and of the interaction between CB154 and naloxone on growth hormone (GH) and prolactin (PRL) secretion were studied in 11 acromegalic patients. CB154 reduced both GH and PRL serum levels, naloxone only GH serum levels. The latter effect deserves further study aimed at a possible new therapeutic approach to GH hypersecretion observed in acromegaly. Naloxone also interfered with the lowering effects of CB154 and GH and PRL serum levels, pointing to the existence of an interaction between dopaminergic and opiate control of GH and PRL secretion in acromegaly.

Amino acid sequence analysis of fragments generated by partial proteolysis from large simian virus 40 tumor antigen.

Large simian virus 40 tumor antigen was bound as immune complex to protein A-Sepharose and then subjected to limited proteolysis which yielded several discrete fragments. Primary structures near the cleavage sites were determined by radiosequencing techniques. Experimental data for five fragments matched an amino acid sequence predicted from a nucleotide sequence at 0.51 map unit of the viral genome. We have thus identified the reading frame of translation beyond the intervening sequence at 0.60 to 0.53 map units. A cleavage map of tumor antigen was established on the basis of the sequence data and of the apparent molecular weights of the fragments. The bond most susceptible to cleavage by trypsin was between arginine-130 and lysine-131 in a cluster of five basis amino acids. Other cleavage sites were located in the COOH-terminal half of tumor antigen. Each fragment was analyzed by complete tryptic proteolysis and peptide mapping on an ion exchange column. Peaks occurring in the peptide map of large tumor antigen could thus be assigned to different segments of the protein. Two specific regions of tumor antigen were shown to be phosphorylated.

Thrombosis of the inferior vena cava after closure of atrial septal defect.

Although complications of atrial septal defect closure was rare, some sources of potential error should be kept in mind. In the case presented an enlarged Eustachian valve, mistaken as the lower rim of an atrial septal defect, was included in the direct repair and led to obstruction of the inferior vena cava. Caval thrombosis occurred subsequently. Early reoperation, resection of the Eustachian valve and removal of the thrombus were followed by an uneventful recovery.

Results after resection of postinfarction left ventricular aneurysms.

Although left ventricular aneurysmectomy (LVA) is a common surgical procedure, the late functional and hemodynamic results have not been well defined. This presentation describes our results with LVA in 135 patients operated between 1969 and 1979. Associated procedures were performed in 57 (42%) including coronary bypass grafting in 50, valve replacement in 5, closure of ventricular septal defect in 2, or combinations of these in 3 patients. One hundred four of the 122 hospital survivors were followed from 2 to 107 months (mean = 37 months). There were 13 hospital deaths (9.6%), 12 late deaths (9.8%) and an actuarial 5-year survival rate of 77%. Clinical improvement of preoperative heart failure occurred in 82%, and of angina in 70%. Only 33 patients (30%) returned to normal work. Bicycle exercise testing in 70 patients showed normal working capacity in 41 (59%). Recatheterization in 49 patients showed no significant changes in left ventricular end-diastolic pressure or cardiac index, and a borderline reduction of the total ejection fraction. Ventricular arrhythmias were detected by long-term ECG in 70% of all patients after surgery. Of those with preoperative life-threatening arrhythmias, rhythm improvement was noted in 50%, but only 2 of 13 patients were free of arrhythmias after operation. This study demonstrates a greater frequency of postoperative symptomatic and functional improvement as compared to hemodynamic and ECG improvement. Ventricular tachyarrhythmias originating from post-infarct scars increased intra- and postoperative risk and aneurysmectomy alone is considered insufficient for treatment of these disturbances. Further electrophysiologic investigations are needed and additional surgical measures may be necessary to improve the subset of patients with life-threatening arrhythmias.

Effect of praziquantel on clinical-chemical parameters in healthy and schistosome-infected mice.

The response of hepatic and haemotopoetic functions to treatment with praziquantel was studied using healthy and schistosome-infected mice. Female CF1 mice harbouring an 18 week old infection with Schistosoma mansoni and healthy uninfected mice of the same age were orally treated with 1 x 250 mg praziquantel/kg. The respective uninfected controls received the vehicle only. Blood samples were taken one, five, 14 and 28 days after treatment. Parameters studied were: activity of GOT, GPT and AP, concentration of glucose, blood clotting time, haemoglobin content, erythrocyte and leucocyte counts, PCV and body weight. The data were analyzed to reveal the effect of the three independent variables involved: infection, treatment and time after treatment. Infection of mice with S. mansoni for 18 weeks resulted in a depression of body weight, in a decrease of plasma GOT activity and of PCV and in increases of plasma GPT and AP activities, leucocyte counts and clotting time. Plasma glucose concentrations remained unaffected. The effects of treament with praziquantel were confined to the infected group. Changes attributable to the variable time were also more pronounced or even restricted to the infected treated group. Treatment of infected mice with praziquantel resulted in a temporary elevation of plasma GOT and GPT activities on Day 1 after treatment. Values had returned to normal on Day 5. Treatment further resulted in a slight but prolonged elevation of AP activities, a high leucocyte count on Day 5 after treatment and a normalization of the underweight and anaemic state of the infected mice. The nature of the effects observed after treatment with praziquantel is discussed in the light of corresponding data on the effect of treatment with hycanthone and SQ 18.506 in schistosome infected mice and Mastomys. It is concluded that the changes observed can be regarded as secondary, reflecting host responses to damaged parasites and healing processes.

Subacute and chronic extradural hematomas: a study of 30 cases.

The authors report 30 cases of extradural hematomas with prolonged course: the interval between injury and development of clinical signs was more than 48 hours. Twenty-nine were operated upon. These delayed cases have a mild clinical course and a better prognosis than the acute. Contrary to the reports that many subacute and chronic epidural hematomas are located other than temporally, the present series shows the high incidence (83%) of temporal lesions. Of the patients 74% recovered and returned to their previous occupations: the two deaths were from causes not directly related to the injury and hematoma. We conclude that most subacute hematomas arise from various, often combined, low-tension sources of bleeding. The importance of recognizing gradual neurologic deterioration and performing craniotomy in these patients is emphasized.
