[Choice of heart valve prosthesis--1985].

Improvement of materials, prosthetic function and perioperative treatment has led to positive results with regard to life expectancy, quality of life and complication rates in heart valve replacement. While mechanical valves are still associated with the risks of thromboembolism and anticoagulant bleeding, there is a continuing risk of early valve dysfunction in bioprostheses. We have preferred the latter during the last 8 years; however, certain disadvantages have become obvious during the follow-up of a group of 132 patients beyond the 6th postoperative year. Anticoagulant treatment has been withheld in some patients in spite of a clear indication while others have received coumadin in the absence of obvious reasons. Valve degeneration as discovered by echocardiography often was not recognized in time, resulting in a significant reoperative mortality. As a result the indication for biological valve replacement, especially in the mitral position, has become more restrictive in our hands.

[Experience with VVI, QT time and activity-controlled pacemaker systems in patients with tranvenous ablation of atrioventricular conduction].

In 36 patients with atrial flutter, atrial fibrillation or supraventricular tachycardia, refractory to medical treatment, who underwent interruption of atrioventricular conduction by means of an electrode-catheter, i.e. His bundle ablation, various pacemakers were studied. In 22 patients (group I) a heart rate programmable VVI-pacemaker was implanted, in 7 patients (group II) an activity-mode pacemaker (Activitrax 8400, Fa. Medtronic) and in 7 patients (group III) a QT-mode pacemaker (Quintech 911, Fa. Vitatron). To study physical work capacity, exercise testing was performed using a bicycle ergometer in group I and group III and a treadmill in group II. Sixteen patients with VVI-mode pacing (72.7%) complained of shortness of breath during exercise in normal daily life, in comparison to only 2 patients in group II (28.6%) and 3 patients in group III (42.9%). The exercise test revealed a normal physical work capacity in only 3 out of 22 patients (13.6%) with a VVI-pacemaker, but a normal physical work capacity in all patients with activity-mode or QT-mode pacing. Heart rate during stress test in patients with an Activitrax-pacemaker increased more rapidly than in patients with a Quintech-pacemaker and also dropped more quickly in the Activitrax-group than in the Quintech-group after cessation of the stress test. It is important to note that pacemaker reprogramming was necessary in 1 out of 7 patients (14.3%) with activity-mode pacing and in 4 out of 7 patients (57.1%) with QT-mode pacing.(ABSTRACT TRUNCATED AT 250 WORDS)

Amino acid sequence of an invertebrate FBP aldolase (from Drosophila melanogaster).

The complete amino acid sequence of FBP aldolase from Drosophila melanogaster has been determined. The enzyme contains four identical subunits of 360 amino acid residues. The primary structure of the monomer was established using automated Edman degradation on fragments prepared by CNBr-cleavage, by partial acid cleavage at the unique Asp-Pro bond and by oxidative cleavage at the three tryptophan residues. Manual Edman-Chang degradation was used on smaller peptides obtained by digestion with Staphylococcus aureus V8 protease, trypsin or chymotrypsin. The primary structure of Drosophila aldolase exhibits very extensive homology with the sequence of rabbit muscle aldolase (71% identity), thus explaining the early observation that Drosophila and mammalian aldolases form active interspecies hybrid quaternary structures (Brenner-Holzach, O. and Leuthardt, F., Eur. J. Biochem. (1972) 31, 423-426).

Primary intracranial lymphomas.

We reviewed the cases of 16 patients with primary intracranial lymphomas. The clinical and radiological data proved to be helpful in suggesting the diagnosis, but only tumoral biopsy provided unequivocal diagnosis. In our cases radiotherapy proved to be the most effective treatment.

Co-trimoxazole prophylaxis during high-dose chemotherapy of small-cell lung cancer.

In 103 patients with small-cell lung cancer, we compared four courses of standard doses of Adriamycin (A) (Adria Laboratories, Columbus, Ohio), vincristine (V), and cyclophosphamide (C) with a regimen of increased doses of cyclophosphamide and to a lesser extent, Adriamycin. We found no significant difference in rate (22% v 21%) or median duration (seven v nine months) of complete remission. Patients not in complete remission after the four cycles of AVC received two courses of VP-16 (etoposide) and cisplatin: the complete remission rate increased to 49% and 48% respectively. Patients on the high-dose arm received co-trimoxazole prophylaxis; those on the standard arm did not. Patients on the high-dose arm had a higher incidence of neutropenia (nadir less than 500 cells/microL) but a lower incidence of infection for similar degrees of neutropenia. However, they also suffered more severe side effects of a different kind. Cotrimoxazole thus allowed for the administration of higher doses of chemotherapy to outpatients by protecting them from infection. However, the higher doses of cyclophosphamide and Adriamycin, did not improve treatment results, produced more severe side effects, and is not recommended.

Hydroxylation and conjugation of phenol by the frog Rana temporaria.

Frogs injected with phenol excrete 67-95% of dose in 15 h; 32-87% of dose are metabolites. Metabolites identified were phenyl sulphate (15-44% of dose), phenyl glucuronide (10-25% of dose), catechol sulphate (up to 7% of dose), quinol sulphate (1-25% of dose), resorcinol and catechol (traces).

Life history model of adaptation to disability: the case of a 'congenital amputee'.

Life histories can be used to describe adaptation over time to disabling conditions and the context and meaning of disabilities. Data from the life history of Diane DeVries, a 33-year old woman born with quadralateral limb deficiencies (upper extremity hemimelia, lower extremity amelia) illustrate a model for eliciting, presenting and interpreting life histories of persons with disabilities. Substantively, the life history of Diane DeVries indicates, at this time, survival within a niche of institutional supports that include marriage, church membership, and government for the disabled. Her life history is also characterized by choices that support independent living in the community and a cognitive orientation toward her own cultural normality.

[Surgical possibilities in the treatment of ventricular tachycardia].

Medical therapy for recurrent sustained ventricular tachycardia is often disappointing. We report on the direct i.e. electrophysiologically guided surgical approach to 44 patients with sustained ventricular tachycardia. 43 patients had previous myocardial infarction, one patient had arrhythmogenic right ventricular dysplasia. During preoperative electrophysiologic study, sustained ventricular tachycardia was induced in 41 patients, three patients had an incessant sustained ventricular tachycardia. 30 patients underwent endocardial catheter mapping. In 28 of 30 cases, the earliest endocardial activation during ventricular tachycardia was detected. Intraoperative mapping was performed in 42 patients, in two cases surgical approach was guided by the result of endocardial catheter mapping. In 34 patients endocardial mapping revealed the earliest site of ventricular tachycardia, in eight patients the arrhythmogenic area was identified by mapping during sinus rhythm. An encircling endocardial ventriculotomy was performed in 14 patients, whereas 29 patients underwent a circumscribed endocardial resection. A cryosurgical technique was performed in the patient with arrhythmogenic right ventricular dysplasia. The hospital mortality rate was 16% (seven of 42 patients), in one patient the cause of death was ventricular fibrillation. The late mortality rate is 14% (five of 37 patients), one patient had sudden cardiac death. Two patients had a recurrence of ventricular tachycardia postoperatively. In one of these an antitachycardia pacemaker was implanted, the other was successfully reoperated with endocardial resection. Postoperatively, a sustained ventricular tachycardia was inducible by programmed stimulation in three patients. Complex ventricular ectopic activity was found in ten patients, all of these were administered antiarrhythmic drugs. With respect to the poor prognosis of medically refractory ventricular tachycardia, we conclude that the electrophysiologically guided surgical approach has become an acceptable therapeutical alternative.

The effect of early meconium evacuation on early-onset hyperbilirubinemia.

Delayed passage of meconium has been associated with hyperbilirubinemia. To determine if early evacuation of meconium is effective in reducing total serum bilirubin level, 84 term healthy neonates were randomly assigned to a suppository group or a nonsuppository group. Use of suppositories resulted in earlier evacuation of meconium as measured by the appearance of transitional stool. No significant effect was noted, however, on total serum bilirubin levels in the first three days of life. Routine use of suppositories to decrease total serum bilirubin level cannot be recommended for term healthy neonates.

Percutaneous trigeminal thermorhizotomy in treatment of malignant facial pain.

Fifty-eight patients suffering from intractable pain in the trigeminal distribution due to neoplasms of the intracranial or facial regions underwent controlled percutaneous coagulation of the Gasserian ganglion (trigeminal thermorhizotomy). Of them, 35 were evaluated at varying time intervals from the intervention (mean: 7 months). While pain was completely relieved in 71 per cent of cases at discharge, the follow-up examination was consistent with 49 per cent of analgesias. The rationale of percutaneous thermocoagulation in malignant facial pain is given and the pertinent literature summarized.

How many cases of true precocious puberty in girls are idiopathic?

Fifteen girls with true precocious puberty were examined by computerized tomography. In seven hypothalamic hamartomas were suspected. A pneumoencephalogram was performed in six cases (one patient refused), with the following results. In five, the radiological features were highly suggestive of tuber cinereum hamartoma. All of our patients with pubertal signs appearing before the age of 2 years and 80% of the girls with early menstruation were in the group with suspected hamartoma. The luteinizing hormone and follicle-stimulating hormone levels of these five girls were significantly higher than those observed in the other girls with idiopathic precocious puberty. We conclude that there is a high frequency of small hypothalamic masses (suspected hamartoma) in girls with true precocious puberty (33% of the patients in our group), that it is important to confirm the presence of the mass with pneumoencephalography, and that surgery for diagnostic and therapeutic purposes should be carefully considered, given the absence of any neurologic symptoms for a long time after the appearance of the first pubertal signs.

Structure and function of L-lactate dehydrogenases from thermophilic and mesophilic bacteria. II) The primary structure of thermophilic lactate dehydrogenase from Bacillus stearothermophilus. Cyanogen bromide fragments and partial sequence.

The polypeptide chain of thermophilic lactate dehydrogenase from Bacillus stearothermophilus was split with cyanogen bromide. The 6 cyanogen bromide fragments were then separated and isolated by gel filtration (Bio-Gel P 10, Sephadex G-75) and ionic exchange chromatography (Biorex 70), respectively. Peptide fractionation was performed in 50% formic acid. Fragment yield varied between 30 and 75%. About 75% of the amino-acid sequence was determined by the automatic N-terminal sequence analysis (amino-acid sequenator) of the cyanogen bromide fragments (41-57 cycles degraded) and N-terminal region of lactate dehydrogenase (74 cycles degraded). Typical structure differences between thermophilic and mesophilic lactate dehydrogenases are already indicated by the comparison of the amino-acid composition of the thermophilic enzyme from B. stearothermophilus with the mesophilic from bacilli and higher organisms. Comparison of the N-terminal sequence reveals that sequence homology is higher (83-98%) between the thermophilic lactate dehydrogenases from B. stearothermophilus, B. caldotenax and B. caldolyticus than between the mesophilic lactate dehydrogenases of bacilli among each other or between thermophilic and mesophilic lactate dehydrogenases (about 60%). High temperature would appear to limit variation in structure.
