Albumin as one-way transport vehicle into sites of inflammation.

Intravenously injected Evans blue combines with albumin. The resulting complex appears in the inflammatory exudate that collects in Sephadex G 200 subcutaneously injected into rats. A comparison between the concentrations of albumin and Evans blue in the blood and in the inflammatory exudate shows that the life-span of albumin in the Sephadex is drastically reduced. After decomposition of albumin, the combined Evans blue is liberated, accumulates in the Sephadex and, after 24 h, attains concentrations higher than in the blood. The experiment is a plausible model for transport and deposition of albumin-bound drugs into and at sites of inflammation.

Laboratory handling and its influence on hormonal and metabolic parameters during acute inflammation in rats. A critique of long-term treatment by repeated injections.

Twice a day, rats were exposed to handling stress by sham i.p. injections during a period of five days. This procedure progressively drove up the basic blood levels of free adrenaline, noradrenaline and dopamine, while insulin decreased to below normal. Blood glucose returned successively from hyperglycemia during the beginning of the experiment to normal later on. If rats subjected to acute inflammation were handled, the parameters blood catecholamines, glucose and the lymphocyte ingress into the site of inflammation all showed the same patterns of changes, suggesting a certain synchronization of the metabolic events. This synchronization failed to occur in animals with inflammation, but without handling. In any case, the disturbed metabolism of handled animals did not normalize during the test period of five days. Therefore, application of test substances by repeated injections is a useless method for the correct investigation of chronic exogenous influences.

[Misoprostol and cimetidine in the treatment of duodenal ulcer. A multicenter Swiss double-blind study].

The efficacy and tolerance of misoprostol a synthetic prostaglandin E1 analogue-and of cimetidine were evaluated in the treatment of duodenal ulcer following a double blind multicenter design. A group of Swiss gastroenterologists recruited 119 patients with endoscopically proven acute duodenal ulcer. They were enrolled and treated according to a uniform protocol either with misoprostol 4 X 50 micrograms daily, with misoprostol 4 X 200 micrograms daily or with cimetidine 4 X 300 mg daily. Patients who were healed after 4 or 8 weeks were followed during 6 months. Cumulative healing rates at 4 weeks in groups receiving misoprostol 200 micrograms, misoprostol 800 micrograms or cimetidine 1200 mg daily were 42%, 58% and 71% respectively. Patients treated with cimetidine showed a more rapid improvement of symptoms than patients treated with misoprostol. Symptomatic recurrences after ulcer healing occurred in almost 40% of the patients within 6 months, irrespective of the type of acute ulcer treatment. Risk factors for slowed healing and increased rate of recurrence were evaluated in the present study, and the results were compared with the results of two previous trials performed according to a similar protocol. Smoking appeared to slow healing and to favor relapses. Moderate consumption of alcohol appeared to decrease the relapse rate. In the present trial, ulcer size was associated with ulcer pain but not with ulcer healing.

Vasoconstriction of the vasa afferentia in the rat kidney after long-term adrenaline application.

Adrenaline applied in the form of a retard tablet was implanted subcutaneously under short ether anesthesia under the neck skin of rats. Animals in the V 15 group received one 15 mg adrenaline tablet and those in the V 30 group two 15 mg tablets. The tablets were removed after 24 h. Twenty-two h later plasma catecholamines with the exception of dopamine are elevated in accordance with the amount of adrenaline applied. Histological examination reveals hyalinization of the vasa afferentia and glomerula which is more expressed in the inner part of the renal cortex. Hyalinization begins in the vas afferents and juxtaglomerular cells and extends to the glomerulus. Electron microscopic examination of the vas afferents shows vasoconstriction and beginning cellular degeneration of this vessel. Vasoconstriction is considered to be responsible for the decrease in kidney function.

Functional and morphological changes in the rat kidney after long-term adrenaline application.

A 24 h lasting long term adrenaline application in the form of subcutaneously implanted retard-tablets causes functional and morphological changes in the kidney, which are dose dependent and either reversible or irreversible. Animals in the V 15 group received one 15 mg and in the V 30 group two 15 mg adrenaline tablets resp. The tablets were removed after 24 h and a clearance depot capsule was implanted in the abdominal cavity. The test period extended from 22 h after removal of the retard tabl. to 21 d thereafter. Inulin (except the V 30 grp.), PAH and urea clearance are showing no significant differences. The data of urine gamma-GT activity, urea and urea-N in serum and urine, S-glucose, S-triglycerides, Combur-8-test, Na+ and K+ levels are in normal ranges. Morphological changes were found in the glomerula and tubular regions. Both the number of damaged nephrons and the extent of the damage depend upon the doses of adrenaline given. All glomerula with visible serious changes are irreversibly damaged; this was not true in case of the tubule. In the latter, after 7 d, the fatty deposits had diminished or disappeared.

Adrenaline application by controlled release system shows that it does play a physiological role in glycogenolysis.

In a frequently cited paper Sokal , Sarcione and Henderson (1964) doubted the physiological glycogenolytic role of adrenaline (A). By using isolated perfused rat livers, they found adrenaline to be effective at doses higher than 140 ng/ml while a mere tenfold increase in glucagon leads to expressed glycogenolysis. Our in vivo experiments carried out with controlled release systems for adrenaline show that marked glycogenolysis takes place at an adrenaline serum level of not more than 20 ng/ml while endogenous glucagon levels do not differ from controls. We think, that the reason for those controversial results lies in the fact that Sokal , Sarcione and Henderson (1964) diminished the glycogenolytic action of adrenaline by blocking its alpha-component for the reason of an easier perfusion, and they further diminished its glycogenolytic action by omitting corticosterone, which is well known for its permissive role in adrenaline induced glycogenolysis in vivo.

[Effect of a weak magnetic field on the course of an acute inflammation in the rat].

To determine the influence of weak magnetic field on the development of inflammations, a non-specific acute inflammation was induced in rats by subcutaneous injection of sterile SEPHADEX-gel in the lower dorsal quadrants. The magnetic field applied had a strength of 10 Gauss (equivalent to 20 times the magnetic field of the earth), rectangular curve and a frequency of 10 Hz. Following a field exposure of 2 and 3 days, cytological and biochemical analyses of the blood and inflammatory exudate were performed. Parameters were leucocytes and proteins in the blood and in the focus of inflammation as well as blood electrolytes. The protein values in the focus of inflammation of the control animals remained unchanged on the second and third day whereas in the animals exposed to the magnetic field, they increased from the second to the third day. In the blood similar effects could be seen, but they were not as pronounced. We conclude from these data that the magnetic field causes an increased exudation of serum proteins in the focus of inflammation. The cellular component remains unchanged.

Results of large scale media antismoking campaign in Australia: North Coast "Quit for Life" programme.

A three community study was carried out in northern New South Wales, Australia, to determine the effectiveness of interventions via media and media plus community programmes in altering behaviours of life style, including smoking, dietary fat intake, and exercise. Results were analysed of one aspect of the "North Coast Healthy Lifestyle Programme" entitled "Quit for Life," which aimed at reducing cigarette smoking over a two year period. There was a significant decline in the prevalence of smoking in both test towns, ranging from 16% in younger men to 6% in older women. These were significantly greater than the declines in the control town. The declines were more sustained over the two year period in the town subjected to media plus community programmes.

[Recurring pseudo-urolithiasis].

We report about a case with simulated recurrent urolithiasis. The patient introduced foreign bodies (gypsum) into the bladder. Often the stones had to be removed from the bladder and the right refluxing ureter. The diagnosis of the foreign bodies was made after 2 years of malingering by X-ray diffraction (analysis: gypsum = CaSO4 . 2 H2O).

Comparison of adrenaline-induced hyper- and hypoglycaemia by histological and biochemical investigation.

Chronic treatment of rats by subcutaneously implanted A-depot capsules lead through a ca. 12 h hyperglycaemia to hypoglycaemia at 18 h p.o. SDH and G-6-Phase activities in the liver were compared during hyper- and hypoglycaemia using both histochemical and biochemical methods. The results show high triglyceride-contents, lack of glycogen and high G-6-Pase activities in the livers of animals during both hyper- and hypoglycaemia. Striking differences were found concerning SDH-activities: While biochemical measurements show enhanced activities during hyperglycaemia, hypoglycaemic values remain in the "normal" range. Nevertheless histochemical investigations show an amazing re-distribution of SDH-activities from the periportal areas towards the lobule centre. Therefore we think, that biochemical and histochemical methods should be combined to unveil such pseudonormalities. Further, this re-distribution shows the ability of the liver to cope with damage by shifting important processes away from fat-infiltrated areas.
