Pathology of prostatic cancer.

From 1972 to 1975, 6758 clinical prostatic carcinomas were recorded at the prostatic carcinoma registry in Homburg/Saar. 52% of the patients are between the age of 65 and 75, 22% younger than 65. We distinguish between 4 basic types: the highly and poorly differentiated adenocarcinomas, the cribriform and solid anaplastic carcinomas. 46% of the carcinomas are of uniform architecture, 54% are pluriform differentiated whereby very often the cribriform carcinoma is present. 82% of all the cases and over 70% of the carcinomas in men under the age of 55 can be allotted to the stages B to D according to FLOCKS. All histologic patterns are found in young as well as in old men. Fastly proliferating and poorly differentiated prostatic carcinomas are predominant. Contrary to these features there are in stage 0 and the latent carcinoma highly differentiated adenocarcinomas in 41.8%, e.g. 57% with obviously low malignancy. They remain symptom-free so that they are only discovered incidentally during lifetime or during autopsy. The incidental carcinoma represents only 7% of the biopsy volume, the latent carcinoma 36.4% in men after the age of 45. Its frequency, the number of tumour foci and extension in the prostate increase with progressing age. The course of the clinical prostatic carcinoma under therapy can be controlled via biopsy. Thus, it is possible to discover in about 20% of the cases primarily therapy-resistant tumours. Other tumours show a complete regression and aside we find tumours with temporary regression and further tumour progression. Primarily resistant to conservative treatment are the transitional cell carcinomas of the prostate which, therefore, have to be differentiated from the solid prostatic carcinomas.

[Indirect lymphography of the urinary bladder in animal experiments (author's transl)].

Of three contrast media injected interdigitally and submucosally as well as instilled into the bladder, Lipiodol UF (particle size of over 15 micron) and the experimental compound AG 52-315 were not resorbed. Another experimental compound (AG 60-99) with an iodine content of 20% and a particle size of ca. 3,5 micron produced, when injected interdigitally, good contrast of the popliteal lymph nodes in all cases (n = 5). Instillation of the same contrast medium into the bladders of five dogs yielded subdued contrast of the paravesicular lymph nodes in one case. Of still another five animals, two showed contrast of the inner iliac lymph nodes after submucosal infiltration of the bladder mucosa with AG 60-99. In all cases (n = 15) inflammatory reaction with necrotic formations were observed. Resorption of the contrast medium was accomplished by heating at 39 degrees C, and was not improved by addition of Hyaluronidase. Because of the low local tolerance of the contrast medium preparation, this successful experiment in indirect lymphography of the urinary bladder of animals cannot be applied clinically.

Latent carcinoma of prostate at autopsy in seven areas. The International Agency for Research on Cancer, Lyons, France.

A world-wide comparative study of the frequency and characteristics of latent carcinoma of the prostate was undertaken in seven areas, using standardized methods and "blind" microscopic evaluation in order to reduce selection and observer bias. The morphological features of 350 latent carcinomas found in 1,327 prostates were examined. Two Chinese populations, from Hong Kong and Singapore, showed a low frequency of latent carcinoma in comparison with western Europeans in Sweden and the Federal Republic of Germany and negroes from Jamaica; an intermediate position was found for Israelis and black Ugandans. The frequency of small latent carcinomas was about 12% in all the areas investigated and did not vary with age. Rates for larger latent carcinomas increased sharply with age ans showed an area-to-area variation resembling that of clinical carcinoma of prostate. The small carcinomas were almost exclusively situated in the outer half of the prostate and latent carcinomas of all sizes were evenly distributed between the anterior and posterior halves of the prostate and the right and left sides of the outer prostatic shell. Certain disagreements in diagnosis were noted when the sections from each area were evaluated independently by a different pathologist. Most of these disagreements were resolved by re-reading the sections; their occurrence had no significant effect on the geographical comparisons.

[Current problems in the pathology of the thyroid gland].

22 chloramphenicol allergics, 10 penicillin allergics and 6 patients with a chromate eczema as well as altogether 34 non-allergic control persons with healthy skin were examined according to the skin chamber method. After addition of the adequate antigen (chloramphenicol, penicillin, ammonium bichromate) into one of the two simultaneously applied skin chambers in sensitized persons an inhibition of the leucocyte migration in the antigen chamber develops. The polymorphonuclear neutrophilic granulocytes mobilised into the inflammation field were regarded as indicator cells. 5 chloramphenicol allergics with eczematous skin abnormalities and all control persons were negative. The investigation method is simply to be performed and therefore it is particularly suitable for clinical routine work.

[Prostate carcinoma: determination of progression following high-voltage therapy (author's transl)].

The fourth report of our long-term study concerns 70 cases of primary radiotherapy. The side effects of our radiation technique are minimal and temporary; in over 80% of the cases, potency was not affected. The morphologically demonstrable changes in the tumor tissue, in the original glandular parenchyma, and in the fibromuscular stroma leading to the vessels following high-voltage therapy are described. Three types of progressive patterns, based on the grade of histologic regression, can be determined. In more than half of the cases, with increasing distance from the source of radiation, no tumor or only minimal residual tumor was demonstrable. Sensibility and resistance of carcinoma of the prostate are not dependent upon the grade of differentiation of the tumor. A prognostic statement concerning the success of radiation therapy from initial histologic findings is not possible here. The occurrence of metastases in 18% of the cases following radiation probably resulted from the inexactness of the purely clinical determination of the stage of the tumor. The isolated, local treatment of prostate carcinoma through radiotherapy is justified only after exact determination of the stage of the tumor, i.e., certain exclusion of metastases via a diagnostic lymphadenectomy.

[Urothelial carcinoma in the prostate (author's transl)].

Some 3% of the prostatic carcinomas in the prostatic cancer register at Homburg are urothelial or transitional cell carcinomas. Based on morphologic and clinical findings, 102 cases are divided into two groups: 50 cases with and 52 cases without involvement of the urinary bladder. In each group there is a subgroup where common prostatic carcinoma is combined with a urothelial carcinoma. Adenocarcinoma can precede the urothelial carcinoma by years. The urothelial carcinoma at first grows predominantly intraductally, is resistent to hormone therapy and carries a bad prognosis. Of the 102 cases, 61 died within 2 years.

[Therapy of the carcinoma of the prostate and case control (III) (Author's transl)].

In a third information of our long-term study with now 304 cases we report on 54 cases with monotherapy-only orchiectomy, only hormones and combination of boths. With it the rectal palpable results have coincided significantly with the histological findings in all 3 therapeutic forms. As for the therapeutic result no difference is to be seen either histological or clinical after a 3 years' observation period. We cannot tell anything now about optimal methods of treatment with regard to the histological starting material. In all 3 therapeutic forms we observed regressions to stage O. The smallest plasma-testosteron concentration was found after orchiectomy, where no increase was recorded during the whole observation period.
