Direct diagnosis of unruptured ectopic pregnancy by real-time ultrasonography.

Real-time sonography was used in a prospective study of 32 patients as the sole diagnostic parameter for ectopic pregnancy. The minimal finding for diagnosis was an identifiable gestational sac with a circular pattern of echoes. Neither the presence or absence of an intrauterine sac nor the knowledge of either a positive or negative pregnancy test was used in the diagnosis. Among the 32 study patients, the diagnosis of ectopic pregnancy was made by sonography in nine (28.4%). Follow-up surgery confirmed the diagnosis in eight (89%). Among 23 patients with negative findings on sonography, only one was found on follow-up to have an ectopic pregnancy, for a 96% diagnostic accuracy for the negative group. Thus, of 32 patients with the potential diagnosis of ectopic pregnancy, 30 (94%) were correctly diagnosed using real-time sonography alone.

Histologic correlates of virulence in ovarian adenocarcinoma. I. Effect of differentiation.

The original histologic material of 87 patients with Stage III or IV ovarian adenocarcinoma was reviewed. These patients had been treated with cisplatin, alone or in combination. There was a significant association among the morphologic parameters of histologic differentiation, mitotic activity, and cellular pleomorphism. However, none of these was associated either with survival or with maximum response of the tumor among the cisplatin-treated patients. The possible significance of these findings is discussed.

Adenocarcinoma↗

Small cell anaplastic lung cancer presenting as an ovarian metastasis.

An unusual case of small cell anaplastic carcinoma of the lung, presenting as a pelvic mass, is presented. Histochemical and electron microscopic findings confirmed the diagnosis. The pathologic features and unique clinical characteristics are discussed with comments on the topic of metastatic ovarian tumors.

Treatment of recurrent and persistent germ cell tumors with cisplatin, vinblastine, and bleomycin.

Four patients were treated with vinblastine, cisplatin, and bleomycin for recurrent or persistent germ cell tumors. In all patients a clinical response was achieved. In 2 patients complete response was attained and they are free of disease following second-look laparotomy. One had a mixed germ cell tumor that persisted as dysgerminoma following primary chemotherapy. The other was a true hermaphrodite with pure dysgerminoma. In patients with endodermal sinus tumor transient response was achieved, but the tumors recurred and the patients died within 6 months.

Cerebral blindness in association with cis-platinum chemotherapy for advanced carcinoma of the fallopian tube.

A 62-year-old woman with a diagnosis of metastatic fallopian tube carcinoma was treated with cis-platinum and doxorubicin (Adriamycin) at a dosage of 50 mg/m2 intravenously every 3 weeks. After the 10th course of cis-platinum and doxorubicin, she complained of headache and inability to see. Physical examination was compatible with cortical blindness. The brain scan and computed tomographic scan were normal. Visual evoked potentials showed a delay in latency. Visual acuity completely returned over the next months, but right homonymous hemianopia persists. The temporal relationship between the findings and the treatment with platinum implicates this drug as the possible causative agent in the cortical blindness.

Chemosensitivity testing with CIS-platinum (2) diamminedichloride in squamous cell carcinoma of the cervix uteri.

The prophylactic Papanicolaou Test contributes in diagnosing dysplasias and carcinoma in situ of the uterine cervix with a realistic 100% cure. Surgery and radiotherapy improved considerably quantity and quality of life in stages 1 and 2 lesions. However, stages 3 and 4 have shown little improvement in results. Systemic treatment with chemotherapy offers hope for control. Most of the drugs are ineffective against squamous cell carcinoma of the cervix. Experimental and clinical trials with CIS-platinum (2) diamminedichloride (DDP) show effectiveness against cervical carcinoma. In an effort to individualize the treatment, a chemosensitivity testing (CST) is applied analogous to the radiosensitivity testing (RST). Basically, a control biopsy and posttest dose of DDP biopsies are studied to evaluate the response using objective morphological criteria. This safe, relatively simple, clinically applicable approach offers an objective dimension in selecting the patients and in monitoring the treatment. An important observation that gives this study an original authenticity is the fact that radiation and chemotherapy affects the cells in an identical way. This similar morphological reaction most probably means an analogous biological expression of cellular injury and necrosis of the neoplastic cells.

Improved chemotherapy for ovarian cancer with cis-diamminedichloroplatinum and adriamycin.

In a prospective controlled randomized trial, patients with advanced ovarian carcinoma (FIGO Stage III or IV) were treated with cis-diamminedichloroplatinum (II), (DDP), alone, DDP and Adriamycin (ADM), or Triethylenethiophosphoramide (ThioTEPA) and methotrexate (MTX). DDP alone produces objective responses in 31% of evaluable patients, ThioTEPA and MTX in 36%. The combination of DDP and ADM produces the best response rate, 80% (.01 less than P less than 0.25). The risk of progression or death is substantially reduced for the two DDP regimens combined when compared with ThioTEPA-MTX (P = .03). Multivariate analysis further suggests the superiority of the two DDP regimens because poorly differentiated tumors and large, greater than 2 cm, residual tumors failed to produce their usual adverse effect on prognosis when patients were treated with the two DDP regimens. Patients with poorly differentiated tumors or tumors of unknown grade treated with platinum or DDP-ADM experienced better survival than similar patients treated with ThioTEPA (P = .01).

Intraperitoneal infusion of doxorubicin in the treatment of gynecologic carcinomas.

Intraperitoneally administered doxorubicin yields high local concentrations that are expected to deliver higher doses to clinically important peritoneal and lymphatic metastatic sites. Two infusions of 20 mg and 30 mg in a patient with residual microscopic intraperitoneal ovarian cancer eradicated her disease. Quantification of serum doxorubicin by HPLC revealed significant levels of the drug and its major metabolite lasting 72 hours. A 50-mg infusion induced complete remission in endometrial carcinoma; the patient died in 6 months without evidence of abdominal tumor. Complications due to subcutaneous infiltration of doxorubicin and possible systemic effects of sustained high levels of the drug need further investigation to enable the full exploitation of this modality.
