Effects of cholecystokinin, food intake and cephalic stimuli on plasma levels of amylase, lipase, and immunoreactive cationic trypsinogen in rats.

Plasma levels of amylase, lipase, and immunoreactive cationic trypsinogen (ICT) were monitored in conscious rats to study the effects of cholecystokinin octapeptide (CCK-8) plus secretin administration, cephalic stimuli, and food intake. Stepwise increasing doses of CCK-8 (1, 5, 15, 30 Ivy dog units: IDU/kg/h) caused significant dose-related increases in plasma levels of each enzyme in a similar manner as those previously observed for response patterns of CCK-induced exocrine protein secretion in the same species. ICT showed the greatest response to CCK-8 with a maximal concentration 30 times above basal levels resulting in the steepest slope of the dose-response curve. Plasma lipase showed a maximal response that was 5 times above the basal level, while the plasma amylase level was increased only by 50% at the maximal response. Computed ED50 of CCK-8 for each enzyme confirmed this relative sensitivity of the response: 3.0, 8.7, 11.0 IDU/kg/h for ICT, lipase, and amylase, respectively. Plasma levels of amylase and lipase did not change significantly in response to the intake of either a liquid diet or fiber pellets containing no caloric value. Plasma ICT levels, however, were elevated significantly by 23 and 53% at the time when the liquid diet or fiber pellets were given and when cephalic stimulation appeared maximally induced. The increased levels declined thereafter and were no longer significantly different from the basal levels for postprandial 2 h.(ABSTRACT TRUNCATED AT 250 WORDS)

Partial and total skinning vulvectomy in treatment of carcinoma in situ of the vulva.

From July 1974 to December 1984, 24 skinning vulvectomies (17 partial and seven total) were performed in 24 patients with carcinoma in situ of the vulva. The objective for partial skinning vulvectomy was preservation of the cosmetic and functional integrity of the vulva in younger and sexually active patients in whom a steady increase in the incidence of vulvar intraepithelial neoplasia has been observed in the last decade. Two groups selected for partial skinning vulvectomy with skin graft were patients with multicentric vulvar intraepithelial neoplasia with colposcopic evidence of normal skin, and patients with localized but a wide surface area of vulvar intraepithelial neoplasia such that poor cosmetic results would be unavoidable should excision be approximated with primary skin closure. Only patients with involvement of the entire vulva were subjected to total skinning vulvectomy with total skin graft replacement. Recurrence and/or persistence were observed in two patients, both treated with total skinning vulvectomy. At present, all 24 patients remain free of detectable neoplasia on follow-up of six months to 11 years and all have very good cosmetic and functional results.

The cyclophosphamide, hexamethylmelamine, 5 fluorouracil (CHF) regimen in the treatment of advanced and recurrent ovarian cancer.

Fifty-one patients with advanced ovarian carcinoma were treated with a combination of cyclophosphamide, hexamethylmelamine, and 5-fluorouracil, CHF. Compared to the hexamethylmelamine, cyclophosphamide, methotrexate, and 5-fluorouracil (Hexa-CAF) regimen, the omission of methotrexate in CHF did not detract from its antitumor activity and it was well tolerated with only mild to moderate toxicity. The CHF combination was as effective as Hexa-CAF and was particularly active in patients who had nonmeasurable/residual disease, classified as less than 2 cm in its greatest diameter at the initiation of chemotherapy. In future studies, CHF should be prospectively compared to other combination using Adriamycin and cis-platinum that with extended use can cause renal and cardiac damage in long-term survivors.

Single agent vs. combination chemotherapy for ovarian cancer.

Forty evaluable patients with advanced epithelial cancers of the ovary received chemotherapy. Twenty-seven previously untreated patients underwent a 1:1 randomization between a combination of Cytoxan, Hexamethylmelamine, and Fluorouracil (CHF) versus a single agent, L-phenylalanine mustard (L-pam). Thirteen patients previously treated with other therapies received CHF as a second-line therapy. Eighty-five percent of the patients receiving triple therapy were responders, versus 57% in the single agent group. Fifty percent of the CHF group had a complete response versus 17% in the L-pam group (p = 0.09). All patients with complete resection of less than 2 cm residual disease at primary surgery were responders, regardless of the type of therapy. Response in these patients is defined in terms of disease-free interval. The importance of maximal surgical resection in management of these cancers is discussed. Five of eight patients treated with CHF undergoing second-look operations had no evidence of disease. One of three L-pam treated patients had no evidence of disease at second-look surgery. Six of 13 patients (46%) had partial response to CHF as a second-line drug.

Spinal cord lesions in heat stroke.

A clinical and pathological report of a patient who died 15 days after suffering a classic heat stroke is presented. The clinical picture was of a flaccid quadriplegia with anhidrosis and sphincter disturbance. The most severe pathological lesions were to be seen in the anterior and intermedio-lateral horns of the spinal cord. Hyperthermia was the only physiopathological mechanism demonstrated in the patient. It is suggested that the motor neurons and vegetative neurons of the spinal cord are specially sensitive to hyperthermia.

[Biochemical characterization and study of the sensitivity to Plasmodium falciparum infection. Apropos of a case of double CN Baltimore hemoglobinopathy discovered in Senegal].

The discovery of a double CNBalt hemoglobinopathy allowed the authors, after a family background investigation, to study the possible biological effects linked to that dual heterozygoty and to look for a protection against Plasmodium falciparum infections. It hasn't been possible to make evident any protection.

Measurement of blood flow through surgical anastomosis using the radioactive microsphere technique.

Two different radioactive microspheres (141Ce and 46Sc) were used to measure blood flow to an area of the large intestine in dogs before and after a surgical resection was performed with surgical staples. The healing of an anastomosis is theoretically related to the blood flow to the anastomotic site. Blood flow studies were conducted in three dogs using this technique. The average blood flow preoperatively was 0.558 mL/minute per gram and 1.04 mL/minute per gram postoperatively. The standard deviation was +/- 0.16. These results indicate a statistically significant (P less than .05) increase in blood flow at the anastomotic site six days after anastomosis when compared with the blood flow to the same area before any surgical procedures.

Stage III epithelial ovarian cancer: the role of maximal surgical reduction.

One hundred forty-two cases of ovarian cancer of epithelial origin treated at Georgetown University Hospital between 1974 and 1980 were analyzed. Seventy-five patients (52.8%) were found to have Stage III disease at the time of the initial laparotomy, and the purpose of this study was to assess the outcome of the management in this group of patients. In spite of an aggressive approach, complete tumor excision could only be affected in 13 of the 75 Stage III cases (17.3%). Residual disease of less than 2 cm was achieved in a further 8 cases (10.7%). The ability to perform complete tumor clearance bore no relationship to the grade of the tumor. The survival rate in the complete clearance group was 100% with a mean survival time of 45 months (range 10-90 months) and 10 of these patients have been shown to be free of disease by second-look laparotomy. The outcome in terms of survival and disease-free status in these two groups of patients was unaffected by various chemotherapy regimens and the only factor of importance appeared to be the success of the initial surgery in clearing the disease.

Preliminary evidence of natural resistance to Mycobacterium bovis (BCG) in lepromatous leprosy.

An assay system has been developed based on radiometric quantification of 3H uracil incorporation into viable BCG in the absence or presence of blood monocytes in cultures from untreated lepromatous (LL) or tuberculoid (TT) leprosy patients. 3H Uracil incorporation into BCG was inhibited when the bacilli were cultivated in the presence of blood-derived macrophages in culture for four days, and that inhibition was always greater with macrophages harvested from LL patients compared to TT patients. The reasons for such an observed difference in humans are discussed according to our knowledge obtained in murine models of mycobacterial infections.

Intraoperative radiation in the treatment of advanced cervical cancer.

Nineteen patients with invasive cervical cancer were treated with intraoperative radiation and most of the patients subsequently received conventional external radiation therapy and intracavitary applications. The technique, resulting complications, and survival of the patients are discussed.

Measure of blood flow by the multiple radioactive microsphere technique in radiated gastrointestinal tissue.

In this study, two different radioactive microspheres were used to measure blood flow of an irradiated segment of small intestine in four dogs before, and 12 days after, irradiation with 2000 rad. The technique and implications are discussed. Using multiple radioactive microspheres, the study demonstrated an increased blood flow in irradiated tissues twelve days after a single dose of 2000 rad. There was also an increase in blood flow to adjoining nonradiated segments of intestine in the same animal. These observations may be of significance in clinical applications of radiation therapy and surgery. A major surgical concern is the impaired healing of irradiated tissue in the immediate postradiation period. The mechanism of this has generally implicated decreases in the perfusion of irradiated tissue. No decrease in blood flow was shown in this study, suggesting that other mechanisms, e.g., stem cell depletion, should be considered. Further studies of this type are recommended to increase understanding of the blood flow in irradiated tissue.

Colostomy closure with the automatic surgical stapler.

Surgical stapling instruments have been used successfully in gastrointestinal anastomosis. A surgical technique using the staples to close a loop colostomy in gynecologic oncology patients is presented. The procedure shortens operating time and results in an enlarged anastomotic site, thus minimizing the possibility of obstruction at the site.

The efficacy of radionuclide liver and bone scans in the evaluation of gynecologic cancers.

Between 1978 and 1980, 33 patients with cervical cancer, 40 patients with ovarian cancer, and 28 patients with uterine cancer underwent clinical diagnostic staging. Fifty-four patients had bone scans, and 101 patients received liver scans as a part of their staging work-up. No positive bone scan results were found. Eight of 33 patients with Stage IV disease had positive liver scan results consistent with hepatic metastases. Only 1 of 69 patients with Stage I-III disease had a positive test for metastases. While these numbers are small, the authors conclude that bone and liver scanning in asymptomatic patients with early stage gynecologic cancers may not be warranted as staging procedures.
