A new adjuvant treatment with polyadenylic-polyuridylic acid in operable breast cancer.

Adjuvant treatment with polyadenylic-polyuridylic acid poly A-poly U was tested in 300 patients with operable breast cancer who had all received the same locoregional treatment. They were randomized into two groups; 155 patients receiving 30 mg poly A-poly U i.v. once a week for 6 weeks and 145 controls. Overall survival was significantly improved in the poly A-Poly U group (P less than 0.05). The most striking difference (less than 0.03) was observed in the group of positive node patients, who had a 5-year relapse-free actuarial survival rate of 71% versus 47% in the controls.

Bacteremia caused by viridans streptococci in 71 children.

A review of the hospital records of 71 patients from whose blood viridans streptococci were isolated showed that in 13 cases the patient's illness was definitely related to the bacteremia: 4 patients had endocarditis, 3 had pneumonia, 2 had peritonitis and 1 each had meningitis, a scalp wound infection, sinusitis and otitis media. The bacteremia may have contributed to the two deaths among these 13 patients. In 45 cases the viridans streptococci may have contributed to the patient's illness: 15 patients had an infection of the lower respiratory tract and 7 an infection of the upper respiratory tract, 8 were neonates with suspected septicemia, 3 had soft tissue infections, 3 had leukemia and sepsis, and 9 had miscellaneous infections; the bacteremia was unrelated to the two deaths in this group. In another 13 cases the viridans streptococci could not be related to the patient's illness. The species most frequently isolated were Streptococcus mitis, S. sanguis II and S. MG-intermedius. The outcome of the bacteremia was generally good, even among the 11 patients not treated with antibiotics. When viridans streptococci are cultured from a single blood sample, further samples of blood and, if feasible, specimens from the associated focus of infection should be obtained for culture; further blood cultures are especially important in cases of suspected endocarditis.

Adjuvant treatment with polyadenylic-polyuridylic acid (Polya.Polyu) in operable breast cancer.

Adjuvant immunotherapy with polyadenylic-polyuridylic acid (PolyA.PolyU) was tested in a randomised trial on 300 patients with operable breast cancer, all of whom were treated by surgery with or without radiotherapy. They were randomly divided into an experimental group of 155 patients who were treated with 30 mg PolyA.PolyU intravenously per week for 6 weeks and a control group of 145 patients who received normal saline intravenously on the same schedule. The mean follow-up time was more than 50 months in both groups. The overall survival was significantly higher in the treated group (p less than or equal to 0.05), in whom the 5-year "relapse-free" surival was also increased. In node-positive patients, treatment increased the relapse-free survival (p less than or equal to 0.03) and overall survival (p less than or equal to 0.07). No side-effects were noted. Thus, immunotherapy with PolyA.PolyU appears to be a simple, non-toxic, and efficient adjuvant treatment in operable breast cancer.

Tuberculous meningitis in children: a retrospective study of 79 patients, with an analysis of prognostic factors.

A study of 79 cases of tuberculous meningitis in children, with an emphasis on prognostic factors, was conducted by means of a review of hospital records and completion of a follow-up questionnaire. The survival rate was 62%. Stage 3 disease (with severe changes in the sensorium and often severe neurologic abnormalities) at the time of admission, age 3 years or less, associated miliary tuberculosis and delay in the initiation of therapy correlated significantly with poor outcome. Severe neurologic complications in the acute phase were also related to poor outcome and severe sequelae. General recommendations pertaining to treatment are made.

Yersinia enterocolitica infection in children: 4 years experience in the Montreal urban community.

From 1973 to 1976, a total of 146 strains of Yersinia enterocolitica was isolated from children at the Ste. Justine Hospital and Montreal Children's Hospital. All strains tested were of serotype 0:3, biotype 4. The majority (80%) of children from whom Y. enterocolitica was isolated were under 5 years of age and in 74 of 93 cases reviewed diarrhea was the predominant symptom. Case reports on 2 patients with unusual clinical features are also presented.

Favus: twenty indigenous cases in the province of Quebec.

Twenty indigenous cases of favus in two families residing in the province of Quebec were studied. Importantly, the disease, although active, remained undiagnosed for many years in most of the cases. The finding of active cases suggests that the disease is still endemic in regions previously described as harboring cases, namely the province of Quebec and possibly Kentucky.
