Intestinal absorption and fecal excretion of 5,6 alpha-epoxy-5 alpha-cholesta-3 beta-ol by the male Wistar rat.

The intestinal absorption of 5,6 alpha-epoxy-5 alpha-cholesta-3 beta-ol, an oxysterol formed by cholesterol autoxidation, has been evaluated in the male Wistar rat. Measurement of the 14C/3H ratio in the serum (by the method of Zilversmit and Hugues) and in the feces showed that a large proportion of the epoxide was absorbed. Epoxide clearance from the blood was very rapid, but its excretion in the stool continued for several days, corresponding to the fraction of the epoxide stored in the animal.

Hypertrophy of liver peroxisomes in type II and type IV hyperlipoproteinemia.

A morphometric study on liver biopsies from patients with primary hyperlipoproteinemia (IIa n = 4, IIb n = 7, IV n = 7) and in controls (n = 7) was performed by light and electron microscopy. We found hypertrophy of peroxisomes in all subjects with hyperlipoproteinemia. As none of our patients had been given lipid-lowering drugs, this increase in volume seems to be due to hyperlipoproteinemia. Hypertrophy of peroxisomes in hyperlipoproteinemia may constitute a response of the hepatocyte to a disturbed metabolic state. Alterations of peroxisomes in hyperlipoproteinemia may thus occur prior to any lipid-lowering therapy.

[Elimination kinetics of organochlorine compounds in the 1st week of breast feeding].

The elimination kinetics of organochlorine compounds (DDE, lindane, aldrin, heptachlor epoxide and heptachlor) from day 2 to day 10 of breast feeding was studied in 39 volunteers who had a baby in the Maternity Department in Nancy. If there was any contamination of the mothers' milk with DDT, the levels were below the level of detection. In contrast, contamination with DDE was high (112.6 ng/g fat content) on day 2, although it decreased rapidly, to 38.6 ng/g by day 3. On day 2, contamination with lindane, aldrin, heptachlor epoxide and heptachlor averaged 43.8, 23.5, 25.4 and 7.4 ng/g fat, respectively, and the levels showed a linear decrease on subsequent days in all cases except that of heptachlor, for which no significant negative correlation with time could be demonstrated. A breast-fed infant thus absorbs measurable quantities of pesticides but not in excess of the acceptable daily doses. Levels of contamination of human milk in France are lower than those reported in Canadian and Italian studies but are higher than the levels in formulated milk.

The hepatocyte and type IIA and IIB hyperlipoproteinemia. A morphometric study.

We obtained liver biopsies in 4 patients with type IIA and 7 patients with type IIB hyperlipoproteinemia, and also in 7 healthy controls. A morphometric analysis of liver tissue was carried out by light and electron microscopy. Structural alterations of the hepatocyte and its organelles in type IIA and IIB hyperlipoproteinemia seem to fall into different categories: smaller nuclei, enlarged peroxisomes and altered mitochondria are found in both types of hyperlipoproteinemia, while an increase in the volume of hepatocytes is found only in type IIA. The enlargement of liver peroxisomes in untreated hyperlipoproteinemia is noteworthy, since it has been considered as a marker of the liver toxicity of lipid-lowering drugs.

Long-term effect of bran ingestion on lipid metabolism in healthy man.

The long-term effect of the addition of 20 g wheat bran to the diet is studied over 7 weeks in 4 healthy subjects. No change is found in plasma cholesterol and triacylglycerol. In this experimental design, we find no modification in the plasma levels of the various lipoproteins, except for VLDL cholesterol, which remains significantly lower after 7 weeks of wheat bran ingestion.

tcpO2, arterial blood flow and lactate/pyruvate modifications induced by a single dose of naftidrofuryl IV during stage III lower-limb arteriopathies.

A single dose of 600 mg of naftidrofuryl perfused IV during three hours in 12 stage III arteriopathic patients did not modify the average blood flow at the calf but did significantly increase transcutaneous oxygen pressure (33 vs 52 mmHg) and also improved lactates (1.44 vs 0.88 mmol/liter), pyruvate (59 vs 79 mumol/liter), and lactates-pyruvates ratio (33.48 vs 20.92).

Ultradian, circadian and circannual rhythms of blood glucose and injected insulins documented in six self-controlled adult diabetics.

The aim of the study was to document during one to two years individual rhythmic patterns in blood glucose and injected insulin in self-controlled insulin dependent (C-peptide negative) diabetics with home blood glucose monitoring. Two females and four males with diurnal activity from 0700 to 2300 self-determined their blood glucose three to six times a day over a period of 12-27 months. Circadian and ultradian rhythms were analysed for each subject on a monthly basis to document annual rhythms. Blood glucose (BG) estimated circadian acrophases were located between 2200 and 0300 for all patients and months with few exceptions. A correlation was found between circadian mesors and amplitudes of BG in four subjects. Annual changes in BG were validated for each subject with large interindividual differences in peak times. The individual mean of injected insulin (II) varied from 40 to 80 iU with annual changes validated for each subject. A group pattern was observed with a peak time either in the autumn (four patients) or in the summer (two patients). A circadian rhythm of II was detected in almost all monthly means and for all patients. Locations of computed peak time phi of II exhibited a great stability for a given individual but large interindividual differences. Thus the rather constant phi location of BG for all subjects contrasted with interindividual differences in phi locations of II. These results suggest that rhythmic changes in BG and II should be recognized when forming a realistic strategy for timing and dosing time(s) of insulin.

Effect of a 12-hour subcutaneous infusion of somatostatin-14 on blood glucose, insulin and glucagon levels in healthy subjects and insulin dependent diabetics.

We compared the influence of a 12-hour subcutaneous infusion of somatostatin-14 on glucose homeostasis in two normal subjects and two insulin-dependent diabetics (IDD). In all cases, somatostatin infusion led to a decrease of blood glucose during the first hours. The diabetogenic effect of somatostatin was confirmed in normal subjects. Postprandial blood glucose response was exaggerated in one insulin-dependent diabetic while in the other, glucose tolerance was improved. Unexpected high levels of immunoreactive somatostatin were measured in insulin-dependent diabetic patients. They might be due to a decreased somatostatin catabolism.

Long-term influence of a wheat-bran supplemented diet on secretion of gastrointestinal hormones and on nutrient absorption in healthy man.

Responses of gastric inhibitory polypeptide (GIP), gastrin and vasoactive intestinal polypeptide (VIP) to a test meal and also nutrient absorption were measured in five healthy men before and after 1, 3 and 7 weeks of daily ingestion of 20 g of wheat bran added to a normal balanced diet. Basal levels of the three hormones were not affected by bran ingestion. Bran ingestion induced a progressive decrease of GIP response to the test meal which became significant after 7 weeks at 30 min (373.9 +/- 71.4 vs 231.1 +/- 47.8 pg/ml, mean +/- s.e.m., P less than 0.05) and at 180 min (389.4 +/- 43.9 vs 262.2 +/- 37.9 pg/ml, P less than 0.01). Gastrin release did not change except for a slight but not significant decrease after 3 weeks. There was no VIP secretion after meal ingestion and addition of bran caused no change. Blood glucose response decreased with time with the greatest effects during the third week with fibre at 30 min (5.00 +/- 0.50 vs 7.38 +/- 0.05 mmol/l before bran), and at 60 min (3.88 +/- 0.34 vs 5.94 +/- 0.27 mmol/l before bran, P less than 0.05). Wet and dry weight of faeces increased by at least 60 per cent from the first week with bran onwards. Faecal nitrogen and fat also increased from 1.77 +/- 0.16 to 2.44 +/- 0.13 g/d for nitrogen (P less than 0.02) and threefold for fat (from 3.78 +/- 0.58 to 10.35 +/- 0.67 per cent dry weight, P less than 0.005) at the third week. [Caloric intake and weight gain according to the shift schedule of shift workers].

Survey involved 84 workers matched for age, socio-occupational and families status and divided into 3 groups : A : 27 shift workers on 3 days rotating shift, B : 47 shift workers on 5 days rotating shift, C : 20 days workers. Each subject was submitted to dietary survey by means of 24 hours recall, realised 3 times for shift workers and once for control. All workers were interviewed about caloric intake of the preceding day off caloric intake. Annual weight gain was studied through the use of occupational health service records. There was no significant difference between the 3 groups for working day caloric intake. Day-off intake was lower (p less than 0.05) in group A (day-off any day of the week). Annual weight gain was not different between the 3 groups. Shift workers with faster weight gain had a higher caloric intake on day-off and after evening meal.

[Sympathectomy of the digital arteries in acro-syndromes].

Digital sympathectomy as proposed by Flatt and al. was performed on 12 Raynaud's diseases, 6 distal arteriopathy and 4 post-trauma ischemic syndrome. Operation was performed under tourniquet. The approach depends on the number of digits involved (Brünner at the proximal part of P1, transversal in the distal palmar crease, zigzag incision at the level of each intermetacarpal space). Under microscope, adventisectomy is started on a lateral side of the artery followed by the posterior side on at least 1 cm, using the adventitia avoid all direct manipulation of the vessels. Closing skin on an aspirative drainage and simple non compressive drainage is maintaining 4 days. Index pressure and arterial blood flow are measured. Results are bad for post-trauma ischemic syndrome and arteritis but encouraging in Raynaud's disease (7 patients asked for the same operation on the opposite side).

Influence of chronic ethanol intake on obesity, liver steatosis and hyperlipidaemia in the Zucker fa/fa rat.

The effect of chronic alcohol intoxication on metabolic disturbances and fatty infiltration and degeneration was studied in genetically obese, hyperlipoproteinaemic, fa/fa Zucker rats. Sixteen obese Zucker (fa/fa) rats, sixteen lean Zucker rats (Fa/-) and sixteen Wistar rats, all male rats aged 7-8 weeks, were given either a control (C) diet (13% of energy from protein, 37% from fat, 50% from carbohydrate) or an ethanol (E) diet (13% of energy from protein, 37% from fat, 14% from carbohydrate, 36% from ethanol) for 4 weeks. The fa/fa rats given diet E consumed more energy than those given diet C, but after 4 weeks the weight gains and degrees of obesity were similar for both groups. With both diets, the developed hyperlipidaemia could be explained by the hyperinsulinaemia. Both hypertriglyceridaemia and hypercholesterolaemia were lower in fa/fa rats eating diet E than in those given diet C. Fatty infiltration of the liver, as assessed by hepatic triacyglycerol and cholesterol contents, was observed with both diets, but for fa/fa rats it was less extreme in those given diet E.

Paradoxical effect of ethanol on liver lipogenesis in the genetically-obese Zucker rat.

Sixteen obese (fa/fa) Zucker rats, sixteen lean (Fa/-) Zucker rats and sixteen Wistar rats, all male rats aged 7-8 weeks, were given either a control (C) diet containing no ethanol or an ethanol (E) diet in which 36% of the energy was supplied by ethanol, for a period of 4 weeks. The activities of glucose-6-phosphate dehydrogenase (EC 1.1.1.49), glucose-6-phosphatase (EC 3.1.3.9) and glycerol kinase (EC 2.7.1.30) and the glycogen content in the livers of obese (fa/fa) rats were lower in animals given diet E than in those given diet C. As a result, hepatic lipogenesis and fatty degeneration of the liver were reduced in obese (fa/fa) rats given diet E.

Human and animal fat cell size determination using an image analyzing computer.

An automatization of the fat cell's morphological parameters determination with an image analyzing computer is proposed. The results obtained with this technique are compared to results obtained from the usual methods. Combined with frozen cut slices of adipose tissue, this automatized determination of fat cell size is well adapted for the large number of samples necessary for a specialized hospital service.

Progressive adaptation of the endocrine pancreas during long-term protein deficiency in rats: effects on blood glucose homeostasis and on pancreatic insulin, glucagon and somatostatin concentrations.

Blood glucose, plasma insulin and glucagon, as well as pancreatic insulin, glucagon and somatostatin contents, were measured in control (C group, 18% casein), deprived (D group, 5% casein) and pair-fed (PF) rats at seven intervals during 23 wk after weaning (wk 0). In C rats, plasma and pancreatic insulin showed parallel variations, in D rats, plasma insulin increased normally until wk 3 after weaning, dropped from wk 3 to 8 and was higher in wk 16 and 23 than in wk 8, while pancreatic insulin increased linearly after a significant drop between wk 0 and 1. Insulin variations in D rats were related to protein deficiency until wk 5, but only to energy deficiency thereafter. Circulating and pancreatic glucagon dropped identically for the three groups until wk 5: its role in adaptation to malnutrition is quite irrelevant, although a dysregulation of its secretion was noted. Protein-energy malnutrition induced an increase of pancreatic somatostatin content that was due to the energy deficiency. On the basis of the insulin-to-glucagon ratio, three phases of adaptation to protein-energy malnutrition appeared. From wk 0 to 3, the metabolic priority was growth, whereas glucose homeostasis was secondary, accounting for the early hypoglycemia. From wk 3 to 8 survival was the main priority. After wk 8, the various biochemical parameters stabilized, and growth was parallel to that of normal animals. Protein-energy malnutrition was responsible for a dissociated adaptation of pancreatic hormones.

Serum concentrations of total T4, T3, reverse T3 and free T4, T3 in moderately obese patients.

Serum total thyroxine (T4), 3,5,3'-triiodo-L-thyronine (T3), 3,3',5'-triiodo-L-thyronine (reverse T3 or rT3) and free thyroxine (FT4), free 3,5,3'-triiodo-L-thyronine (FT3) concentrations were measured by radioimmunoassay in 52 moderately obese patients and 32 age-matched control subjects with constant body weight and no dietary restriction. The comparisons of iodothyronine concentrations in the obese and control subjects gave the following results: serum T4 and FT4 concentrations were not different in the two samples (mean +/- s.e. T4: 105.8 +/- 3.0 vs 109.4 +/- 4.0 nmol/l, FT4: 10.9 +/- 0.4 vs 10.8 +/- 0.8 pmol/l). A slight but non-significant decrease in serum rT3 concentrations was observed in the obese patients (0.29 +/- 0.02 vs 0.32 +/- 0.02 nmol/l). A slight (10 per cent) but highly significant (P less than 0.01) decrease was observed in the obese patients for serum T3 (2.20 +/- 0.06 vs 2.44 +/- 0.05 nmol/l) and FT3 (6.0 +/- 0.2 vs 6.9 +/- 0.2 pmol/l). Serum FT4 and T4 on the one hand and serum FT3 and T3 on the other were positively correlated both in the obese and control subjects. No correlation was found between serum T3 and weight expressed as body mass index. The results suggest that the most important factor responsible for the decrease in the serum total and free T3 concentrations is the nutrient intake.
