Meta-analysis of alcohol intake in relation to risk of liver cirrhosis.

The heterogeneity in the results of observational studies that investigated the association between alcohol consumption and risk of liver cirrhosis was analysed by means of a meta-analysis that included 15 articles published from 1978 to 1997. Relative risks associated with low levels of alcohol intake (25 g/day) ranged from 1.5 [95% confidence interval (CI): 1.4-1.5] for a linear model fitting the results of the six studies performed in Mediterranean areas, to 3.6 (95% CI 3.1-4.3) for a quadratic model fitting the results of the nine studies performed in other areas. A strong indication of heterogeneity was observed when combining all studies. Quadratic term of alcohol intake, quality of the study and area in which the study was performed explained most of this heterogeneity. Efforts should be made to explain the strong heterogeneity in the trend estimates. Reproducible methods to collect relevant and valid information on alcohol intake should be developed and the role of drinking patterns and viral and nutritional factors in modifying the effect of alcohol on the risk of liver cirrhosis should be investigated.

Incidence of malignant lymphoproliferative diseases by stage and histological variants in central Italy: a population based study 1982-1994.

A population-based epidemiological study that concerns the estimate of incidence rates of Hodgkin's Disease (HD) and Non Hodgkin's Lymphoma (NHL) in a Central Italy area was performed. All the new cases of HD and NHL diagnosed from 1982 till 1994 in the target population were collected by multiple information sources. The temporal trend of incidence rates and their relation with age, gender and histological variant were analyzed. In the considered period 95 cases of HD and 297 cases of NHL were collected. Constant age-adjusted incidence rates of HD were observed, while an increasing trend of NHL incidence rates was observed especially for cases at presentation in stage III and IV and for histotypes G, H and I. The investigations carried out on patients with HD have shown that this condition prevails in women in the younger age groups, while in men presentation occurs more frequently at an advanced age. Moreover the authors have confirmed previous reports of a distinctly increased incidence of nodular sclerosis in contrast to the other three histological variants which do not show a juvenile peak but a gradual increase in incidence with advancing age. The epidemiological features of NHL's observed correspond to the standard incidence rates obtained in Europe and throughout the world. NHL's appear to be a pathological entity typical of elderly patients: however high-grade NHL's, in contrast to low-grade NHL's, was present with increased frequency also in childhood and in patients under the age of 30.

[Incidence risk of major cardiovascular events among socio-occupational classes. 11-year follow-up of the MONICA Brianza and PAMELA cohorts].

The aim is to assess socio-occupational (SO) class differences in incident coronary and stroke. Methods. In a prospective fixed-cohort study 2959 35-74 year old men free from coronary (CHD) and stroke events were recruited in four population-based cohorts from 1986 to 1994 and followed-up until the end of 2002 to ascertain first CHD and stroke events. At baseline, major cardiovascular risk factors were investigated according to a standardised protocol. SO classes were based on current or last employment. Age-adjusted and multi-factors-adjusted risk ratios (RR) and 95% CI were calculated from Poisson regression models. Results. In 11-year median follow-up, the cohorts accumulated 33,926 person-years and generated 168 first major CHD events and 56 stroke events. Non-manual workers showed the lowest incident rates of both endpoints, and were considered as the reference category. Higher relative risks of CHD events were foundfor lower administrators and professionals, unskilled manual workers (UMW) and self-employed. Higher relative risks of stroke were found for skilled manual workers and UMW. Higher relative risks of CVD (either CHD or stroke) were found for lower administrators and professionals, skilled and unskilled manual workers and self-employed. Adjustments for major risk factors (cholesterol, systolic blood pressure, cigarette smoking, diabetes) did not modify the RRs. Conclusion. The higher risk of major cardiovascular events among lower SO classes is confirmed. Higher relative risks of CHD among lower administrators and professionals, and self-employers were also found. Further studies are needed to assess the etiologic role of job stress conditions.

[Aromatic hydrocarbons: environmental pollution and biological effects].

A complete explanation of the problem concerning aromatic hydrocarbons as polluting materials is presented. Both collective and individual sources of pollution, the collective sources, due to the human work activities, and the individual ones, caused by the habit to smoking have ben considered. The effects these compounds have in the human organism and particularly the carcinogenic action of polycyclic hydrocarbons have also ben discussed.

[Environment and biology of the population in the determination of blood lead levels].

The survey presented is an attempt to explain the reasons for the higher average lead concentrations in the blood of certain groups living in two mountain valleys than encountered in residents of Turin and other towns in the Province. To this end the influence of several environmental, individual and genetic variables was investigated.

[Analysis of maternal-fetal exchange in the study of environmental exposure to lead].

398 women at various stages of pregnancy and living in two areas subject to different levels of atmospheric pollution were examined. Blood samples were taken from about 200 women and their funiculi at the moment of parturition. The results showed a difference in lead concentrations in the blood and in the ratio of mother and funicular lead concentrations according to the place of residence. Several hypothesis to explain these results are formulated.

[Carbon monoxide: environmental and biological aspects].

The problem of carbon monoxide as a pollutant is examined. Its physico-chemical features are considered along with the sources of pollution (natural and technological), the characteristics of the pollutant in the environment and, particularly, the effects of car and industrial emissions and of cigarette smoke on the human organism.

Beta-interferon, retinoids and tamoxifen combination in advanced breast cancer.

PURPOSE: To investigate, in patients with metastatic breast cancer (MBC), a combination of retinoids, beta-interferon (beta-IFN) and tamoxifen (TAM). PATIENTS AND METHODS: This trial was conducted from January 1987 to June 1992 in 85 consecutive patients with MBC. All patients had been treated for metastatic disease: 43 patients received chemotherapy, 35 received hormone therapy and 7 received both. Forty-nine patients, with a better prognosis, and a longer disease-free survival (DFS) were treated with beta-IFN 1 x 10(6) IU/m2 subcutaneously three times a week, TAM 10 mg TID, retinyl palmitate (R) 50,000 IU BID. Thirty-six patients with shorter DFS, were first treated with 8 courses of chemotherapy followed, in responders, by beta-IFN, R and TAM. RESULTS: With a median follow-up of 7 years (minimum 4 years) 59% of 85 evaluable patients responded, 20% had stabilization of disease and 21% progressed. Median response duration was 43 months. Median survival was 28 months and 25% of patients were alive after 9 years. Toxicity of biological therapy was moderate. CONCLUSIONS: Our data show that beta-IFN, TAM and R combination is effective as salvage therapy in patients with MBC.
