Clinical evaluation of patients with squamous carcinoma of the oral cavity.

AIMS: To make a clinical evaluation of patients with squamous carcinoma of the oral cavity. METHODS: The clinical condition of 72 patients with squamous carcinoma of the mouth, hospitalized at the Institute of Oral and Maxillo-Facial Surgery, 2nd University of Naples, Italy, between 1992 and 1996, were examined. The patients were evaluated system by system, staged by TNM-UICC and classified by ASA and by the Karnofsky and Burchenal and the Zubrod et al. Performance status methods. RESULTS: Evaluation by the two methods produced different results. Of the patients examined, 34.5% showed more than one systemic disorder. 51.4% were in ASA classes 3-4. CONCLUSIONS: The presence of a disease with poor prognosis associated with advanced neoplastic stage (stages III-IV; 69.4% of the total) precluded radical surgery in 32% of patients, who were treated with chemotherapy and/or radiotherapy.

[A case of soft tissue alveolar sarcoma localized in the tongue].

Alveolar soft-part sarcoma is a rare form of malignant tumour. It is generally localized at the limb muscles, rarely in the head and neck. Its histogenesis is still uncertain but its specific cellular characters make the diagnosis easier. The case of a woman with an ASPS of the tongue is described. The morphological aspects and the immunohistochemical results seem, according to the data reported in literature, suitable with the diagnosis of alveolar soft-part sarcoma; however, it is difficult to explain the presence of local positivity to specific neuronal enolase.

[Palatal angioleiomyoma. A clinical case].

A case of angioleiomyoma localized at the level of the soft palate is reported. The 31-year-old patient presented a non-pathognomonic clinical picture and the anamnestic finding of the relatively slow growth of the neoformation raised problems of differential diagnosis in relation to benign growth and lesion of a non-neoplastic nature. The differential diagnosis was only obtained with histological examination of biopsy material.

[Surgical treatment of hepatocellular carcinoma: resection or transplantation? Results of a multivariate analysis].

This retrospectively study presents the results of a large series of transplanted or resected patients, with the aim of defining the characteristics of those patients who may benefit from resection or transplantation in an era in which these two surgical options can both be offered with low risks and extremely satisfactory results. Two hundred and seventy-five patients (154 resected and 121 transplanted) with hepatocellular carcinoma were submitted to surgical treatment in our department from December 1985 to December 1999. Age, sex, presence of cirrhosis, aetiology of liver disease, Child-Pugh classification, and alpha-fetoprotein levels were considered. Twenty-two of the 121 (18.1%) transplanted patients and 7 of the 154 (4.5%) resected patients died within 3 months of surgery. All curves show an evident trend towards increased mortality or recurrence rates in the resected group after prolonged follow-up. Liver transplantation appears to offer better survival and recurrence-free rates than liver resection in patients with hepatocellular carcinoma. Liver resection should be considered a good therapeutic alternative in patients who do not fulfill the transplant criteria.

[Epizoology of leishmaniasis in the province of Foggia: leishmaniasis in the dog].

The authors value the sensibility and the advantage of the different serological tests for the diagnosis of canine leishmaniasis (immunodiffusion, complement fixation and immunofluorescent tests) and give the results obtained from an epizoological survey carried out on dogs in the province of Foggia (Italy). The immunofluorescent test, whose positivity sera titre has fixed at the diluition of 1:10, resulted the most appropriate. In more than 400 animals examined, all from the 50 communes of the province of Foggia, the infection level of the dog by leishmania resulted to be 7.42%. If the three geographical zones, in which the province has been subdivided, are considered, this level varies from 10.21% for the subjects from the Gargano promontory, to 6.89% for those of the Subapennine Dauno, and to 5.55% for those of the Apulia plateau.

Canalicular adenoma of the parotid gland: a case report.

Canalicular adenoma is an uncommon benign neoplasm that occurs almost exclusively in the upper lip and, very rarely, in other sites. We describe a case arising in the left parotid gland as a firm, painful mass, in order to underline morphological and immunohistochemical findings, particularly in relation to differential diagnosis with low-grade carcinomas of the salivary glands.

Merkel cell carcinoma of the head and neck region: report of two cases.

Two cases of Merkel cell carcinoma (MCC) of the head and neck region are reported in order to stress their diversity in morphological, immunohistochemical and clinical findings. The remarkable variability of MCC has been analyzed, particularly in relation to the differential diagnosis and prognostic implications.

Glomus tumor of the lower lip. A case report.

A glomus tumor is a benign neoplasm originating from the modified smooth muscle cells of the vascular glomus. This neoplasm was firstly considered by Masson in 1924, and generally it has a solitary lesions but in some cases multiple lesions were described. Differential diagnosis is usually made with Kaposi sarcoma and hemangiopericytoma. The histopathologic examination is essential together with the immunohistochemical study. The clinical case of a 65-year-old man with a glomus tumor of the lower lip is described.

Absolute and relative activities of 4' -iodo-4'-deoxydoxorubicin against human colo-rectal tumors, as evaluated by a short-term in vitro assay.

A short-term antimetabolic assay based on the interference with 3H-thymidine and 3H-uridine incorporation after 3 hours of in vitro treatment was used to compare the cytotoxicity of a new halogenated anthracycline, 4'-Iodo-4'-deoxydoxorubicin (IDX), with that of its parent compound Doxorubicin (DX) against 44 human colorectal carcinomas. IDX had a marked dose-dependent effect, with frequencies of activity consistently greater than those of DX at all concentrations. The minimal dose of IDX required to induce a significant antimetabolic effect obtained by extrapolation from the dose-effect plots for each drug was 1/10 that of DX (2.3 micrograms/ml vs 23 micrograms/ml). When the relative activities of the two drugs on the same tumor specimen were determined, there was 71% to 86% overall agreement, depending on the concentration used. Lack of agreement was always attributed to sensitivity to IDX and resistance to the parent compound.

[Tooth heterotopy with endosinus localization].

Among the various cases of etherotophy, the only and multiple, that has observed, the Authors consider only a few among the significant ones and with endosinusal localization-after examining the probable mechanism patogenetic we pay attention to the necessity of an exact diagnostic judgement and on the opportunity of a precocious and exact treatment.
