[Experiences with oxytetracycline treatment of non-gonorrhea urethritis caused by Ureaplasma urealyticum].

In a retrospective study, the efficacy of tetracycline therapy was assessed in 48 men with non-gonococcal urethritis who only harbored Ureaplasma urealyticum in their urethras. After 2 weeks of therapy with 2.0 g oxytetracycline per day, U. urealyticum was still found in the urethra of 8 patients (17%). Urethritis was still present in 8 patients (17%) according to clinical criteria and in 11 patients (23%) according to microscopic findings. The persistence of U. urealyticum in the urethra did not correlate with the persistence of urethritis to a statistically significant degree. Therapy compliance, antibiotic resistance in a few cases, reinfections in 2 cases and a special host-parasite relationship are discussed with respect to the treatment failures.

[Risk factors for the development of malignant melanoma in West Germany. Results of a multicenter-case control study].

In this multicenter case-control study (1,079 melanoma patients, 778 control persons), the significance of the well-known risk factors for the development of malignant melanoma (MM) was assessed for a German population. The multifactorial analysis of the data confirmed the total number of melanocytic nevi (MCN) as the most robust indicator for an increased melanoma risk. For persons with more than 50 MCN the relative risk was 4.8 times as high as for persons with fewer than 10 MCN. Hair color was found to be another valuable indicator. In persons with red hair the risk of MM was 4.7 that in individuals with black hair. Remarkably fair persons with skin type 1 (always sunburn, never tanning) had two times the risk of that in persons with skin type 4 (never sunburn, always tanning). The habit sun bathing for recreation showed no influence on the development of melanoma. A 2.7 x increased melanoma-risk was detected in persons with occupational sun exposure and out of doors work.

[Florid oral papillomatosis: an indication for etretinate?].

Oral fluoride papillomatosis is a pseudocarcinoma of the mucous membranes, which often creates diagnostic and therapeutic problems. This paper reports oral flouoride papillomatosis in one patient, in whom the tumor mass could preoperatively be reduced by etretinate therapy (200 mg/day).

Follicular lymphoid hyperplasia of the skin with high content of Ki-1 positive lymphocytes.

We report the case of a spontaneously regressing lymphocytic lesion on the neck of a 76-year-old man. Routine histopathological examination revealed a heterogeneous infiltrate composed of both lymphoid follicles and preferentially perifollicular and interfollicular sheets of large atypical cells. Immunohistologically, large atypical cells were positive for Ki-1/Ber-H2 (CD30), anti-Il-2 (CD25), Ber T9, anti-HLA-DR, Leu-5b (CD2), T3 (CD3), Leu-3 (CD4), Leu-1 (CD5) and, occasionally, Leu-M1 (CD15); that is, they showed an arrangement and immunophenotype similar to those of perifollicular Ki-1 positive cells in reactive lymphoid tissue. In contrast, antibodies To15 (CD22), anti-immunoglobulins, and anti-CALLA (CD10) stained polyclonal B-cells restricted to lymphoid follicles and small clusters representing remnants of follicles. We suggest that this lesion be termed "follicular lymphoid hyperplasia of the skin with high content of activated T-helper cells."

[Fulminant acne in Klinefelter syndrome treated with testosterone. A side effect of anti-tallness therapy].

Acne lesions usually do not occur in patients with Klinefelter's syndrome (47 XXXY). But a 19 year old patient with Klinefelter's syndrome, under therapy with testosteronenantat (500 mg every two weeks over a period of 18 months) for treatment of excessively tall stature developed acne fulminans. Following discontinuation of testosterone treatment and isotretinoin therapy over 16 weeks skin lesions healed almost completely although with severe scars. In conclusion, high doses testosterone-treatment in excessively tall boys needs the additional care of dermatologist when mostly after a 7 months period acne begins to develop under this treatment.

Primary and secondary cutaneous Ki-1+ (CD30+) anaplastic large cell lymphomas. Morphologic, immunohistologic, and clinical-characteristics.

Skin biopsies were collected from 15 patients with cutaneous tumors morphologically similar to Ki-1+ anaplastic large cell (ALC) lymphoma of the lymph nodes, including Ki-1+ ALC lymphoma of childhood. The histology and immunophenotype of these cutaneous tumors are reported and follow-up data on the patients are given. The tumorous infiltrates were composed of large, sometimes very bizarre cells with one nucleolus or multiple nucleoli. All tumor cells expressed the lymphoid cell activation antigen Ki-1 (CD30) in conjunction with CD25 and the beta-chain of the T cell receptor. In 11 patients, Ki-1+ cutaneous tumors developed primarily in the skin. In nine patients, these were restricted to the skin in follow-up periods ranging from 3 months to 6 years. Two patients developed lymph node involvement after 2 months and 2 years, indicating the spreading potential of these cutaneous tumors. Morphologic and immunophenotypical identity of the atypical cells found in primary cutaneous ALC lymphoma, in regressing atypical histiocytosis (RAH), and in lymphomatoid papulosis (LyP) of type A, together with the protracted clinical course in all three conditions, suggests that primary cutaneous ALC lymphoma, RAH, and LyP of type A represent clinical variants of the same lymphoma entity. Secondary development of Ki-1+ ALC skin tumors was observed in two patients with cutaneous T cell lymphomas of mycosis fungoides type. These secondary Ki-1+ ALC lymphomas of the skin showed rapid systemic progression similar to the primary lymphonodal Ki-1+ ALC lymphomas. Concomitant or subsequent occurrence of Ki-1+ ALC tumors in cutaneous T cell lymphomas thus may be a bad prognostic sign.

[Malignant atrophic papulosis (Köhlmeier-Degos disease)].

On the basis of 106 patients with malignant atrophic papulosis in the literature, including one case of our own, the clinical, histopathological and nosological features of this disease are reviewed. Histopathologically, the typical papules with central porcelain-white atrophy show broad wedge-shaped necrosis of the connective tissue due to thrombotic occlusions of small vessels in the corium. The disease leads to death in 50% of these cases, mostly due to involvement of the gut and the central nervous system. Malignant atrophic papulosis is the prototype of a disease, in which skin lesions are the primary sign of a systemic disorder demanding the interdisciplinary cooperation of various specialists.

[Keratoacanthoma centrifugum marginatum].

Keratoacanthoma centrifugum marginatum (KCM) is a rare distinct variant of keratoacanthoma. Based on three personal observations and a review of the literature, the authors describe the clinical and histological features of this neoplasm. Clinically KCM is characterized by the lack of a tendency for spontaneous remission and by continuous centrifugal spread. Histologically there is a subclinical, iceberg-like growth pattern. Like keratoacanthoma, KCM is a highly differentiated, biologically benign, non-metastasizing tumour. The treatment of choice is early excision of the tumour.

Association of herpes simplex virus-induced erythema multiforme with the human leukocyte antigen DQw3.

We studied the genetic markers in human leukocyte antigen (HLA) region HLA-A, -B, -C, -DR, -DQ, C2, Bf, C4A, and C4B of 31 patients with erythema multiforme (EM) and their families. In contrast to the complement allotypes, which showed no deviation from the distribution in the normal population, two HLA class II antigens occurred in much higher frequency in patients with EM. The frequency of HLA-DQw3 (77.4%) increased with high significance compared with normal Caucasian control individuals (41.2%). An even stronger DQw3 association was found in the patient group with postherpetic EM (88.8%; relative risk, 9.41). Interestingly, all patients suffering from frequently recurrent EM were found to have the DQw3 allele (relative risk 44.2). The previously reported association to HLA-B15 was also seen and may be due to a linkage disequilibrium with the HLA-DR4 allele (66.6% in recurrent EM). Our data provide further evidence that classic recurrent EM is related to herpes simplex virus infection and should be regarded as a distinct entity within the enigmatic EM syndrome. DQw3 may serve as a helpful marker for distinguishing this entity from other diseases with EM-like lesions.

Immunostaining in atypical fibroxanthoma of the skin.

We have studied 12 cases of cutaneous atypical fibroxanthoma using immunohistochemistry to demonstrate lysozyme, alpha-1-antitrypsin, S-100-protein, receptors for peanut agglutinin, and intermediate filaments. Results were compared with immunostaining in 24 cases of other so-called fibrohistiocytic tumours. In addition 2 cases of atypical fibroxanthoma and 6 cases of fibrohistiocytic tumours were stained by monoclonal antibodies specific for the monocyte cell lineage (Ki-M1, Ki-M2, Ki-M6, Ki-M7, Ki-M8, OKM-1 and Leu-M1) and double-stained by monocyte-markers and Ki-67. The immunophenotype of atypical fibroxanthoma was rather similar to the marker profile found in malignant fibrous histiocytoma. All atypical fibroxanthomas were positive for vimentin and negative for epithelial markers. Monocyte lineage-specific determinants could be demonstrated in varying amounts in cells suggestive of being reactive. In contrast proliferating--Ki-67 positive--cells did not express monocyte/macrophage related antigens in atypical fibroxanthoma and malignant fibrous histiocytoma both. As to the histogenesis of these tumours our findings speak in favour of a derivation from primitive mesenchymal cells rather than from histiocytes.

Congenital spiradenoma.

A case of an adenomatous sweat gland hamartoma present since birth is described. The histological and ultrastructural findings were similar to those of spiradenoma, and carcinoembryonic antigen, usually present in cutaneous sweat gland tumors, was also demonstrated. Sweat gland nevi are rare, and a spiradenomatous variant has not been previously documented.

Trends in allergic contact sensitization.

A standard patch test series was tested for 7 years (1977-1983) in a total of 11,962 patients. The annual frequency of positive reactions to the compounds tested was assessed for the total and for males and females separately. During the observation period, there were significant increases in positive reactions in the total group from 6.2% to 12.7% for nickel sulphate, from 5.3% to 7.0% for balsam of Peru and from 3.8% to 6.5% for potassium dichromate, reflecting significant changes in both sexes. The frequency of positive reactions to wool alcohols, formaldehyde, neomycin sulphate, paraben mix and gentamycin sulphate significantly increased, while that of positive reactions to clioquinol, mercuric chloride and turpentine peroxide significantly decreased in either males or females, sometimes leading to significant changes in the total group.

[Antibodies against HTLV I in T-lymphoproliferative diseases of the skin].

The possibility that lymphoproliferative diseases may have a viral etiology has been discussed for many years. The first piece of evidence - in combination with convincing epidemiological data - was the detection of a virus of the HTLV group (HTLV I, ATLV) in patients with adult T-cell leukemia in Japan. There have been no comparable observations made in Europe. In 7 of 72 sera of patients with T-lymphoproliferative diseases in Central Europe, we were able to detect antibodies against HTLV I (ELISA and immunoblot), which might be an indication that retroviruses play a role in the etiology of these diseases.

[Local recurrent centroblastic lymphoma of the skin].

A 57-year-old patient suffered from a cutaneous centroblastic polymorphic non-Hodgkin lymphoma. This tumour first occurred 6 years ago and showed local relapses after two surgical excisions without any internal manifestations. The tumour had unusual morphological features with a sarcomatous growth pattern and numerous multilobated nuclei, which first led to the diagnosis of a malignant fibrous histiocytoma. Additional immunohistological studies of both the primary tumour and the lesions demonstrated the presence of lymphocytic marker proteins and allowed the definitive classifications as a high-grade malignant non-Hodgkin lymphoma of the B-cell type.
