[Early diagnosis of cancer in the area of the gastrointestinal system].

The early recognition of carcinomas is also in the abdominal region the aim of our permanent effort in the improvement of the prognosis of this disease. Only by a timely diagnosis a permanent cure is possible. Endoscopy and radiology are the corner-pillars of the diagnostics. At the least suspicion of a tumorous disease these methods should be used with all consequence. Since the methods mentioned cannot be used as mass examinations the control of risk groups are of great importance. In these groups of patients an actual care is possible. With the help of the operative endoscopy in form of polypectomy a decrease of the frequency of carcinoma might be achieved.

Colonic Neoplasms↗

[Tetracycline ulcer of the esophagus].

On the basis of 3 observations the author deals with acute ulcers in the oesophagus after oral application of oxytetracycline capsules. Sudden retrosternal pains, beginning in most cases during night, are in the foreground of the clinical picture. Discontinuation of the medicament and prescription of an antacid lead to the regression of the ulcer. For the development of such ulcers probably several factors play a role (pH-changes disturbances of motility and cytolysis). Oxytetracycline capsules should be taken always with abundant fluid and not immediately before bedtime.

Bronchitis↗

[Chronic peritonitis with ascites in chronic pancreatitis].

With the help of 2 casuistics is referred to the participation of the peritoneum in chronic pancreatitis. Within the disease mentioned a chronic peritonitis with ascites was observed. Such lesions at the peritoneum are probably rare. When proving a chronic peritonitis differential-diagnostically should also be thought of the presence of a chronic pancreatitis.

[Endoscopic demonstration and importance of proximal esophageal varices].

Proximal varicose veins of the oesophagus are endoscopically not so rarely found, when one seeks for them intensively. In altogether 2,041 endoscopies of the upper digestive tract they appeared 22 times in the course of 1 year. As cause are reported condition after resection of the struma, struma, bronchial carcinoma, mediastinal fibrosis and thrombosis of the subclavian artery. Among our patients dominated resection of the struma and struma. The proof of proximal varicose veins of the oesophagus should be a hint for seeking for all possible cause. In the intestinal haemorrhages they should be included in the differential diagnosis.

Bronchial Neoplasms↗

[Diabetic gastroparesis].

It is reported on a 61-year-old patient in whom since 1959 has been existing a diabetes mellitus in need of insulin. 1973 in the endoscopic control of roentgenological findings of a stomach suspect to tumour by means of the Wolf-Schindler gastroscope the suspicion to a gastric carcinoma was expressed. In May 1975 he was again admitted to hospital on account of a hypoglycemic shock. Roentgenologically large filling defects in the stomach were found, gastroscopically macroscopically no clearly explained whitish yellow masses (Histology: remains of food, chronic superficial gastritis, extended settlement of fungi). Only after several days of food carency and daily gastric lavages gastroscopically normal findings could be made. Together with the roentgenological findings of the stomach the diagnosis gastroparesis diabeticorum was made. Therapeutically an optimum stopping of diabetes is recommended and the application of metoclopramide. A surgical intervention is not advisable.

Diabetes Complications↗

[Diagnosis of an organic disease in an accentuated personality].

In a female patient with Dunbar-syndrome the cause of the complaints is not recognized during 18 years. Therefore, the symptomatology of the stenosis of the truncus coeliacus is briefly outlined. The patient reacts more and more neurotic on the misunderstanding of organically conditioned complaints, so that at last a consilium of experienced specialists makes the diagnosis "functional intestinal disturbances, hypchondriac depressive syndrome in accentuated personality". Therefore functional disturbances are to be diagnosed only then (also in the psychopathologic behaviour of the patients!), when an organic disease is excluded. The difficulty to justify such a demand is great.

Adult↗

[Morphological and clinical liver changes after taking oral contraceptives].

In 155 women of the age group 16-45 years after intake of oral contraceptives in 68.4% (106 cases) histologically provable liver changes were found. Sinusoidal ectasias of different size and drug-conditioned hepatitides most frequently occurred. An isolated intrahepatic cholestase only rarely existed. Increases of transaminase are the most frequent reference to a lesion of the liver parenchyma after intake of oral contraceptives. The author adopts a definite attitude to the differential diagnosis and its difficulties between a drug-conditioned hepatitis and the usual virus hepatitis.

[Experiences with ultrasound hepatography in the A picture method in the diagnosis of liver metastases].

In 600 patients by means of the A-picture method ultra-sound investigations of the liver were performed in order to find metastases of the liver. After an intermediate evaluation of 120 patients anatomical control findings by laparoscopy, operation and section are present. In 88% the result of the ultra-sound investigation proved as correct. False positive findings were provoked above all by the coarse-nodular liver cirrhosis. Despite certain restrictions the ultra-sound investigation of the liver is an important diagnostic remedy to find liver metastases, since it is painless, simple and harmless.

Aneurysm↗

[Urinary hydroxyproline excretion in liver cirrhosis].

In 50 patients with a laparoscopically ascertained liver cirrhosis the hydroxyproline excretion was estimated in the 24-hour-urine. There could not be found any significant differences between hydroxyprolinuria in normal persons and in patients with liver cirrhosis. The increased hydroxyproline excretion is an unspecific symptom and thus cannot contribute to the diagnosis and the observation of the course of a liver cirrhosis.

Adult↗

[The Saint syndrome].

At the instance of a 78-year-old female patient the possible causes of the simultaneous appearance of gallstones, hiatal hernia and diverticulum of the colon are discussed. The syndrome might be the sequel of a weakness of the connective tissue. But it would be also thinkable that it is an accidental combination of diseases, the number of which increases at growing age.
