[The site of recurrent ulcer following stomach surgery (Billroth I and II)].

In 69 patients with recidivation ulcer after an operation of the stomach (51 times Billroth II and 18 times Billroth I) was investigated whether or not there are differences in the localisation of an ulcer. In patients operated on according to Billroth II ulcers of the anastomoses are dominating, whereas in patients operated on according to Billroth I the majority of ulcers is to be found in the residual stomach. The causes for the deviating distribution pattern are not unequivocal. Changed diagnostics (in the first place endoscopy), different effect of the bile reflux, differing colonisation with Campylobacter pylori and differences in the size of the resection are discussed. The free interval between operation and recidivation of ulcer is distinctly shorter in Billroth I patients.

[Rate of recurrence of duodenal ulcer in smokers and nonsmokers].

In 70 patients (35 non-smokers and smokers each) with an endoscopically established duodenal ulcer the number of recidivations was investigated. In this case in the group of smokers an inclination to recidivations was found which was nearly double the size. Patients with a great consumption of nicotine per day were particularly affected. Apart from this the question was investigated whether the form of the ulcer has any influence on the frequency of recidivations. No remarkable differences between the forms of ulcer round, irregular, oval and linear could be proved. The economic disadvantages by inability to work considerably differ in the two groups. The number of days at which was not worked on account of putting the patients on the sick-list is nearly thrice as great in the group of smokers.

[Therapy of diarrhea in diabetic enteropathy with clonidine].

The syndrome of neuropathy belongs to the frequent complications of diabetes. Within a visceral neuropathy a diabetic enteropathy may appear, in which cases diarrhoeas and incontinence stand to the fore. The therapy of the symptoms and signs is often difficult. It is reported on the influence by means of the antihypertensive drug clonidin (Haemiton). In 4 out of 9 patients a regression of the frequency of motion from on an average 8 to 2 a day took place. 2 other patients examined reported on an improvement. Also the nocturnal incontinence could positively be influenced. The antidiarrhoeic effect of clonidin is perhaps based on a direct stimulation of alpha 2-receptors.

[Spontaneous perforation of the esophagus (Boerhaave syndrome)].

It is reported on 2 patients with spontaneous perforation of the oesophagus (Boerhaave's syndrome). The picture of the disease is rare. Possible causes, symptomatology, diagnosis, differential diagnosis and therapy are described. Severe thoracic pains, which in most cases appear after vomiting, are in the foreground. Evident for the disease is the escape of the contrast medium in esophagography. The lethality is 30%. Prerequisite for better results of the treatment is a rapid diagnosis and early surgical approach.

[Contribution to retinopathy in acute pancreatitis].

It is reported on 2 cases of pancreatitic retinopathy in acute pancreatitis. In the first place of the clinical symptoms there are acutely beginning bilateral vision disturbances 2 to 3 days after the onset of the disease which exist for a long time. Causally an obstruction of the precapillary and capillary parts of the vascular system of the retina, but also of the choriocapillary system by fat particles and corpuscular blood elements is discussed. With certainty still other factors play a part, for example preinjury of the vascular parts mentioned by circulating pancreatic enzymes and toxins. In patients with acute pancreatitis vision disturbances should be taken into consideration, even when these changes are very rare.

[Effect of nicotine on duodenal ulcer].

In 30 non-smokers and smokers each with and endoscopically ascertained duodenal ulcer the healing of the ulcer, the rate of recidivations, the clinical symptomatology and the duration of the unability to work were examined. In these cases in the group of patients with use of nicotine a delayed healing, a higher rate of recidivations, a greater clinical symptomatology and an essentially longer unability to work in contrast to non-smokers could be proved. Therefore it is advisable to stop smoking during the treatment of ulcer. It would be desirable that patients with a diagnosis of ulcer fully do without nicotine. The economic damage by appearing cost of treatment and prolonged unability to work is considerable.

[Body weight behavior after cholecystectomy].

In 47 patients with cholecystectomy the behaviour of the weight after the surgical intervention was investigated. In this case in 32 patients an increase of the weight could be established. 39 patients had additionally an increase of cholesterol. It is not acceptable that changes in the bile acid metabolism or fatty metabolism are a cause for the increase of weight. Probably, a change of the way of life with increase of the supply of fat is responsible for this.

[Clinical contribution to enteropathy in diabetes mellitus].

In 50 patients aged between 19 and 78 years was searched for symptoms of a possible enteral neuropathy. 18 patients, 11 of them in need of insulin, spoke intermittently of diarrhoeas, which partly changed with periods of constipation. Altogether 21 patients had a reduced tonus of the anal sphincter in the rectal digital examination, 9 patients were incontinent. The syndrome of neuropathy belongs to the most frequent complications of diabetes mellitus and shows a diverse picture of a disease. The enteropathy in diabetes mellitus in perhaps caused by disturbances of motility. By an aimed establishment of the history and exact performance of the rectal digital examination reference to a diabetic enteropathy can be proved.

[Secondary findings in gastroscopy detected stomach ulcer].

In 478 patients with gastric ulcer 121 times (25.3%) secondary pathological findings were found in the oesophagus, in the stomach and the duodenum. Erosions, scars and polyps were most frequently to be seen. But concomitant pathological findings with high clinical importance, such as carcinoma of the stomach and the oesophagus as well as varicose veins of the oesophagus, could also be established. The demand is of importance to search intensively for further lesions also after diagnosis of the ulcer. From this resulted conclusions, such as operation in carcinoma, continuation of the diagnostics in varicose veins of the oesophagus or control in polyps.

[Monthly and seasonal distribution of gastric and duodenal ulcer in an endoscopy study group].

The question of a monthly and seasonal dependence of the gastroduodenal ulcers was attended to. The gastroscopic reports of a period of 5 years were evaluated. Here, 2,497 florid ulcers were found, 1,114 in the stomach (= 10.4%) and 1,383 in the duodenum (= 13.2%). Neither monthly nor seasonally an accumulation could be established. On the contrary a nearly regular distribution was found, so that in accordance with other authors a seasonal dependence of the ulcerous disease cannot be derived.

[Gastric stump cancer].

In 10.500 gastroscopies 704 resection stomachs due to benign diseases of the stomach (ventricular ulcer and duodenal ulcer) were found. A carcinoma of the gastric stump could be found 97 times, 58 times of them as a sequel of an operated ventricular ulcer and 39 times as sequel of an operated duodenal ulcer. 14.2% stump carcinomas could be proved after Billroth-II-resection and 10.1% after Billroth-I-technique. The free interval was on average 23.1 years. An increased risk of cancer in the operated stomach is to be assumed. Regular endoscopic controls by biopsies are necessary.

[Effectiveness of metoclopramide in stomach ulcer].

The effect of metoclopramide and placebo was tested in 50 patients with an endoscopically and bioptically ascertained ventricular ulcer. In this case was started from the consideration that the motility of the stomach might play a role in the pathophysiology of the origination of ulcer. Indeed, metoclopramide is superior to placebo, however, significant effects cannot be found. The preparation has a favourable effect on dyspeptic complaints in the ulcerous disease and should be used here. Metoclopramide is no substitute for effective therapeutics in ulcer. A general use in the therapy of ulcer cannot be recommended.

[Diabetes mellitus and exocrine pancreas function].

In 60 patients with diabetes mellitus type II needing insulin for the control of the exocrine pancreas function a secretin-pancreomycin test was performed, which had a pathological result in 26.6% of the patients, in which case the functional disturbance achieved only a small size. In one part of these patients concomitant diseases of diabetes mellitus, such as cholelithiasis and hyperlipidaemia could be found. A connection between the duration of the diabetes and disturbed function of the pancreas could not be established. Corresponding with other authors the relations between incretory and excretory organ should not be overestimated.

[Is Monroe's point the site of choice for the establishment of pneumoperitoneum in laparoscopy?].

On the occasion of a case of death during performance of pneumoperitoneum before peritoneoscopy the presence of adhesions in the left lower quadrant of the abdomen in 850 peritoneoscopied patients was investigated. In 91,55% of the evaluated results the left lower quadrant of the abdomen was free from adhesions. Thus, the Monroe-point is even further on the place of choice in order to make the pneumoperitoneum. The Verres-needles are preferable to all other types of needles.

[Esophageal diseases in the differential diagnosis of patients with angina pectoris symptoms].

It is reported on 7 patients who on account of angina pectoris symptoms came to the consulting hour. A heart disease could be excluded. Since the complaints persisted the diagnostics was continued. Twice were found a carcinoma of the oesophagus, three times an oesophagitis and once an ulcer in the oesophagus and once a hiatal hernia. In unclear thoracic pain diseases of the oesophagus should always be included into the differential diagnosis. An adequate diagnostics must be performed.

[Hyperplasia of Brunner's glands].

3 observations of hyperplasias of Brunner's glands are described. Such proliferations are not too frequent. The diagnostics is performed by means of radiology and endoscopy. The clinical symptomatology is multivarious, however, non-characteristic. Intestinal haemorrhages are possible. The therapy can be performed surgically and endoscopically.

[Stomach carcinoma in young patients. Presentation of a case report].

At the instance of a casuistic description in a 24-year-old female patient is reported on the problems concerning the carcinoma of the stomach in patients younger than 30 years. At this age carcinomas are very infrequent and, moreover, they are diagnosed too late on account of the juvenile age. Therefore the prognosis is extremely bad. The demand is of importance to think of a carcinoma of the stomach also in young patients with unclarified epigastric complaints and to strive for an endoscopic and bioptic clarification as quickly as possible.
