Hospitalization and case-fatality rates for stroke in Canada from 1982 through 1991. The Canadian Collaborative Study Group of Stroke Hospitalizations.

BACKGROUND AND PURPOSE: The purpose of this study was to estimate rates of hospitalization and in-hospital case-fatality for cerebral infarction and intracerebral hemorrhage in Canada and to describe variation in rates by age, sex, and calendar period. METHODS: Data were obtained from hospitalization databases for each of Canada's 10 provinces for the 10 fiscal years of 1982 through 1991. All hospitalizations of persons 15 years of age or older with a primary diagnosis at discharge coded 431, 434, or 436 according to the International Classification of Disease, 9th Revision, were included. Rates per 100,000 population were calculated for intracerebral hemorrhage and cerebral infarction, for men and women, and for five age groups. Annual age- and sex-specific, 30-day, in-hospital case-fatality rates were also calculated. RESULTS: A total of 335,283 discharges for stroke were enumerated over the 10-year period (309,631 cerebral infarctions and 25 652 intracerebral hemorrhages). A significant decline of approximately 1% per year was observed for the rate of cerebral infarctions. For hemorrhages, the reverse was seen. For men there was a 44% increase over the 10-year period, and for women there was a 34% increase. In-hospital case-fatality rates for cerebral infarctions increased with age but did not differ by sex when age was considered. For the five age groups of 15 to 54, 55 to 64, 65 to 74, 75 to 84 and > or = 85 years, rates were 6%, 8%, 12%, 18% and 27%, respectively. For intracerebral hemorrhage, the in-hospital case-fatality rates declined significantly over time from approximately 36% to 29%, 55% to 37%, 49% to 41%, 66% to 45%, and 72% to 59% for the five age groups, respectively. CONCLUSIONS: The possibility that these changes are artifactual could not be ruled out, but because there is no obvious risk in assuming that they are not, it would be prudent to investigate their causes further.

Digital image display station performance requirements based on physician experience with a prototype system.

The authors report on observations of and interviews with physicians using a prototype digital image display and reporting station. While the users generally agree that image quality is clinically satisfactory, they are unanimous in their opinion that improvements in the man-machine interface are required before case review by this mechanism is clinically acceptable in a production environment. A model image and information user interface is presented. It was developed in answer to the needs of radiologists and referring physicians operating in the imaging department of a community acute-care facility. In such an environment images and related information must be communicated quickly and often simultaneously to different parts of the department and hospital. The user interface with the management system and the management system itself must address the varied functions and the needs of both the medical and clerical staff. Image enhancement processes, for example, must be restricted to those that quickly provide significantly more perceivable diagnostic information. Little-used processes that may occupy significant portions of the display and the console's computing power must be trimmed or eliminated.

Influence of estrogen receptor status on dietary risk factors for breast cancer.

It has been suggested that the relation between diet and breast cancer may depend on estrogen receptor (ER) status. We examined the responses to a self-administered questionnaire on frequency of consumption of various foods by 493 women with breast cancer (160 with ER-negative tumours and 333 with ER-positive tumours) and 527 controls whose menopausal status was known. Analysis of the reported consumption of foods selected for their fat or carotene content showed no clear distinction in dietary factors between the ER-negative and ER-positive groups. Frequent consumption of meat fats generally increased the risk of both ER-negative and ER-positive tumours; there were no clear trends in risk associated with vegetable consumption for either ER group. Fish was the only item affecting the risk for ER-negative and ER-positive tumours differently, frequent consumption reducing the risk for the former (p = 0.02). The results do not support the hypothesis that ER status influences the relation between dietary fat consumption and risk of breast cancer.

Childhood and recent eating patterns and risk of breast cancer.

A case-control study was done to examine the relationship between childhood and recent eating practices and risk of breast cancer. Eight hundred forty-six cases and 862 controls returned questionnaires indicating their menopausal status. In premenopausal women, breast cancer risk was increased with recent consumption of foods high in fat content (gravy, beef, pork) and reduced with foods low in fat content (fish); in postmenopausal women, risk was increased with pork consumption only. Regarding carotene sources, risk was reduced with carrot consumption in postmenopausal women only. Similar trends in risk were not found for childhood eating practices. Body weight influenced the breast cancer risk differently for pre- and postmenopausal women: Heavier weight in childhood and teens reduced the risk of premenopausal breast cancer, and heavier weight in adulthood increased the risk of postmenopausal breast cancer. We conclude that fat consumption is associated with breast cancer, especially in premenopausal women.

[Chemical analysis of methacrylate plastics].

It was the object of this study to find quick but exact methods for determining the components of quickly setting methacrylate materials for dentures, dental fillings (PMMA) and for bone surgery. Furthermore, lower-molecular components still being present in the hardened resin were tested. By measuring refraction index and by infrared spectrum and gas-chromatographical analyses the composition of the liquid is obtained. Dimethyl-p-toluidine, hydrochinon and cross-linking agents were determined spectrophotometrically. Filling agents, pigments, plasticizers and soluble components of the powder were determined gravimetrically. The presence of polystyrol is established by infrared analysis, that of benzoyl peroxide by spectroscopic analysis. As to the hardened material, the molecular weight is determined by viscosity measurements and the molecular weight distribution by gel permeability measurements. Residual monomer is found by gas chromatography.

JavaScope: A Web-based TEM control interface.

JavaScope is a Web-based java applet that implements an "exploration/browser" tool for operating a Philips CM200 transmission electron microscope and viewing digital images remotely. The primary use of the application is as a collaborative tool for remote consultations.
