[Floctafenine in the treatment of painful syndromes of neurologic interest].

The clinical results obtained with phloctaphenin in an "open" study comprising cases of real neuralgia and spinal radicular pain syndromes and in a "double-blind" study comparing the preparation with a placebo, and involving algico-vasomotor syndromes of the face, are reported. The results obtained confirm the antalgic power of phloctaphenin for by the third day, subacute or chronic pain syndromes already presented a significant improvement in simptomatology, while in algico-vasomotor syndromes of the face, the treatment proved effective in eliminating pain attacks.

Comparative Doppler directional sonographic and ophthalmodynamographic study in the occlusion of the internal carotid artery.

In the occlusion of the internal carotid artery, the change occurring in the internal-external carotid pressure gradient may cause flow reversal within the ophthalmic artery and its branches. Ophthalmodynamographic, Doppler directional sonographic and angiographic investigations were performed in 21 cases (11 total occlusions, 6 severe and 4 moderate stenoses of the internal carotid artery). The authors compare the results of these investigations and discuss the practical importance of Doppler directional sonography and the significance of flow reversal in the ophthalmic artery.

Insulin secretion in migraine: influence on the blood levels of tryptophan.

The blood levels of free tryptophan, glucose and insulin were determined in different groups of patients suffering from headache, initially and after a glucose tolerance and a tolbutamide tolerance test. The control group consisted of subjects without neurological or psychiatric disturbance. A sharp drop of blood tryptophan levels was observed in migraine patients during the tolbutamide tolerance. Migraine attacks may be induced in certain subjects by a disturbance of 5-hydroxytryptamine turnover.

Long-term idebenone treatment of vascular and degenerative brain disorders of the elderly.

A multicentre study with an open experimental design was carried out on 118 patients suffering from mild to moderate cognitive decline due to cerebrovascular and degenerative disorders (chronic cerebrovascular disorders, CCVD; multi-infarct dementia, MID; aging brain, AB; dementia of Alzheimer's type, DAT). All patients, after a wash-out period of 3 weeks, were treated with idebenone (45 mg twice daily by oral route) for a period of 6 months. Behavioral and cognitive measures (Sandoz Clinical Assessment of Geriatrics, SCAG; Serial Learning Test) were applied to evaluate the long term therapeutical aspects. The results, analyzed by multivariate analysis of variance and chi2 test, showed a significant improvement of the cognitive profile in all patients, more evident in CCVD and AB groups. No remarkable side-effects were found in all groups of patients, thus confirming good tolerability of idebenone.

Pramiracetam effects on scopolamine-induced amnesia in healthy volunteers.

Pramiracetam has been evaluated for its potential antiamnesic properties in scopolamine-induced amnesia in healthy volunteers. Two groups of twelve males, 18-42 and 55-65 years old, respectively, were randomly assigned to oral treatment with pramiracetam (600 mg twice a day) or with placebo for 10 consecutive days. On day 11 each subject was injected intramuscularly with scopolamine hydrobromide (0.5 mg). Before scopolamine injection and then 1, 3 and 6 h after it, subjects were administered the following psychometric tests: simple and choice visual reaction times, digit symbol substitution test, Rey's 15 words test for short and long term verbal memory. Scopolamine significantly impaired episodic memory and selective attention tests in both scopolamine and placebo groups. Instead visuo-motor and incidental learning measures were unaffected. Pramiracetam, when compared to placebo, was able to partially reduce the amnesic effects induced by scopolamine both in young and old subjects.

Assessment of inter-observer differences in the Italian multicenter study on reversible cerebral ischemia.

A standardized protocol was used for duplicate examination by two neurologists of the clinical history and neurological signs in 55 patients with reversible cerebral ischemia. One trained examiner from each of eight clinical centres involved in the Italian Cooperative Study on reversible cerebral ischemia participated in this research. Duplicate examination were compared in order to evaluate the percentage of agreement achieved in the responses. Several discrepancies relating to either historical data or neurological signs were detected. Possible causes of this disagreement are discussed. The inter-observer differences appear to be an important problem to be faced in cooperative studies on reversible ischemic attacks. The purpose of the study was to estimate these differences in the attempt to increase the degree of agreement by repeated training sessions and discussion between the different examiners. These efforts are needed to improve the quality of clinical investigations on reversible cerebral ischemia and to increase the validity of their results.
