Lithium carbonate in cluster headache: assessment of its short- and long-term therapeutic efficacy.

The short- and long-term effects of administration of lithium carbonate in cluster headache (CH) have been investigated. Of the 90 patients treated (78 males and 12 females), 68 had episodic CH and 22 had the chronic form of the disease. The doses used were almost always 900 mg/day. Eleven of the 22 patients with chronic CH showed a definite, constant improvement both short and long term. In 7 of the 22 patients, lithium treatment provided excellent results initially but was later followed by some transient worsening; in the remaining 4 only partial benefits were observed initially and treatment proved still less effective after a few months. The effects of cessation of lithium administration after at least five months of continuous treatment were studied in 9 cases. In 6 of them the attacks re-appeared immediately, whereas in 3 the attacks occurred again only after free intervals of four to six months. Of the 68 patients with episodic CH, 26 proved highly responsive to treatment, 26 only partially responsive, and 16 refractory. In 3 cases, after one to three years of continuous treatment, euthyroid goitre developed, which disappeared after the drug was discontinued.

Progesterone, estrogen, LH, FSH and PRL concentrations in plasma during the estrous cycle in goat.

Progesterone, estrogen, LH, FSH and PRL variations in plasma were measured in 7 goats by RIA. Mean progesterone level was 0.0-0.8 ng/ml in estrus and 1-4 ng/ml in diestrus. Estrogen levels ranged from 10 to 20 pg/ml in diestrus; around estrus, a peak of 26.9 +/- 3.18 pg/ml was observed. LH values were 0.5-3.0 ng/ml with a preovulatory peak (40.7 +/- 10.12 ng/ml) 8 to 24 h after the onset of estrus. Except for a peak (14.0 +/- 3.09 ng/ml) coincident with LH surge, plasma FSH level ranged between 2 and 4 ng/ml. PRL level averaged 2-5 ng/ml in diestrus but high concentrations of this hormone were seen around estrus.

Italian study of cerebral reversible ischemic attacks. II. The hematocrit and hemoglobin distribution and its relationship between clinical pattern and angiographic abnormalities.

A clinical and angiographic study was conducted in 360 patients with previous reversible ischemic attacks (RIAs) in order to verify the influence of hematocrit, hemoglobin and red cell count within the normal range in the clinical picture of cerebral infarction and its relationship with angiographic atherosclerosis. No significantly different distribution of the three parameters considered has been found according to the presence and the degree of atherosclerosis. On the contrary, our data have shown a significant correlation between levels of hematocrit, hemoglobin and red cell count and some clinical aspects of the RIA when angiographic atherosclerosis is present.

Headache temporal patterns: towards a chronobiological model.

Rhythmical changes in terrain susceptibility and trigger occurrence can induce periodic temporal patterns in headache patients, with readily foreseeable frequency cycles of attacks. The impaired ability of biological functions to adjust to cyclic environmental changes represents the minus, the handicap, the increased risk of clinical symptomatology (headache); attacks occur when the precarious systems of these patients are forced by cyclical environmental stimuli towards phase shiftings or to cope with new levels of performance. A dyschronic hypothesis of primary headache must be based upon observations demonstrating that along with dysfunctions of the pain control systems there is also a vulnerability of the rhythmic physiological organization of CNS.

Lithium salts in cluster headache treatment.

The short and long-term effects of Lithium carbonate in 90 patients with episodic (68 cases) and chronic (22 cases) cluster headache are reported. The results are discussed in particular in relation to the response of the episodic forms, the development of tolerance, the ability of the substance to modify the natural history of the disease, and the side-effects from long-term treatment.

Ergot derivatives in the prophylaxis of migraine: a multicentric study with a timed-release dihydroergotamine formulation.

In this multicentric random double-blind study versus placebo, the therapeutic activity of timed-release dihydroergotamine preparation 10 mg/per os/day has been tested in 90 patients affected by common migraine. The drug presented a prophylactic effect on recurrent headache, mainly on night and awakening migraine. A low incidence of side-effects with no weight changes or other effects on physiologic events was registered.

Neuroendocrine data in chronic post-traumatic headache.

Twenty chronic post-traumatic headache subjects (16 men and 4 women) were considered, together with age and sex matched normal controls. Pituitary function was challenged by means of luteinising hormone releasing hormone (100 microg, i.v.), thyrotropin (TSH) releasing hormone (200 microg, i.v.) and regular insulin (.1 U/kg, i.v.). The responses of gonadotropins, TSH, somatotropin (STH) and prolactin (PRL) were studied in the group as a whole and in clinical subgroups. No differences were found in the baseline levels of the hormones. No significant changes were observed in the responses of gonadotropins and STH. A decreased response was found for TSH and, in the female series, an enhanced response of PRL was observed. Patients complaining for a decreased libido showed a larger PRL response. The smaller response of TSH exhibits a straight inverse relationship to the clinical severity. The periodicity of PRL secretion studied in male patients with a harmonic analysis according to the Fourier's theorem showed the preservation of the physiological sleep-entrained night surge. The per cent incidence of the harmonics with a period shorter than 24 hours decreases according to the clinical severity, at variance to the finding in normal controls. The only persisting harmonic is the one with a 24 hour period. In the absence of major post-traumatic endocrine syndromes, post-traumatic headache subjects show changes of the neuroendocrine regulation of pituitary secretion. The findings are consistent with the hypothesis of an hyperactivity of serotoninergic neurotransmission.

Concentrations of progesterone, 17 alpha-hydroxyprogesterone and 20 alpha-dihydroprogesterone in the plasma of mares during pregnancy and at parturition.

Plasma concentrations of progesterone and 17 alpha-hydroxyprogesterone were high in the 2nd and 3rd months of gestation, but 20 alpha-dihydroprogesterone increased from a level of 2 ng/ml, during the first 3 months, to 10-15 ng/ml during months 5-10, to reach 80-120 ng/ml during the last 30 days before foaling.

Prostaglandin F-2 alpha and LH release in immature ewes.

The effect of PGF-2 alpha on LH release in immature ewes was studied. One intramuscular injeciton of PGF-2 alpha induced LH release 36-72 h after treatment in 3/4 ewes. The LH variations were not associated with changes in the plasma concentration of progesterone and oestrogens. These results suggest that the LH release in immature ewes is due to PGF-2 alpha acting at central structures.
