[Antibody deficiency without decrease in the level of serum immunoglobulins].

The paper reports the case of a 14 year-old boy, born at full term with intra-uterine growth retardation (1,640 g) who presented with short stature, generalized eczema and recurrent infections. During the first years of life, hypogammaglobulinemia with antibody deficiency occurred. The in vitro T lymphocyte function was normal. The infections have become less severe. The plasma levels of IgA, IgG, IgM and IgE are normal. There is hyperimmunoglobulinemia D. The deficiency of antibodies against most of the tested antigens persists. T and B lymphocyte counts are normal. The in vitro lymphocyte proliferation with Concanavalin A, Phytohemagglutinin, Pokeweed mitogen and Nocardia is normal. The OKT4+ and OKT8+ cell counts are normal. The ratio "helper T cell/suppressive T cell activity" appears to be abnormal. A very prolonged maturation delay, possibly associated with fetal hypotrophy may be hypothesized.

Elastase-type activity of human serum. Its variation in chronic obstructive lung diseases and atherosclerosis.

Human serum was found to contain enzyme activities hydrolyzing succinyl trialanine paranitroanilide and 3H-kappa-elastin Sepharose substrates. Both types of activities could be partly abolished by serine active site titrants (phenylmethanesulfonylfluoride, diisopropylphosphorofluoridate) and partly by neutral chelating agents (EDTA; 1-10-phenanthroline). The combination of phenylmethanesulfonylfluoride and EDTA gave a complete inhibition of human serum elastase-type activities indicating the presence of at least two different types of elastases (serine and metalloproteases) in human serum. In nonsmokers, the average serum elastase-type activity on succinyl trialanine paranitroanilide was found equal to 78.1 ng/ml porcine pancreatic elastase equivalents and on 3H-kappa-elastin sepharose beads equal to 688.8 ng/ml. No statistically significant differences were observed in elastase levels in the sera of individuals presenting clinical symptoms of atherosclerosis. The sera of patients suffering from chronic obstructive lung diseases contained, however, higher amounts of elastase-type activities, respectively equal to 237.2 ng/ml on succinyl trialanine paranitroanilide and 1,096 ng/ml on 3H-kappa-elastin Sepharose beads and was quantitatively significant when compared with control subjects.

An evaluation by sequential extraction of the proportions of collagen types from medium sized arteries.

Samples of human Common Iliac Arteries, obtained post-mortem from patients without evidence of atherosclerosis, were analyzed for their content in the various types of collagens using alternatively sonication, pepsin digestion and dithiothreitol reduction followed by sodium dodecyl sulfate polyacrylamide gel electrophoresis of the extracts. A final average proportion of 66% of the total collagen was solubilized. The statistical calculation of the proportions of the different collagens, deduced from the densitometric scannings of the electrophoretic runs, showed that a larger amount of type I was extracted when the extraction was more complete. Type I collagen appeared as the most insoluble collagen in these arteries and its final proportion was probably higher than 60% of the total collagen.

[A simple method for detection of functional and genetics alpha-1-antitrypsin deficiencies of human serum (author's transl)].

Insoluble elastin immobilised in Agar plates was used as substrate to quantitate the elastase inhibitory capacity of human sera. This technique was found valuable to evaluate the genetic deficiency of alpha-1-antitrypsin (Z phenotypes) and also the functional loss of the elastase inhibitory capacity in smokers. A significant decrease of elastase inhibitory capacity was demonstrated in the sera of smokers between 40 and 80 years of age, as compared to non smokers of the same age. This method may be useful for the evaluation of patients suffering from elastic tissue diseases (emphysema, arteriosclerosis).

[Theophylline in childhood asthma. Pharmacokinetic and clinical study (author's transl)].

After an intra-venous injection of a single dose (4.24 +/- 0.94 mg/kg) of anhydrous theophylline in 20 children aged 4 to 16 years, the pharmacokinetic characteristics (mean and standard deviation) were as follow: half life (T 1/2 6.06 +/- 2.53; apparent distribution volume (Vd) 0.585 +/- 0.148 l/kg and clearance from the body (Cl) at 0.078 +/- 0.35 l/kg/w. The absorption of anhydrous theophylline syrup, given to 13 children during an exacerbation of their asthma, was consistent, quick and complete. Two hours after a dose of 6 mg/kg, the plasma levels were 11.39 +/- 2.04 mg/litre. On seven occasions, the clinical results were good. The absorption of the tablet form of anhydrous theophylline was more irregular. A prolonged course of treatment in 20 subjects aged 4.5 to 17.2 years confirmed the wide variety of doses required to maintain plasma theophylline levels in the 10 to 20 mg/litre range. The doses required varied from 10 to 22.5 mg/kg/day. The correlation between the actual and calculated needs on the basis of theophylline clearance was poor (due to the bioavailability of the drug and the close dependent character of the clearance). Seven times, the results were good (twice mean theophylline levels below 10 mg/litre). The proposition of good results in this study is less than that reported in the literature.

Isolation and partial characterization of an elastase-type enzyme from human arterial wall by lima-bean trypsin inhibitor affinity chromatography.

A serine protease active on insoluble elastin at neutral pH has been isolated from human aortic media employing a Lima-bean trypsin inhibitor - Sepharose column. It is also hydrolyzed Suc (Ala)3 pna and casein but was found inactive against Benzoyl-Tyr-pna and Benzoyl-Arg-pna. Its apparent molecular weight as determined by SDS-PAGE was 22,300 Daltons. It differs from other elastases of human origin (human pancreatic elastase-1, human pancreatic elastase-2, human leucocyte elastase) on the basis of amino-acid composition and immunological specificity.

[Chronic active hepatitis with a favorable course, after fulminant hepatitis].

The case reported here is that of a girl with presumably viral non A, non B, acute hepatitis with a very unusual course. Fulminant hepatitis with submassive and bridging hepatic necrosis and a 17-day coma began during the 7th week of evolution. Prolonged chronic active hepatitis followed. Treatment was initiated 6 months after the beginning of the affection and was maintained for more than 2 years, with an apparent cure persisting after follow-up period of 6 months; fibrous scars were the only abnormalities demonstrable on histologic examination of liver biopsy. It is possible that such type of fulminant hepatitis with unusual course will become more frequent, as survival of the initial acute episode increase.
