The contact site A glycoprotein, a cell adhesion protein of aggregating Dictyostelium cells, was labeled with fatty acid, myo-inositol, phosphate and ethanolamine in vivo, indicating that the protein is anchored in the membrane by a lipid. This lipid was not susceptible to phosphatidyl inositol specific phospholipase C. When cleaved with nitrous acid or when subjected to acetolysis, the anchor released lipids which were different from those released from Trypanosoma variant cell surface glycoprotein, a protein with a known phosphatidyl inositol-glycan anchor. Resistance to weak and sensitivity to strong alkali indicated that the fatty acid in the contact site A glycolipid anchor was in an amide bond. On incubation with sphingomyelinase, a lipid with the chromatographic behavior of ceramide was released. These results suggest that the contact site A glycoprotein is anchored by a ceramide based lipid glycan.
Of the population of a small Tyrolean village, 185 (56%) of the 329 inhabitants over 40 years were investigated by means of Doppler and duplex sonography, electroencephalography (EEG), electrocardiography (ECG), and neurological examination. Four subjects (2%) previously had a transitory ischemic attack (TIA) or stroke in the carotid territory. Sonographically detectable abnormalities in one or more extracranial arteries were present in 42 (23%) persons. Of the 14 subjects with more than slight abnormalities in the common or internal carotid artery two were symptomatic. Two additional cases with TIA or stroke did not show relevant lesions on sonographic examination. The presence of narrowing in the extracranial artery was not related to risk factors (hypertension, smoking, obesity) or abnormalities on ECG and EEG. This study shows 1) that the clinical relevance of ultrasound screening of the carotid arteries in an average population is 15%; 2) that significantly more patients with TIAs or strokes are found in the group with more severe sonographic findings (p = 0.001) than in the group with normal ultrasound results.
The various types of meniscal lesion including longitudinal, horizontal and transverse tears, bucket handle tears (displaced or non-displaced) as well as tears of the meniscus base can be visualized in differing sonographic views. Our results on 39 patients, whom we sonographed and then controlled operatively, show that the different meniscal lesions are not reliably differentiated by ultrasound. Depending on the view chosen, similar sonographic displays result for all of the lesions under discussion. Only a smooth reflecting plane in the otherwise homogeneous structure of the meniscus is proof of the existence of a meniscal lesion.
Calcaneal fractures are extremely rare in children with intraarticular fractures occurring between 8 and 14 years. This report refers to two cases with displaced intraarticular fractures of the calcaneus sustained from skiing. Results of surgery were proven to be excellent. As essential prerogatives of successful treatment elaborate diagnostics including computerized tomography, experience in doing surgery on the calcaneus and knowledge about the special entities of the growing skeleton are mandatory.
In a 33-year-old HIV-positive homosexual male suffering from unexplained headache, cryptococcosis was diagnosed in a progressive secondary stage. After treatment with the standard combination therapy of amphotericin B + flucytosine for 34 d, the patient was clinically symptom-free and discharged, upon his own request, from the hospital. He remained under ambulatory mycological control. After an interval of 65 d during which the urine had been free from Cryptococcus neoformans (Cr.n.), the fungus could not be isolated from urine but 3 X 10(5) CFUs/ml were found in the seminal fluid. Andrologically, teratospermia and hyposemia were present. There were no clinical signs in the genitourinary tract including the prostate. The significance of ecological niches for Cr.n. colonization of the genitourinary tract after antimycotic therapy is discussed. In such cases, in addition to cultural examination of urine for Cr.n. by the membrane filtration technique (MFT) and Staib agar, an additional cultural examination of seminal fluid is recommended. It is also proposed to pay more attention to Cr.n. in andrological examinations. Special regard should be given to a possible occurrence of Cr.n. in the seminal fluid of AIDS patients. In cytology of the seminal fluid, use of the Giemsa stain is unsuitable for the purpose of Cr.n. detection. For this reason, it should be supplemented by PAS staining.
The concept of discovery learning is exemplified in this account of four students and their instructor who began and developed a nurse-run clinic for the homeless in a community health project. The students were registered nurses returning for their bachelor's degrees. They experienced frustration at learning the difference between care based on their assessments of patients' needs and care geared to clients' assessments of desired interventions. The journals that they kept reveal self-discovery as well as new respect for other humans. In addition, a new type of community care emerged, which gives all indications of surviving.
Research work using easily resorbable HA- und TCP-ceramics does not give any proof for pathologic deposition in different organs. Based on current information, there is no objection against the clinical use of HA-ceramics.

The most frequent pathologic findings causing death within the autopsies performed for the Health Authorities in Vienna in the years 1959, 1976 and 1987 are shown; the percentages of the findings in 1987, taken collectively, are compared to those seen in corpses found on the W.C. The evaluation reveals an increase of acute opposite to chronic heart diseases; furthermore that - concerning deaths on the toilet-the acute findings in general are clearly dominating.
Using radiolabeled specific cDNA glutamine synthetase mRNA could be detected by in situ hybridization exclusively within those few perivenous hepatocytes which stained immunocytochemically for glutamine synthetase. This localization of glutamine synthetase mRNA was recently reported by Moorman et al. [(1988) J. Histochem. Cytochem. 36, 751-755]. Biotinylated cDNA was not suitable for mRNA detection because of a very high background staining under the conditions of in situ hybridization. Dot blot and Northern blot analysis of RNA isolated from periportal and perivenous subfractions of hepatocytes also demonstrated the exclusive perivenous localization of two hybridizable glutamine synthetase mRNAs of length 2.8 and 1.6 kilobases. These results indicate that the unique heterogeneity of glutamine synthetase in rat liver parenchyma is controlled at the pretranslational level.
Austria is also becoming increasingly aware of the problems involved in the giving of information to the patient by the doctor. In individual cases, the patient seeks to enforce claims on the basis of having been inadequately informed. The principle that the extent of the information must, in the first place, take into consideration the patient's wellbeing and only in second place the patient's right to self-determination is generally valid, in particular with respect to ad hoc decisions taken during an operation. Notwithstanding, especially during operations conflict situations arise whenever the duty to give medical aid in the genuine interest of the patient and the patient's right to self-determination have to be weighed up against each other. General medico-legal principles and relevant jurisdiction with provide support for the doctor in these difficult decisions.

At the Vienna Institute, in the year 1986, 79 carcinoma findings were recorded in 1736 cases of natural death. The most common were carcinomas of the respiratory and gastrointestinal tracts. Seventeen unidentified tumors were found to have been the cause of death. Public health autopsies, therefore, contribute to the clarification of tumor disease and thus to cancer statistics. Although diagnosed fatal cancer cases correspond to only 0.33% of all cancer deaths in Vienna, the question arises why a patient with very advanced cancer is not under medical care and why the affliction is revealed only in autopsy, the purpose of which is to elucidate sudden, unexpected deaths. The comparatively few individual cases should nevertheless give cause for reflection and are discussed from a medicosociological view, taking into consideration the statistical and epidemiological data relating to cancer diseases.
We describe a case of Schilder disease in a young woman pointing out the most reliable diagnostic criteria for differentiating the disease from adrenoleukodystrophy. We stress the diagnostic usefulness of ultrastructural analysis, even of poorly preserved specimens.
Real-time scanning of the meniscus with different sector and linear ultrasound heads were tested. Visualisation of all meniscal components as well as reliable differentiation from surrounding structures was only possible with a 7.5 MHz head in the sector scanning mode. Both, the medial and lateral meniscus, were shown as a homogeneous, grey, wedge-shaped structure between the femoral condylus and the tibial plateau on the longitudinal ultrasound scan. A standardised method of examination is described.
