Tamoxifen treatment in oligozoospermia.

This study of the effects of long-term tamoxifen administration on semen analysis of oligospermic males confirms the potential therapeutic efficacy in normogonadotrophic oligospermia. 38 out of the 56 patients responded well to long-term treatment with 30 mg tamoxifen daily. According to the nomenclature of Eliasson, 32 patients reached normal sperm density and 16 patients normal sperm motility after tamoxifen treatment. In the group of responders a pregnancy rate of 34% is obtained. As far as the endocrinological parameters are concerned normogonadotrophic patients (responders and non-responders) showed an increase in testosterone, 17beta-estradiol, LH and FSH levels, whereas the levels of prolactin and testosterone/estradiol-binding globulin remained unchanged. No alterations at all were seen with regard to semen volume, during the time of tamoxifen treatment.

The effect of antiestrogen, antiandrogen, and the prolactin inhibitor 2 bromo-alpha-ergocriptine on the stromal tissue of human benign prostatic hyperplasia. Correlation of stereological data and plasma hormones.

In an attempt to shed light on the action of endogenous androgens and estrogens on the stromal tissue of the human prostate, we studied the effect of selective androgen (cyproterone acetate), estrogen (tamoxifen), and prolactin (bromocriptine) antagonists. The application of cyproterone acetate led to a reduced relative volumetric amount of the smooth muscle cell organelles, whereas the administration of tamoxifen effected no alteration as compared to pretreatment values. The administration of bromocriptine led to further activation of the smooth muscle cells in benign prostatic hyperplasia. Hormones, such as leutinizing hormone, follicle stimulating hormone, testosterone, 17-beta-estradiol, and prolactin were measured to assess alterations of the pituitary gonadal axis.

Orgotein, a new drug for the treatment of Peyronie's disease.

Twenty-three patients with Peyronie's disease have been treated with the new anti-inflammatory metalloprotein drug orgotein, which exhibits pronounced superoxide dismutase activity. Administration was done into the indurated areas of the penis by a special syringe. Evaluation of signs and symptoms included measurement of the induration size and consistency as well as the degree of the deviation of the penis. The patients responded well to orgotein therapy, especially regarding the loss of pain on erection. On long term results also a diminishment of the induration size and penis deviation on erection was observed; the drug administered intraplaqueally was of outstanding safety.

Influence of exposure to moderate altitude on the plasma concentraton of cortisol, aldosterone, renin, testosterone, and gonadotropins.

The influence of 11 days at moderate altitude (2,000 m) combined with exercise on plasma concentration of testosterone, FSH (follicle-stimulating hormone), LH (luteinizing hormone), cortisol, aldosterone, and renin activity was studied in ten healthy subjects. Within 48 h of arrival at moderate altitude a significant increase in testosterone was found whereas FSH had decreased significantly and LH showed a tendency to decrease. Cortisol increased significantly at the beginning and reached a maximum at the end of altitude exposure. The plasma aldosterone level rose continuously and on the last day of altitude was significantly elevated. Plasma renin activity showed a tendency to decrease. On return to low land all measured parameters returned to base line values within 2 days. The findings of increases in plasma levels of aldosterone and testosterone (and serum T3 and T4, as reported by others) are in contrast to the previously found decrease of urinary excretion of all these hormones. This appears to be a distinct dissociation of serum levels of adrenal (and thyroid) hormones from their urinary excretion. The observed increase in plasma aldosterone is probably mediated through ACTH and the rise in plasma potassium, since plasma renin activity showed an opposite trend. The rise in plasma testosterone is probably of adrenal origin since plasma gonadotropins declined simultaneously. The increase of plasma levels of glucocorticoids, mineralocorticoids, and androgens after an ascent from 600 m to 2,000 m above sea level is compatible with an ACTH-mediated stimulation of the entire adrenal cortex and/or a diminished elimination of adrenal steroids: The concomitant fall of FSH, LH, and plasma renin would then be a consequence of a direct negative feedback inhibition of these hormones.

Human benign prostatic hyperplasia: a stromal disease? New perspectives by quantitative morphology.

Stereology is of great value for better understanding the pathomorphogenesis of benign prostatic hyperplasia (BPH) and the hormonally controlled mechanism of abnormal overgrowth of the prostate gland, especially the stromal component. In human BPH a fourfold absolute increase of the stromal part and a nearly double increase of the glandular part is indicated. The relative and absolute increase of the stromal part is surprisingly constant in spite of variation of human BPH. Stromal overgrowth can be viewed as a central feature of BPH implicating altered mesenchymal-epithelial relationship. At the ultrastructural level in man and dog the volume density of the secretory droplets and lysosomes of the glandular cells is reduced. Stereology data confirm furthermore the activation of the smooth muscle cells suggesting an induction of stromal growth possibly induced by estrogen and/or androgen and an important role in glandular-stromal interactions.

Successful renal revascularization of unilateral traumatic renal artery thrombosis.

We report 3 patients with traumatic renal artery thrombosis treated successfully with revascularization. The diagnosis was made preoperatively in 2 cases and intraoperatively in 1. Treatment consisted of reanastomosis with a saphenous vein patch on the side of the arteriotomy in case 1, an aortorenal artery saphenous vein bypass in case 2 and a bypass graft from the aorta to the distal renal artery in case 3. Two patients had functioning kidneys and normal blood pressure, and 1 patient dies 24 hours after reconstruction.

Testicular torsion: late results with special regard to fertility and endocrine function.

Late results were determined for 42 patients who had undergone detorsion and fixation for unilateral testicular torsion in the prepubertal and pubertal age. Exocrine and endocrine function for the testes was determined in 30 patients who had reached postpuberal age. Patients who underwent detorsion and fixation 8 hours or less after the onset of symptoms had normal-sized testicles and only slight changes in testicular morphology. When treatment was delayed and detorsion was done more than 8 hours later a marked decrease was observed in testicular size. The exocrine function in patients with torsion was reduced. The semen quality, as judged by 2 semen analyses, was normal in 15 patients, doubtful in 3 and pathological in 12. Even when detorsion was done 4 hours or less after the onset of symptoms the exocrine function of the testes was normal in only 50 per cent of the cases. In patients with doubtful and pathological sperm analyses higher follicle-stimulating and luteinizing hormone levels were observed.

The dog prostate under defined hormonal influences: an approach to experimental induced prostatic growth.

The prostate of castrated dogs following treatment with testosterone, dihydrotestosterone and androstanediol combined with 17-beta-estradiol application was analyzed by light microscopic stereology. In all three groups there is a statistically significant increase of prostatic volume and weight. There is a statistically significant increase of the relative and absolute volume of the glandular as well as of the stromal part. Seen by the absolute data there is only a statistically significant increase of the volumetric amount of the smooth muscle cells in the dihydrotestosterone and 17-beta-estradiol treated animals. Regarding tissue distribution there is no difference between the controls and the castrated testosterone and 17-beta-estradiol treated animals. Regarding the stromal part, there is no statistically significant difference between the testosterone and 17-beta-estradiol treated group and the control animals. TIn the androstanediol and 17-beta-estradiol group there is a double and in the dihydrotestosterone and 17-beta-estradiol group a four-fold increase of the stromal part. The synergistic effect of estrogens and androgens on the growth of both parts of the dog prostate--the glandular and stromal compartment--is discussed.

Comparative light and electron microscopic study of the human, dog and rat prostate. An approach to an experimental model for human benign prostatic hyperplasia (light and electron microscopic analysis)--a review.

Stereology was shown in several studies to be a relevant method to study the quantitative amount of tissue and cell compartments of normal and pathological altered prostatic tissue. A morphological comparison of the rat (ventral lobe), dog and human prostate was performed by quantitative morphology. In comparing the light microscopic analysis of the rat ventral prostatic lobe with that of the dog prostate, the volumetric amount of the glandular cells in the dog prostate is two times greater than in the rat (ventral prostatic lobe). In the normal human prostate the volumetric amount of the glandular part was calculated to be 55% of the outer part, respectively 45% in the inner part of the prostate. Regarding the stromal tissue, there is no difference between the dog and the normal human prostate, but a striking difference compared to the rat ventral prostatic lobe, where stromal development seems to be sparse (25%). The quantitative measurements of spontaneous dog and human prostate hyperplasia show that the dog hyperplasia is primarily a glandular disease whereas human benign prostatic hyperplasia reflects more stromal activation. As shown by the electron microscopic measurements in the normal human and dog prostate, there is a similar great volumetric amount of secretory granules. In comparing the data of the rough endoplasmic reticulum, in the rat there is a higher amount than in the normal human and dog prostate. Conversely, the amount of secretory granules is higher in the dog and human prostate than in the rat (ventral prostatic lobe).

Pregnancies associated with sperm concentrations below 10 million/ml in clinical studies of a potential male contraceptive method, monthly depot medroxyprogesterone acetate and testosterone esters.

A potential male contraceptive approach was evaluated in clinical trials involving monthly injections of depot medroxyprogesterone acetate and either subdermal implants of testosterone propionate or monthly injections of testosterone enanthate. Pregnancies occurred in partners of 9 men with recent sperm counts of 10 million/ml or below. In 5 of the 9 instances, the sperm counts were less than 1 million/ml. It appears that male contraceptive methods involving spermatogenic suppression may require attainment and maintenance of azoospermia. The pregnancy rate cannot be calculated, because the extent of other contraceptive use is uncertain. There were no spontaneous abortions. 6 pregnancies were carried to term, and all progeny were normal, based on physical examination at birth or 3 months after birth.

Electron microscopic stereological analysis of the normal human prostate and of benign prostatic hyperplasia.

Normal and benign hyperplastic prostatic tissue was studied by quantitative electron microscopic measurements. Quantitative morphological procedures provide values for volume, surface, number of tissue and cellular components within human prostatic tissue. When a comparison is made of the stereological data of the glandular cell of benign prostatic hyperplasia to that of the normal human prostate no statistically significant difference in the relative volumetric amount of the cell organelles is indicated. An attempt was made to characterize the fine structure of the smooth muscle cells of the stromal area (fibromuscular) in normal and benign hyperplastic prostatic tissue. In benign prostatic hyperplasia a significant increase in the relative amount of organelles in the smooth muscle cell was found, indicating an activation of these cells. Light microscopic analysis has revealed that benign prostatic hyperplasia is primarily a stromal disease.

Light microscopic stereological analysis of the normal human prostate and of benign prostatic hyperplasia.

Normal prostate glands from 6 men less than 30 years old and enucleated tissue of benign prostatic hyperplasia were analyzed by stereological methods. Studies on the relative volumetric amount of fibromuscular (stromal) and glandular areas of the gland reveal no statistically significant difference between the inner and outer parts of the normal prostate and between the inner part of the normal prostate and benign prostatic hyperplasia. However, in benign prostatic hyperplasia there is a significant increase in the volumetric amount of the fibromuscular tissue and a decrease in the glandular area compared to the outer part of the normal prostate and the whole normal prostate (sum of the inner and the outer parts). These stereological data are discussed with respect to the pathogenesis of benign prostatic hyperplasia.

[Adrenal scintigraphy-value and indications in adrenal disease].

Adrenal scintillation scanning is a very useful method in the diagnosis of steroid producing diseases of the adrenal glands. The diagnosis of bilateral hyperplasia and primary adenoma of the adrenal cortex in Cushing's syndrome and Conn's syndrome is possible by this non-invasive procedure. Morphological questions (e.g. the localization of a phaeochromocytoma) cannot be answered. It is not a screening examination.
