Lipoprotein(a) plasma levels and apo(a) isoforms are not associated with longevity or disability in a sample of Italian octo-nonagenarians. Associazione Medica Sabin.

Cardiovascular diseases are the leading cause of disability and mortality in western countries. Lipoprotein(a) [Lp(a)] is now considered an independent risk factor for atherosclerosis, and might consequently be related to longevity and/or disability. In the context of a study on metabolic and anthropometric parameters in a sample of Italian octo-nonagenarians, Lp(a) and apo(a) isoforms were evaluated. One-hundred and fifty Italian octo-nonagenarians were classified as free-living or disabled, according to Katz's index, and compared to 91 healthy control adults. All the study subjects were recruited from a valley (Val Vibrata valley) near Teramo, in the central part of Italy. The median Lp(a) concentration of the whole group was 17 mg/dL (range 1-161 mg/dL), which is much higher than the values observed in Caucasian populations. No differences were detected between the octo-nonagenarian group (median 16 mg/dL, range 1-126 mg/dL) and the control group (median 19.5 mg/dL, range 1-161 mg/dL), nor between the free-living and the disabled groups. Apo(a) isoforms were similarly distributed among free-living, disabled and control subjects. While our findings suggest that Lp(a) plasma levels and apo(a) isoforms are not factors associated with longevity or disability, we cannot exclude that the low incidence of other major risk factors for atherosclerosis in our free-living octo-nonagenarians hampered the full expression of the lipoprotein(a) atherogenic potential, and thus allowed the achievement of a very old age in a good healthy status, even in carriers of high Lp(a) levels or small apo(a) isoforms.

The main saponins from the aerial parts and the roots of Solidago virgaurea subsp. virgaurea.

Two new (3, 4) and two known (1, 2) genuine acylated bisdesmosidic triterpenoid glycosides of polygalacic acid (2 beta, 3 beta, 16 alpha, 23-tetrahydroxyolean-12-en-28-oic acid) were isolated from the aerial parts, as well as from the roots, of Solidago virgaurea L. subsp. virgaurea. The structures of these esters were elucidated as 4-O-fucopyranosyl-acylated compounds (1, 2, 3, 4) of 28-[O-alpha-L-rhamnopyranosyl-(1-->3)-O-beta-D-xylopyranosyl- (1-->4)-O-alpha-L-rhamnopyranosyl-(1-->2)-beta-D-fucopyranosyl]-glycosid es of 3-O-[beta-D-glucopyranosyl]-polygalacic acid (5), and 3-O-[O-beta-D-glucopyranosyl-(1-->3)-beta-D-glucopyranosyl]- polygalacic acid (6).

Safety and immunogenicity of two acellular pertussis vaccines with different pertussis toxoid and filamentous hemagglutinin content in infants 2-6 months old.

The optimal composition and antigen content of acellular pertussis vaccines is not known. Two vaccines with different quantities of pertussis toxoid (10 and 20 micrograms) and filamentous hemagglutinin (5 and 20 micrograms) and identical 69 kD protein (3 micrograms) and fimbriae 2 and 3 (5 micrograms) combined with diphtheria and tetanus toxoids were compared in a randomized, double-blind study in 2,050 infants undergoing their primary immunization series at 8 centers in the US and Canada. A 6:1 increased antigen to lower antigen allocation was used; 96% of infants received 3 doses and completed the study. A 'clinically significant' local reaction was reported in 3-6% of participants after each dose. Erythema was the most common reaction occurring in 3-5% of infants after the second or third dose. A clinically significant systemic adverse reaction was reported in 28-34% of vaccinees (or vaccinated children) after each dose; fever (7-18%) and fussiness (12-17%) were most common. There were no differences in adverse events between the 2 vaccine formulations. Antibody responses were measured in 292 infants at 1 center. At 7 months, geometric mean anti-filamentous hemagglutinin antibody titers were higher in recipients of the higher antigen content vaccine (p < 0.001) whereas recipients of the lower antigen content formulation had higher anti-fimbriae antibody (p < 0.001) and agglutinin titers (p < 0.05). No differences were detected in anti-pertussis toxin or other antibody responses between the formulations. We conclude that increasing the antigen content of the acellular pertussis vaccine had a variable effect on antibody response but was not associated with increased adverse reactions.

The importance of the highest normokalemia in the treatment of early hepatic encephalopathy.

An inverse relation is known to link blood potassium with renal synthesis and the release of ammonia. Given the liability of hyperammonemia for precipitating hepatic encephalopathy (HE), 28 patients affected by stage I HE were equally divided into two groups and maintained up to their death at the highest (5.4-5.5 mEq/l) or the lowest (3.5-3.6 mEq/l) normokalemia levels. When compared with the lowest normokalemia group, the highest one showed an early, albeit transient, improvement in the mental state (as assessed by both EEG and psychiatric investigations) and to a lesser extent in hepatic functions (as assessed by the variations in serum bilirubin, GPT, GGT and plasma prothrombin time). In the highest normokalemia group the survival was also prolonged. The cause of this improvement may be related to the induced decrease in blood pH, the consequent depression of renal ammoniagenesis and the rise in the arterial and urine NH+4/NH3 ratios. These factors reduce the entry of ammonia into the cells and enhance the urinary excretion of this metabolite, respectively.

Virgaureasaponin 3, a 3,28-bisdesmosidic triterpenoid saponin from Solidago virgaurea.

A new 3,28-bisdesmosidic triterpenoid glycoside was isolated from the mixture of deacylated saponins from the aerial parts of Solidago virgaurea. The structure of virgaureasaponin 3 was determined as 3-O-beta-D-glucopyranosyl-(1----3)-beta-D-glucopyranosylpolygalacic++ + acid 28-O-beta-D-fucopyranosyl-(1----2)-alpha-L-rhamnopyranosyl-(1----3)-beta -D- xylopyranosyl-(1----4)-alpha-L-rhamnopyranosyl-(1----2)-beta-D-fucopyran oside mainly by various 2D NMR techniques.

[New triterpenoids saponins from Helianthus annuus].

Three new bisdesmosidic triterpenoid saponins, helianthoside 1(1), 2(2), and 3(3), were isolated from the flowers of Helianthus annuus L. and a new monodesmoside 4 was isolated after the cleavage of the ester-glycosidic linkages of 2 and 3. The structures of the compounds were elucidated by 13C-NMR, FAB-MS, GC/EI-MS of partially methylated alditol acetates and degradation methods.

[The antifungal action of polygalacic acid glycosides].

Triterpenoid glycosides obtained from Solidago virgaurea L. and Bellis perennis L. (Asteraceae) inhibit the growth of human-pathogenic yeasts (Candida and Cryptococcus species). First results are given of the investigations on the mode of action and the relationships between structure and activity of these compounds. The intensity of growth inhibition is influenced particularly by the carbohydrate chains of the glycosides. Monodesmosidic as well as bisdesmosidic glycosides of polygalacic acid exert fungicidic effects.

Absolute scattering probabilities for subexcitation electrons in condensed H2O.

Absolute elastic and inelastic collision probabilities per unit length for subexcitation electrons scattering in condensed water were determined by analysis of electron energy loss and transmission data for electron energies between the vacuum level (zero incident kinetic energy) and 3.2 eV. Highly disordered solid films were deposited on a metal substrate with thickness varying between 1 and 50 layers. Analysis by an N-channel model of hot electron transport provides values of 0.017 and 0.068 per layer for the elastic and total inelastic collision probabilities, respectively.
