Out-of-pocket medical spending for care of chronic conditions.

We examined out-of-pocket medical spending by persons with and without chronic conditions using data from the 1996 Medical Expenditure Panel Survey (MEPS). Our results show that mean out-of-pocket spending increased with the number of chronic conditions. The level of this spending also varied by age and insurance coverage, among other characteristics. Out-of-pocket spending for prescription drugs was substantial for both elderly and nonelderly persons with chronic conditions. As policymakers continue to use cost sharing and design of benefit packages to contain health spending, it is important to consider the impact of these policies on persons with chronic conditions and their families.

Social competence and behavioural problems in children with hearing impairment.

The focus in this study is on social competence and behavioural problems in a group (n=57) of hearing-impaired 7- to 12-year-old children. Results were related to hearing status, school placement, and gender. Parent and teacher questionnaires were used to measure two aspects of social competence, prosocial orientation and social initiative, and three aspects of behavioural problems, externalizing, internalizing, and concentration problems. A sample of 214 normally-hearing children was used as a comparison group. Overall, the results showed only minor differences between the two groups, with the exception that the hearing-impaired children showed less social initiative in parent ratings. There were no relationships between social functioning and hearing status or school placement (mainstream vs. special classes for hearing-impaired children) nor any clear pattern of gender differences. These findings indicate that hearing-impaired children in Sweden fare well. The results are discussed in terms of the need for longitudinal studies and possible interventions targeted at social behaviours.

Maxillary adenoid cystic carcinoma with annulate lamellae.

Of 29 reviewed ultrastructural studies of adenoid cystic carcinoma, only 1 mentions the presence of annulate lamellae. The present report describes a 65-year-old woman who presented with an adenoid cystic carcinoma in her left maxillary sinus. At the light microscopic level, most of the connective tissue was loose and myxomatous and contained fine strands, cords, and islands of epithelium that in places had a cribriform pattern. The ultrastructural appearance was similar to that described by others. Two epithelial cell types predominated. The cytoplasm of one type contained mainly ribosomes, and that of the other type also had peripheral microfilaments. Also present were pseudocysts, lumina with microvilli, and replicated basal lamina. In addition, a number of both types of cells contained parallel arrays of cisternae that had regular small pores. These were annulate lamellae.

Continuing education for nurses that incorporates genetics.

BACKGROUND: The discoveries of the Human Genome Project are bringing new knowledge of genetics to health and illness arenas. The nursing profession will need to incorporate these new advances into clinical practice, research, and education. METHOD: A survey of 68 nursing specialty organizations was conducted to determine current and future offerings in genetics to members. RESULTS: Responses from 43 organizations' top administrators who are influential in decisions regarding continuing education offerings revealed that 30% were planning to offer content in genetics in future programming. In contrast, none of the respondents had plans to present consumer perspectives of genetic illnesses. CONCLUSION: The results of this survey alert leaders in continuing education to the urgent need for dissemination of genetic advances to inform nurses in basic and advanced roles in health care delivery.

Nursing education and genetics. Miles to go before we sleep.

The discoveries of the Human Genome Project (HGP), established in 1990 at the National Institutes of Health and the United States Department of Energy, are bringing important new technologies for genetic diagnosis and treatment to nearly all areas of health care delivery. Nurses, who play an integral role in supporting health consumers as they respond to health and illness, require up-to-date genetic knowledge for conducting clinical practice, engaging in nursing research, and educating a new generation of nurses.

Immunoglobulin-producing cells in the human thymus.

Ninety-nine human thymuses were examined by the immunoperoxidase technique to determine the number and distribution of cells containing cytoplasmic immunoglobulin (Ig). Such cells were found in greatest concentration in the medulla and least in the collagenous interlobular septa with intermediate concentrations in the cortex and paraseptal region. In the adult, IgG is detected most frequently with lesser amounts of IgA, and IgM least commonly. In the fetus, significant numbers of immunoglobulin-containing cells are present with a similar distribution to that seen in the adult and with IgG predominating. However IgM is demonstrated more frequently than IgA. There is a tendency for the number of Ig-containing cells to rise after birth and to continue into early adult life. Few are seen after the age of 20. There is also a tendency for the numbers to rise during the process of involution but few are seen in completely involuted thymuses. No IgD is demonstrated and IgE is present only on the surface of mast cells. Lambda light chain is present in greater quantities than kappa light chain. Although Ig-containing cells represent only a minor population, their presence must be taken into account when considering the function of the thymus and the mechanism of involution.
