Occupational history and exposure and the risk of adult leukemia in Shanghai.

PURPOSE: Evaluation of the association of selected occupational exposures with leukemia risk. METHODS: Population-based case-control study of 486 leukemia subjects and 502 healthy controls residing in Shanghai from 1987 to 1989. Adjusted odds ratios (OR) were calculated for the association between occupational factors and leukemia risk. RESULTS: Significant increase in leukemia risk was observed in chemical manufacturing industry workers (OR=3.1, 95% CI=1.0-9.8). Increased risks for leukemia were observed from self-reported exposures to benzene (OR=1.7, 95% CI=1.1-2.6), radioactive materials (OR=3.7, 95% CI=1.3-10.2), synthetic fiber dust (OR=2.0, 95% CI=1.2-3.5), and toluene (OR=1.6, 95% CI=1.0-2.5). Dose-response relations of leukemia risk was observed with the duration of exposure to benzene (OR=3.3, 95% CI=1.6-6.9 for >or=15 years exposure; p for trend<0.01), radioactive materials (OR=5.2, 95% CI=1.1-24.7 for >or=15 years exposure; p for trend=0.02), paints (OR=2.3, 95% CI=1.2-4.7 for >or=15 years exposure; p for trend=0.09), and toluene (OR=2.9, 95% CI=1.3-6.7 for >or=15 years exposure; p for trend=0.02). CONCLUSIONS: Adult leukemia risk may be associated with working in the chemical industry, and exposure to benzene, synthetic fiber dust, radioactive materials, and toluene in the study population.

A pooled analysis of case-control studies of thyroid cancer: cigarette smoking and consumption of alcohol, coffee, and tea.

OBJECTIVE: To analyze the role of smoking, alcohol, coffee and tea in relation to thyroid cancer, we conducted a pooled analysis of 14 case-control studies conducted in the United States, Europe, and Asia. METHODS: The sample consisted of 2725 thyroid cancer cases (2247 females, 478 males) and 4776 controls (3699 females, 1077 males). Conditional logistic regression with stratification on study, age at diagnosis, and gender was used to compute odds ratios and 95% confidence intervals. RESULTS: Thyroid cancer risk was reduced in persons who had ever smoked. The relationship was more pronounced in current smokers (OR = 0.6, 95% CI = 0.6-0.7) than former smokers (OR = 0.9, 95% CI = 0.8-1.1). There were significant trends of reduced risk with greater duration and frequency of smoking. For consumption of wine and beer, there was a significant trend of decreasing thyroid cancer risk (p = 0.02) that was not maintained after adjustment for current smoking (p = 0.12). Thyroid cancer risk was not associated with consumption of coffee or tea. These findings were consistent in both gender-specific and histology-specific (papillary and follicular) analyses. CONCLUSIONS: Pooled analyses of these geographically diverse case-control data indicate a reduced thyroid cancer risk associated with current smoking. A reduced risk associated with alcohol was eliminated after adjustment for smoking, and caffeinated beverages did not alter thyroid cancer risk.

Soy food consumption is associated with lower risk of coronary heart disease in Chinese women.

Soy food intake has been shown to have beneficial effects on cardiovascular disease risk factors. Data directly linking soy food intake to clinical outcomes of cardiovascular disease, however, are sparse. We examined the relationship between soy food intake and incidence of coronary heart disease (CHD) among participants in the Shanghai Women's Health Study, a population-based prospective cohort study of approximately 75000 Chinese women aged 40-70 y at the baseline survey that was conducted from 1997 to 2000. Included in this study were 64915 women without previously diagnosed CHD, stroke, cancer and diabetes at baseline. Information on usual intake of soy foods was obtained at baseline through an in-person interview using a validated food-frequency questionnaire. Cohort members were followed biennially through in-person interviews. After a mean of 2.5 y (162277 person-years) of follow-up, 62 incident cases of CHD (43 nonfatal myocardial infarctions and 19 CHD deaths) were documented. There was a clear monotonic dose-response relationship between soy food intake and risk of total CHD (P for trend = 0.003) with an adjusted relative risk (RR) of 0.25 (95% CI, 0.10-0.63) observed for women in the highest vs. the lowest quartile of total soy protein intake. The inverse association was more pronounced for nonfatal myocardial infarction (RR = 0.14; 95% CI, 0.04-0.48 for the highest vs. the lowest quartile of intake; P for trend = 0.001). This study provides, for the first time, direct evidence that soy food consumption may reduce the risk of CHD in women.

Dietary calcium intake and breast cancer risk among Chinese women in Shanghai.

Limited epidemiological evidence suggests that calcium intake may be related to breast cancer risk. Data from a large, population-based, case-control study (n = 1,459 cases, 1,556 controls) conducted in Shanghai, China, between 1996 and 1998 were used to investigate the association between calcium intake and risk for breast cancer. Diet was assessed using a quantitative food-frequency questionnaire. The multivariate-adjusted odds ratio (OR) comparing all women combined in the highest to lowest deciles of total calcium intake was 0.74 (95% confidence interval [CI] = 0.46-1.20). Whereas calcium primarily derived from poultry was inversely associated with risk for breast cancer (comparing the highest to lowest quintile OR = 0.71, 95% CI = 0.55-0.93) with a statistically significant test for trend, calcium derived from milk, seafood, fruit, and vegetables was not associated with risk of breast cancer. Given that breast cancer is one of the top contributors to cancer incidence worldwide, even a moderate inverse association between calcium and breast cancer risk, if confirmed, could have important public health implications in breast cancer prevention.

Urinary phytoestrogen excretion and breast cancer risk: evaluating potential effect modifiers endogenous estrogens and anthropometrics.

Steroid sex hormones play a central role in breast carcinogenesis. Evidence from in vitro and animal studies suggests that phytoestrogens may inhibit the development of mammary tumors through their role in regulating the synthesis, metabolism, and signal transduction of steroid hormones. In a study of 117 case-control pairs of postmenopausal women in Shanghai, we investigated whether the association between urinary phytoestrogen excretion and breast cancer risk may differ by levels of endogenous steroid sex hormones, sex hormone binding globulin (SHBG), body mass index (BMI), and waist:hip ratio (WHR). Fasting morning blood and urine samples were collected for the analysis of urinary isoflavonoids and mammalian lignans, as well as blood levels of SHBG and selected steroid hormones. For cancer patients, samples were collected before any cancer therapy. Conditional logistic regression models were used to estimate odds ratios and 95% confidence intervals after adjusting for potential confounding factors. The inverse associations between urinary phytoestrogens and breast cancer risk were found to be more evident among women with a high BMI or WHR than those with a low level of these anthropometric measurements. Although a reduced risk of breast cancer was observed among women with a high excretion rate of urinary isoflavonoids in all of the strata defined by blood SHBG and steroid hormones, the inverse association was more pronounced among women with a high blood concentration of estradiol, a low level of estrone sulfate, or a low level of SHBG. The risks of breast cancer were also reduced with increasing excretion rate of mammalian lignans, although no test for a linear association was statistically significant in stratified analyses. Findings from this study suggest that the potential protective association of phytoestrogens may be modified by BMI, WHR, and blood levels of SHBG, and steroid hormones.

Genetic polymorphisms in the estrogen receptor alpha gene and risk of breast cancer: results from the Shanghai Breast Cancer Study.

Estrogen receptor (ER) is a ligand-activated transcription factor that mediates estrogen actions in target tissues. Several common polymorphisms of the ER-alpha gene have been reported to be associated with alterations in receptor expression and function. We evaluated the hypothesis that genetic polymorphisms in the ER-alpha gene may be associated with breast cancer risk in a population-based case-control study conducted in urban Shanghai during 1996-1998. Two RFLPs at the ER-alpha gene locus, denoted as PvuII and XbaI, were examined in 1069 breast cancer cases and 1166 age frequency-matched controls. PvuII polymorphism was associated with an increased risk of breast cancer with the age-adjusted odds ratios for genotypes Pp and pp being 1.3 [95% confidence interval (CI), 1.0-1.7) and 1.4 (95% CI, 1.1-1.8), respectively, comparing to genotype PP. The XbaI polymorphism was associated with a nonsignificantly elevated risk. The odds ratios for genotypes Xx and xx were 1.2 (95% CI, 0.7-1.9) and 1.3 (95% CI, 0.8-2.0), respectively, and the elevated risks were mainly confined to older or postmenopausal women. No apparent synergetic effect of these two polymorphisms was identified. Results of this study indicate that genetic polymorphisms in the ER-alpha gene may play a role in the etiology of breast cancer.

Joint effect of insulin-like growth factors and sex steroids on breast cancer risk.

Insulin-like growth factors (IGFs) and estrogens are essential hormones regulating the growth and differentiation of mammary cells. Studies have shown that IGFs and estrogens are strong mitogens for breast cancer cells and that high circulating IGF-I and estrogens are risk factors of breast cancer. Laboratory experiments further indicate that these hormones act synergistically on the pathogenesis of breast cancer. Estrogens increase the effect of IGF-I on breast cancer cells by stimulating the expression of IGF-I and IGF-I receptor; IGFs enhance the action of estrogens by regulating the production of estrogen receptor. Furthermore, the two systems have substantial signal transduction cross-talk. Despite the laboratory indications, there is no epidemiological evidence in support of the joint effect of IGFs and estrogens on breast cancer risk. To test this hypothesis in human populations, we compared plasma levels of IGFs and sex steroid hormones among 300 incident breast cancer patients and 300 age- and menopause-matched control women derived from a large population-based case-control study conducted in Shanghai, China between 1996 and 1998. Fasting morning blood samples were measured for plasma concentrations of IGF-I, IGF-II, IGF-binding protein (IGFBP)-3, estradiol, estrone, estrone sulfate, testosterone, and dehydroepiandrosterone sulfate using commercial immunoassay kits. Levels of these hormones were classified into two groups based on the median levels in the control group and combined between IGFs and steroid hormones. Conditional logistic regression analysis was performed to examine the association of breast cancer risk with the combined variables of sex steroids and IGFs. A synergistic effect on breast cancer risk was suggested for IGF-I or IGFBP-3 with estrone or testosterone among both pre- and postmenopausal women. Compared with premenopausal women with low circulating levels of both IGF-I and estrone, the odds ratios were 1.21 [95% confidence interval (CI), 0.61-2.40] for high estrone, 1.50 (95% CI, 0.77-2.93) for high IGF-I, and 2.30 (95% CI, 1.21-4.37) for both high estrone and high IGF-I. Similar patterns of association were also seen for IGF-I with testosterone as well as for IGFBP-3 with estrone or testosterone, and some of the interactions were statistically significant or borderline significant (P < 0.10). No joint effects were found for IGF-I or IGFBP-3 with estradiol or estrone sulfate. In conclusion, the study suggests a possible synergy between IGF peptide hormone and sex steroid hormones, including both estrogen and androgen, in relation to breast cancer risk.

A population-based case-control study of the Arg399Gln polymorphism in DNA repair gene XRCC1 and risk of breast cancer.

XRCC1 (X-ray repair cross-complementing group 1) is a base excision repair protein that plays a central role in the repair of DNA base damage and strand breaks. A common polymorphism (Arg-->Gln) at codon 399 of the XRCC1 gene has been previously linked to functional changes of the gene product and risk of cancers. We evaluated the association between XRCC1 Arg399Gln polymorphism and breast cancer risk in the population-based Shanghai Breast Cancer Study involving 1088 cancer patients and 1182 community controls. Genomic DNA from peripheral blood was used in genotyping assays, and exposure information and anthropometrics were collected through in-person interview. Plasma estrogen and sex hormone-binding globulin (SHBG) levels were measured for 190 postmenopausal breast cancer patients who had donated a pretreatment blood sample and 407 postmenopausal controls. Conditional logistic regression model was used to estimate odds ratios (ORs) and 95% confidence intervals (95% CIs) adjusting potential confounders. Approximately 27% of controls carried the variant allele (Gln), and cases and controls had a similar distribution for both allele type and genotype of this polymorphism. We found that 7.8% of cases and 6.3% of controls were homozygous for the variant allele, resulting in an OR of 1.20 (95% CI, 0.85-1.69). The OR was slightly higher among younger women [<45 years of age (OR, 1.39; 95% CI, 0.82-2.36)] than older women [> or = 45 years of age (OR, 1.07; 95% CI, 0.68-1.67)], but neither OR was statistically significant. No modifying effect of major breast cancer risk factors, including years of menstruation, body mass index, waist:hip ratio, and blood estrogen levels, was noted. Homozygosity for the variant Gln allele was associated with an elevated risk of postmenopausal breast cancer among subjects with a higher blood level of SHBG (OR, 3.27; 95% CI, 1.16-9.20) and a reduced risk among those with a lower level of SHBG (OR, 0.60; 95% CI, 0.18-1.97). The overall results of the study suggest that Arg399Gln polymorphism of the XRCC1 gene alone may not play a substantial role in the risk of breast cancer among Chinese women.

Occupations and breast cancer risk among Chinese women in urban Shanghai.

BACKGROUND: Although, an elevated risk of breast cancer has been suggested for a number of occupations, many earlier studies were limited by selection biases, the incomplete assessment of job histories, and the inability to control for confounding. METHODS: We examined the relationship between occupational history and breast cancer risk using data from a population-based case-control study of 1,458 cases and 1,556 age-matched controls (90% response rate) conducted in Shanghai, China. Unconditional logistic regression models were used to derive odds ratios (ORs) and 95% confidence intervals (95% CIs) of breast cancer risk associated with occupations and duration of employment adjusting for non-occupational risk factors. RESULTS: The following occupations were found to be associated with an increased risk of breast cancer: laboratory technicians (OR 9.94, 95% CI 1.20-82.37), telephone and telegraph operators (OR 4.63, 95% CI 1.85-11.59), leather and fur processors (OR 3.25, 95% CI 1.11-9.53), and glass-manufacturing workers (OR 2.08, 95% CI 1.14-3.82). A dose-response pattern for years of employment was observed for leather and fur processors (P = 0.02) and glass-manufacturing workers (P = 0.01). Stratified analyses also revealed dose-response relationships between the risk of breast cancer and years of employment as inspector and product analysts among pre-menopausal women (P = 0.02), and as farmers among post-menopausal women (P = 0.04). CONCLUSIONS: This study found that several occupations are associated with an increased risk of breast cancer among women. Studies examining various occupational exposures in these high-risk occupations are warranted to identify carcinogens that may play a role in the increased breast cancer risk.

Risk factors for medullary thyroid carcinoma: a pooled analysis.

OBJECTIVE: To investigate risk factors for medullary thyroid cancer (MTC). METHODS: We conducted a pooled analysis of 14 case-control studies from Europe, North America, and Asia, including 67 medullary cancers (43 women and 24 men) diagnosed in ten studies. Of the original 4776, we selected five controls per case matched on study, gender, and age. The pooled odds ratios (OR) were estimated using conditional logistic regression. RESULTS: Education, weight, and body mass were not associated with MTC, but a significant positive relationship was seen with height (OR = 2.6 for highest vs lowest tertile). Significant excess risks were associated with a history of thyroid nodules (OR = 12), hypertension (OR = 2.3), gallbladder disease (OR = 4.3), and allergies (OR = 2.2). Among current smokers, a decreased risk of MTC was observed with increasing number of cigarettes. The risk was significantly elevated among women having a first birth after age 25 years, but no clear pattern emerged for other reproductive factors. CONCLUSIONS: Although the number of MTC was small, we detected several significant associations, including prior thyroid and other diseases.

A pooled analysis of case-control studies of thyroid cancer. VII. Cruciferous and other vegetables (International).

OBJECTIVE: To investigate the association between cruciferous and other vegetables and thyroid cancer risk we systematically reanalyzed the original data from 11 case-control studies conducted in the US, Asia, and Europe. METHODS: A total of 2241 cases (1784 women, 457 men) and 3716 controls (2744 women, 972 men) were included. Odds ratios (OR) and the corresponding 95% confidence intervals (CI) were estimated for each study by logistic regression models, conditioned on age and sex, and adjusted for history of goiter, thyroid nodules or adenomas, and radiation. Summary ORs for all studies combined were computed as the weighted average of the estimates from each study. RESULTS: A decreased risk for the highest level of cruciferous vegetable intake, as compared to the lowest, was observed in Los Angeles, Hawaii, Connecticut, southeastern Sweden, Tromsø, and Switzerland; the OR were above unity in Japan and Uppsala, whereas no material association was found in northern Sweden, Italy, or Greece. The OR values for all studies combined were 0.87 (95% CI 0.75-1.01) for moderate and 0.94 (95% CI 0.80-1.10) for high cruciferous vegetables intake. The results were similar in studies from iodine-rich areas and endemic goiter areas, and were consistent when the analysis was restricted to papillary carcinomas and women. The summary OR values for vegetables other than cruciferous were 1.04 (0.88-1.22) for moderate and 0.82 (0.69-0.98) for high consumption. CONCLUSIONS: This combined analysis indicates that cruciferous vegetables are not positively related to thyroid cancer risk. Their effect does not seem to be substantially different from that of other vegetables, which appear to be protective on this cancer.

Birthweight, childhood growth and hypertension in adulthood.

BACKGROUND: Low birthweight (BW) and childhood growth have been hypothesized to be associated with an increased risk of hypertension in later life. METHODS: We analysed data among 13,467 women with a recalled BW from the Shanghai Women's Health Study. Cases included those with a self-reported hypertension with ('confirmed cases') or without ('possible cases') antihypertensive medication(s) use. Logistic regression was used to derive adjusted odds ratios (OR) and 95% CI. RESULTS: Birthweight was inversely associated with the odds of early onset (at age 20-40 years) hypertension in a dose response manner (P for trend = 0.01). This association is stronger for 'confirmed' hypertension (only OR for 'confirmed' hypertension are referred to subsequently). Being heavier or taller than average at 15 years of age were both related to elevated odds of early onset hypertension. Women who had a low BW but were heavier than average at age 15 were more than four times (OR = 4.63, 95% CI: 2.40-8.94) more likely to have an early onset hypertension, and those who had a low BW and became taller at 15 years of age had an OR of 1.87 (95% CI: 1.05-3.31). A significant interaction between BW and weight at age 15 was observed (P = 0.04). CONCLUSION: Our study suggests that low BW, particularly if accompanied by accelerated childhood growth, may increase the risk of early onset hypertension in adulthood.

[Detection of multiple loci in single cell by primer extension preamplification and nest PCR]

OBJECTIVE: To investigate the feasibility of multiple loci detection in single cell by primer extension preamplification (PEP) followed by nest PCR. METHODS: Using PEP, the whole genomic DNA in single lymphocyte or single blastomere was amplified. In addition, CD17, nt-28 and linked ATTTT repeat for beta-thalassemia, F508 and linked GATT repeat for cystic fibrosis, DMD exon 17 and 48 for Duchenne muscular dystrophy, short tandem repeats of D18S51, D21S11 and D21S1411, and sex-determination gene SRY of the Y chromosome were all detected using nest-PCR from a small aliquot of the PEP reaction. RESULTS: The rate of successful single lymphocyte amplification was 89.5%(false positive 0.48%false negative 2.5%). The rate of successful single blastomere amplification was 85.56%(false positive 3%). CONCLUSION: The PEP technique followed by nest PCR analysis of single cell is very useful for simultaneous detection of multiple gene loci. It may be applicable for preimplantation genetic diagnosis.

[Follicle-stimulating hormone receptor levels in relation to ovarian cycling response from gonadotropin hormone stimulation]

OBJECTIVE: To investigate the relationship between the ovarian response, induced by gonadotropin hormone, and levels of follicle-stimulating hormone receptor (FSHR). METHODS: The level of ovarian response was divided into three groups based on the number of follicles obtained during oocyte retrieval: poor responders (<4 follicles), normal responders (4 approximate, equals 13) and high responders(>13 follicles). FSHR mRNA was measured using reverse transcription polymerase chain reaction (RT-PCR). RESULTS: The expression of FSHR mRNA in the poor responders was 0.54+/-0.07. This was much lower than that of the normal responders who were at 0.90+/-0.17,and the high responders who were at 1.20+/-0.45(P<0.05). CONCLUSION: Ovarian response induced by gonadotropin hormone stimulation is correlated with the level of FSHR mRNA in the granulosa cells.

[Correlation between polyspermy and the outcome of in vitro fertilization-embryo transfer]

OBJECTIVE: To explore the influence of polyspermy on IVF outcomes in in vitro fertilization and embryo transfer(IVF-ET). METHODS: The data from 496 IVF-ET cycles and 5349 oocytes were analyzed retrospectively. A comparison of a number of fertility parameters with and without polyspermy was done. The fertility parameters were the number of oocytes retrieved, percentage of mature oocytes, fertilization rate, cleavage rate, occytes for ET, pregnancy rate. RESULTS: The percentage of mature occytes, fertilization rate, cleavage rate was 67.0 %,76.7 %and 95.6 %, respectively( P< 0.01). The pregnancy rate was higher in polyspermic fertilization cycles (25.7 %) than in cycles without polyspermy(23.6 %),but with no statistical significance ( P>0.05). CONCLUSION: Polyspermic fertilization is correlated with improved oocyte receptibility to sperm and could be considered as an encouraging sign for the success of IVF.

Insulin-like growth factors and breast cancer risk in Chinese women.

Insulin-like growth factor (IGF)-I has mitogenic and antiapoptotic effects on breast cancer cells. High-circulating IGF-I was found to be associated with increased risk of breast cancer in several previous epidemiological studies, mostly conducted in the Caucasian populations. Little is known about the association between IGF and breast cancer in Asian women whose dietary habits differ considerably from their Caucasian counterparts. A population-based case-control study was conducted to assess the associations of IGFs and IGF binding protein-3, a major IGF binding protein in the circulation, with breast cancer risk in Chinese women. The study included 300 incident breast cancer patients diagnosed between August 1996 and March 1998 in Shanghai and 300 age- and menopause-matched controls selected randomly from the general population. Plasma levels of IGF-I, IGF-II, and IGFBP-3 were measured using commercial ELISA kits (Diagnostic Systems Laboratories, Webster, TX). Conditional logistic regression analysis was performed to examine the association between IGF and breast cancer risk after adjusting for potential confounding factors. Breast cancer patients had higher plasma levels of IGF-I and IGFBP-3. A dose-response relationship was observed between breast cancer risk and the level of IGF-I or IGFBP-3. The adjusted odds ratios were 2.01 (95% confidence interval, 1.26-3.19) or 3.01 (95% confidence interval, 1.81-4.99), respectively, for women with the highest tertile of IGF-I or IGFBP-3 compared with those with the lowest tertile of these molecules. These associations were more evident in premenopausal women or women with high body mass index or high waist-to-hip ratio. No significant association was found for IGF-II. The study confirms that high circulating levels of IGF-I are associated with elevated risk of breast cancer. In contrast to the findings from several studies conducted in Caucasian women, we found that IGFBP-3 was positively associated with breast cancer risk in Chinese women.

Consumption of animal foods, cooking methods, and risk of breast cancer.

Cumulative evidence suggests a possible interaction of cooking methods with diet in the pathogenesis of breast cancer. Studies, however, are few and inconsistent. We evaluated the association of animal food intake and degree of browning by deep-frying with breast cancer risk in a population-based case-control study conducted during 1996-1998 among Chinese women in Shanghai, a population with a traditionally low risk of breast cancer. Included in the study were 1459 cases and 1556 age-frequency-matched controls with response rates of 91.1 and 90.3%, respectively. A validated food frequency questionnaire was used to obtain information on usual intake of animal foods and cooking oils and usual cooking methods. Increasing intake of red meat and freshwater fish was related to a moderately elevated risk of breast cancer risk. Stratified analyses showed that the positive association with red meat intake was primarily restricted to those who used deep-frying cooking method, particularly among those who deep-fried foods to well-done (odds ratio, 1.92; 95% confidence interval, 1.30-2.83 for the highest versus the lowest quintile; P for trend, 0.002). On the other hand, high intake of nonhydrogenated soybean cooking oil was related to a reduced risk of breast cancer among women who never deep-fried animal foods (odds ratio, 0.48; 95% confidence interval, 0.28-0.82 for the highest versus the lowest quintile; P for trend, 0.02). The positive association of breast cancer risk with red meat intake, especially well-done red meat, was more pronounced among women with a high body mass index than those without this risk factor, and the test for multiplicative interaction was statistically significant. This study suggests that high intake of deep-fried, well-done red meat may be associated with an increased risk of breast cancer, and the positive association may be modified by body weight. This study also suggests that nonhydrogenated soybean oil, if not used in high-temperature cooking, may be associated with a reduced risk of breast cancer.
