Progress in the cytological diagnosis of gastric lymphoma: a report of 32 cases.

Thirty-two cases of gastric lymphoma have been examined cytologically in our laboratory. In one series (1953-1972), a technique combining washings and abrasion under fluoroscopic control was used. A positive diagnosis of malignancy was made in 7 of 17 cases. In a recent series (1972-1976), 15 cases of gastric lymphoma have been studied by direct abrasion during endoscopy. Malignant cells were identified in 13 cases and a correct diagnosis of lymphoma was made in 11 cases. Biopsy was positive for malignancy in 12 cases and lymphoma was specifically diagnosed in 9 instances. Combining cytology and biopsy, a diagnosis of cancer was made in 14 of 15 cases. Endoscopic cytology appears to be far superior to blind abrasion and washings techniques in the diagnosis of lymphoma of the stomach.

The value of laparoscopy in the diagnosis of primary cancer of the liver.

A retrospective survey organized by the Spanish Association for Digestive Endoscopy among its members has revealed that a correct diagnosis of primary liver cancer could be made by guided biopsy during laparoscopy in 153 of 208 cases. A diagnosis of malignancy was made in 103 of 145 cases of carcinoma associated with cirrhosis and in 50 of 63 cases without cirrhosis, but in the latter, the lesions were endoscopically indistinguishable from metastatic nodules in most instances. Biopsy was positive in 88% of all suspicious cases. Carcinomatous invasion of seemingly benign cirrhotic nodules could be demonstrated in 22 other patients.

[Diagnosis of gastric cancer].

Gastric cancer is still a major cause of death in Europe. Radiologic examination continues to be the diagnostic method of choice, but its accuracy is no more than 50% at the first examination and 75% after repeated procedures. Fiberscopy with biopsy and/or cytology may provide a definitive preoperative diagnosis in as many as 90% of instances, including early cancer cases. With the widespread use of endoscopy it is possible to predict the discovery of one early cancer every 300-400 endoscopies in patients with gastric symptoms. In 5 years, 1,195 early gastric cancers have been found in Europe. These data afford grounds for greater optimism in gastric cancer management.
