Direct invasion of the trachea by an aggressive tumor originating in the thyroid gland is an unusual but well-documented phenomenon. The reverse situation, that is, a primary tracheal neoplasm invading by direct extension into the thyroid gland and presenting as a thyroid mass is apparently an even less common occurrence. A careful search of the literature has failed to reveal any recorded instances of this type of occurrence. We report a case of an adenoid cystic carcinoma arising in the trachea and invading the thyroid gland and recurrent laryngeal nerves causing vocal cord paralysis. The presentation of this lesion as a primary thyroid tumor is an unusual occurrence that serves to remind the clinician that in the differential diagnosis of thyroid masses one must include the possibilities of metastases or direct extension of a primary neoplasm from another organ.
Clinical and pathological aspects of an unusual case of bilateral, multiple glandular cysts of the nasal mucosa, originating from both inferior turbinates, are herewith described, as an occasional finding in rhinoplasty. Although cystic structures are common in the upper respiratory tract, the development of a mucous cyst in the nasal cavity is a very rare occurrence. The differential diagnosis of nasal glandular cysts from other cystic conditions appearing in the nasal cavity is discussed.
Osteomas of the frontal sinus are usually symptomless. When they enlarge they may produce pressure symptoms such as headaches. Small osteomas developing near the fronto-ethmoidal duct may interfere with the patency of the duct and impair the aeration of the sinus. This is a report of a case in which a small osteoma was found to be the cause of severe chronic headaches.

The role of herpes simplex virus (HSV) and varicella zoster virus (VZV) in acute peripheral facial palsy (APFP) was evaluated in 153 patients. The sera of patients were examined for IgG and IgA antibodies to HSV and VZV by the immunoperoxidase antibody-membrane antigen (IPAMA) technique. The prevalence of IgG and IgA seropositivity to HSV was significantly higher in the APFP patients than in the matched control group. No significant difference was found in the geometric mean titer (GMT) to HSV antibodies in the APFP patients compared to the matched control groups. The prevalence of IgA antibodies to VZV was significantly higher in the APFP patients group than in the matched control group. The GMT for VZV IgG antibodies was significantly higher in the APFP patients than in the matched control group. No significant difference was found in the GMT of VZV IgA antibodies. Eleven of 83 patients for whom paired sera were available had significantly increased or decreased IgG and IgA antibody titers to VZV on subsequent examination. Four of these patients did not show any evidence of zosterian eruption. These studies support the concept that VZV and HSV might have a role in the etiopathogenesis of APFP in some of the patients.

A case of a hemangioma in a 56-year-old man is given. The hemangioma interfered with the anterior movement of the condyloid process and caused severe trismus. Removal of the tumor produced immediate relief of the trismus.
Silent mastoiditis is a common complication of acute otitis media since the advent of antibiotics. Although most pediatric middle ear inflammations manifest themselves in both ears, otitic facial palsy almost always presents unilaterally. We report an unusual case of bilateral simultaneous facial palsy occurring during the course of a persistent otitis media in a 7-month-old child. Appropriate antibiotic therapy and insertion of ventilating tubes cleared the infectious inflammatory process, resulting in full recovery from nerve dysfunction.
Two children with subdural empyema of paranasal sinus origin presented as neurological emergencies. Both had severe acute pansinusitis with complicating subdural empyema. The need for a high index of suspicion in the proper clinical setting is mandatory even in the absence of confirmatory contrast studies. Surgical proof should be immediately followed by adequate treatment. The high mortality rate even in the antibiotic area calls for this approach.

A patient who had repeated episodes of bradycardia while on cisplatin therapy is described. Cisplatin-induced bradycardia has not been reported previously. Deposition of cisplatin in the sinoatrial-node area might explain this cardiotoxic effect.
