An unusual case of pseudoaneurysm of the internal carotid artery secondary to tonsillectomy is reported. The management consisted of combined intraoperative transcatheter embolization with surgical ligation of the internal carotid artery.
A case of pseudoaneurysm of the cervical internal carotid artery secondary to tonsillectomy is presented and the pathogenesis of this vascular abnormality is discussed. Due to morphologic and inflammatory changes in the lateral pharyngeal wall, the entrapment aneurysm was maintained. The internal carotid artery was initially occluded above the carotid bifurcation. The transcatheter occlusion of the distal stump of this vessel, by steel coils, replaced a mutilating approach for exposure and ligation near the base of the skull.
Four hundred and twenty seven idiopathic peripheral facial palsy (IPFP) patients admitted to the Otolaryngology Department of the Soroka Medical Center in southern Israel between 1978 and 1982 were characterized and analyzed by age, sex and season of illness. An average annual incidence rate of 33.6 patients per 100,000 inhabitans was calculated which is considered to be the highest incidence reported so far, and may be explained by the very high coverage of the "Sick Fund" insurance in the region and by the warm arid climate that prevails in the area. Knowledge of the demographic characteristics of the patients and the population in the catchment area, allowed the calculation of age-specific incidence rates and it was found that this rate increases with age, reaching a peak of 68.2/100,00 in the older age group (greater than 65 years of age). Peak morbidity in young patients (less than 30 years of age) occurs in the winter while elderly patients lack seasonal variability.
Pachymeningitis may be the presenting symptom of an adjacent paranasal sinus or ear infection. Inflammatory disease of the posterior sinuses (sphenoid and posterior ethmoid cells) may not clinically be apparent and might be overlooked. We report three patients presenting with symptoms and signs of pachymeningitis due to posterior sinus infection. Surgical and/or medical treatment of the local disease results in resolution of the pachymeningitis.
An injury of the inferior alveolar nerve is not an uncommon occurrence. We envisage the physiopathologic process of this involvement. One case of injury of the nerve by endodontic overfilling is presented and analyzed. Prednisone therapy appears to shorten the course of the disease, prevent the secondary fibrosis, and lessen the severity of sequelae.
Two cases of initially undiagnosed silent fractures of the medial wall of the maxillary sinus complicated by a delayed osteomyelitis are presented. The presenting feature was periorbital cellulitis. The etiopathogenesis of this process is based on an interference with the sinus ostium function by ingrowth of hypertrophic scar tissue.
We treated a case of congenital membranous stenosis of the esophagus successfully by transendoscopic incision. We discuss various types of true congenital esophageal stenosis and their treatment, to distinguish these rare anomalies from the more frequently encountered acquired variety.
Local infiltration of adjacent anatomic structures and soft tissues of the neck from well-differentiated carcinoma of the thyroid gland is a relatively infrequent occurrence. We report our experience with 21 such cases seen in our department over the past 20 years. All patients were treated by total thyroidectomy and total or partial excision of the infiltrated adjacent structures. Papillary carcinoma was the most frequent type of primary tumor seen. Following the definitive surgery, all patients were scanned with radioactive iodine (131I). In case of isotope entrapment, a curative dose 131I was given. All patients in our series were required to receive thyroid hormone replacement. Four patients died as a result of their disease. Uncontrolled local disease and distant metastases were present at the time of death. Three patients died of unrelated causes. Two-thirds of the patients are still alive (from 1 to 19 years after the treatment).
Acute suppurative thyroiditis is a rare disease in childhood. The etiology of this condition is not always obvious. The description of a pharyngeal pouch fistula extending from the pyriform sinus to the thyroid gland or perithyroid area, high-lighted the etiopathogenesis of some of the thyroiditis cases. Presented here are the clinical, radiological and pathological aspects of an internal pharyngeal fistula manifesting with recurrent suppurative thyroiditis, until it was completely removed. The clinical features of caudal branchial pouch remnants and their role in the development of acute suppurative thyroiditis is reviewed.

An unusual thyroglossal duct anomaly characterized by a branched tract terminating in two separate cysts is described. The patient presented with a long-standing fistula following previous incomplete excision. Thyroglossal cysts are common congenital anomalies (Moussatos and Baffes, 1963; Allard, 1982), which do not usually present any diagnostic difficulty. However, they may sometimes present with unusual clinico-pathological features. In this report we describe a branching and polycystic thyroglossal duct abnormality occurring in a young woman with a history of previous surgery for a midline cervical mass.
A case of congenital salivary gland tumour occurring in the nasopharynx is reported. Congenital neoplasms of the head and neck (of any histological type) and congenital tumours of the nasopharynx are discussed and the literature is reviewed.
